Registration
The account on file will automatically be charged a non-refundable REGISTRATION FEE upon completion of this enrollment to reserve their spot in the class. Single student registration is $30 and family registration is $55 for families who enroll multiple students. Applies only to students enrolling in a class with space available; does not apply to camp registration; does not apply to students enrolling on a wait list unless a space becomes available and student receives a space in class. Registration for all 2020-2021 dance classes ends on October 31st 2020.
Tuition Policies
All registrants: PRJ requires automatic bill pay via credit card, tuition is non-refundable and non-transferable and refunds will not be given for any reason. Tuition comes out automatically on the 10th of the month for those signed up for monthly withdraw. Parents who wish to pay with cash or check must do so by the 10th of the month, or tuition will be deducted from the account on file automatically on the 11th with a $20 late fee. Parents are responsible for tuition payments for the entire school year August 17, 2020 - April 29, 2021 unless the child is withdrawn from the class. Tuition for additional immediate family members, living in the same household, will be discounted 20%. The tuition rate is for the entire season (August-April). It is not based on the number of classes during a month, but on the entire duration of our season. Some months will have more/fewer classes, but tuition charges remain the same. No refunds or deductions are given if a student misses a class for any reason. Makeup classes are available for missed lessons. Please contact the office for more information. We will be closed for Labor Day, MLK Day, Thanksgiving, Christmas, and Spring Break. Patti Rutland Jazz Dance Center follows the Dothan City Schools Calendar. This also includes and inclement weather closures that may occur.
Overdue Accounts
All payments received are applied to outstanding charges in the order in which they were billed. No exceptions. Payment methods include: online payments through the Parent Portal, Visa, MasterCard, Discover, cash and check. A $20.00 late payment fee will be added to accounts not paid by the 10th of each month. Tuition comes out automatically for monthly withdraw accounts on the 10th of the month. If the card on file is declined, parents have 5 days to update this card or a $20 late fee will be posted to the account. Parents who wish to pay with cash or check must do so by the 10th of the month, or tuition will be deducted from the account on file automatically on the 11th with a $20 late fee. There will be a $38.00 charge for all returned checks and late fees will be applied. If your check is returned by the bank, we require you to make all future payments in the form of credit card through the Parent Portal, cash, or money order/cashier's check. Accounts must be paid in full to participate in the Recital. If an account becomes 60 days overdue, the account holder will be notified that the student cannot participate until the account has been paid in full, or a payment plan has been agreed upon with our office.
Withdrawal Policy
School Year Registrants: To withdraw from a class, notification must be received in writing by the 15th of the month to discontinue charges to the account for the following month. Email notification is preferred or a hard copy withdrawal form may be completed and turned in at the studio front desk. Summer Registrants: No option to withdraw. Tuition is automatically charged upon registration and no refunds will be given for any reason.
Drop Fee
A $10.00 drop fee will be added to your account if you drop a class after September 30th.
Medical Emergency
I, the undersigned give permission to Patti Rutland Jazz, its directors, officers, employees, agents, independent contractors and volunteers to seek medical treatment for the participant in the event they are not able to reach a parent, guardian or emergency contact. I also agree that I will be responsible for any financial debt incurred by said action. I have declared on this form any physical/mental disabilities, limitations, restrictions, or condition and/or declare the participant to be in good physical and mental health.
Release of Liability and Assumption of Risk
I, the undersigned and/or being the legal and acting guardian representing a minor participant, hereby release, waive and discharge Patti Rutland Jazz, its directors, officers, employees, agents, independent contractors and volunteers from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Patti Rutland Jazz. I understand that dance activities as conducted and taught at Patti Rutland Jazz have inherent risks of injury. I recognize that the participant is exposing himself/herself to such risks when undertaking dance activities. I, the undersigned and/or being the legal and acting guardian representing a minor participant assume and accept all risks of injury or damages resulting from such dance activities.
Recital Costumes
Students participating in the Recital will be required to purchase one costume for each subject.
(Acro classes will get their own costumes). Regular costumes cost $70.00 each and combo class costumes cost $85.00 each. We will be taking payments on costumes from August 1st-November 8th. All balances on costumes are due by November 6th, 2020. Payments can be divided over the course of those three months. All remaining balances will be deducted from the card on file on November 10th with tuition. The 2020-2021 PRJ Dance Center Recital location is TBD. All children participating in the recital are required to be at all rehearsals the week of the recital including Dress Rehearsal.
Marketing Release
I understand that images of the participant may be used in Patti Rutland Jazz ads, promotional videos, website material, or various other marketing. These images will be used for Patti Rutland Jazz purposes only, and will not be given or sold to outside companies or individuals.
