TRANSPORTATION
I hereby give my consent for my child to be transported and supervised by the operation's employees for emergency care only.
My child will NOT be transported at any time from the facility, except in emergency situations only.
PROGRAM INFORMATION
I understand that ALL Zenith Learning Before School, After School, and Before & After School Combination programs are charged a monthly tuition.
I will pay the applicable monthly tuition rate assigned to my student based on which school my student attends and which program I have selected.
I understand that tuition is not charged based on actual attendance and there are no discounts for days not attended.
WITHDRAW / DROP SERVICES POLICY
In order to stop services from Zenith Learning, a parent must send an email to info@zenithlearning.org and provide a 10 business day notice.
From the date of the email, tuition will be prorated for that month including the 10 business day notice. If the tuition already paid is greater than the notice of withdraw and the prorated tuition, it will be refunded. If the parent initially paid a deposit, the deposit will be refunded, minus any fees owed.
If a parent chooses to re-enroll in Zenith Learning after a drop, the registration fee is required again. If a parent withdraws from services and the account has a balance, information will be sent to a collection agency and re-enrollment is not possible until the account balance is cleared.
I understand this policy and will adhere to the required 10 business day notice.
LATE FEES
Tuition is due on the 1st of each month, regardless of when the 1st falls on the calendar.
A late fee will be charged for accounts that hold a balance on the 6th of each month.
Accounts with unpaid tuition on the 10th of the month will be dropped, once dropped, parents must contact the office to have their account reinstated. A new registration fee will be required each time a student is dropped.
Parents can contact the office at any time before these deadlines by email to info@zenithlearning.org to make an arrangement for the payment and to avoid late payment fees or their student being dropped.
AUTHORIZATION FOR EMERGENCY MEDICAL ATTENTION:
In the event I cannot be reached to make arrangements for emergency medical care, I authorize the person in charge to take my child to the Physician and Emergency Medical Care Facility provided on this form or contact Emergency Services through 911.
RECORDS
I acknowledge that my child's /children's immunization, vision and hearing records are on file in his/her respective school campus . Required immunizations are current.
COVID19 NOTICE & RELEASE
ASSUMPTION OF THE RISK AND WAIVER OF LIABILITY RELATING TO AFTER AND BEFORE SCHOOL PROGRAMS, ENRICHMENT AND OTHER ACTIVITIES AND THE CORONAVIRUS/COVID-19
This agreement is made by and between ZENITH LEARNING (hereinafter "Zenith") and the undersigned Parent/Guardian below. The parties collectively state as follows:
WHEREAS, the novel coronavirus (hereinafter referenced as "COVID-19") has been declared a worldwide pandemic by the World Health Organization. The Centers for Disease Control and Prevention has also issued Guidance Documents. COVID-19 is extremely dangerous and at this time is believed to be spread mainly from person-to-person contact. As a result, federal, state, and local governments as well as federal and state health agencies recommend social distancing and have, in many locations, prohibited groups of people from assembling.
WHEREAS, the Secretary of Health and Human Services declared a public health emergency on January 31, 2020 under section 319 of the Public Health Service Act (42 U.S.C. 247d), in response to COVID-19; and
WHEREAS, the President of the United States found and proclaimed that the COVID-19 outbreak in the United States constituted a national emergency on March 1, 2020; and
WHEREAS, the Centers for Disease Control and Prevention ("CDC") has stated "COVID-19 is a new disease and we are still learning how it spreads, the severity of illness it causes, and to what extent it may spread in the United States," and
WHEREAS, Zenith has put in place preventative measures to reduce the spread of COVID 19; however Zenith cannot guarantee that you and/or your child(ren) will not become infected with COVID-19. Zenith desires to provide safe and enjoyable programs for all students. Parents understand that there are risks involved with after and before school programs and other associated activities.
NOW THEREFORE, in light of these advisements, each party signing below acknowledges and agrees to the following:
1. Acceptance and assumption of the risk that the disease will spread, even in ideal
conditions;
2. That risk of serious illness or death may result from the spread or transmission of the COVID-19 virus resulting from their own actions or inactions, but also from the actions, inactions or negligence of others, any they voluntarily agree to assume this risk;
3. To regularly check for CDC updates about the spread of the disease and proper control to take all appropriate actions to protect themselves and their child(ren) as dictated by the latest guidance material from authoritative sources;
4. That they and their child(ren) are in good health and have no physical symptoms associated with COVID-19 per the CDC.
5. That they will notify the other in a timely manner if they or their child(ren) are experiencing signs or symptoms of COVID-19, or been confirmed positive for COVID-19;
In consideration for allowing me and my child(ren) access to and use of Zenith before and after school programs (whether as a participant or observer/guest), I voluntarily agree to assume all risks and accept sole responsibility for any injury(ies) to my child(ren) or myself including but not limited to personal injury, disability, illness, damage, loss, claim, liability, loss of limb, or death of any kind ("Claim"), that I or my child(ren) may experience or incur in connection with said access and use, whether foreseen or unforeseen. This waiver is absolute and unconditional, and applies whether or not the party has knowledge of any potential cause of action for such claims. This waiver additionally applies to the heirs, legal representatives, and successors and anyone else by and claiming under me. I understand and agree that this includes any Claim based on the actions, omissions, or negligence of Zenith, its employees, agents, and representatives, whether a Claim or a COVID-19 infection occurs before, during, or after participation in any Zenith program.
ON MY BEHALF, AND ON BEHALF OF MY CHILD(REN), I HEREBY RELEASE, COVENANT NOT TO SUE, DISCHARGE AND I FURTHER AGREE TO INDEMNIFY, DEFEND, AND HOLD HARMLESS ZENITH AND/OR ITS AGENTS AGAINST ANY AND ALL CLAIMS, INCLUDING ANY ACT OR OMISSION ON THE PART OF ZENITH AND/OR ITS AGENTS DURING MY OR MY CHILD(REN)'S TIME USING ZENITH BEFORE AND AFTER SCHOOL FACILITIES FOR ANY PERSONAL INJURY, DISABILITY, ILLNESS, DAMAGE, LOSS, CLAIM, LIABILITY, LOSS OF LIMB, OR DEATH OF ANY KIND, THAT I OR MY CHILD(REN) MAY EXPERIENCE OR INCUR IN CONNECTION WITH SAID ACCESS AND USE, WHETHER FORESEEN OR UNFORESEEN FROM AND AGAINST ANY ORDERS, JUDGMENTS AND/OR DECREES WHICH MAY BE ENTERED THEREIN.
ZENITH LEARNING PARENT AGREEMENT
1. Notice: All registrations or re-registrations are required to pay the registration fee and/or supply fee and the first month's tuition (prorated if applicable). Accounts will be charged when the office receives and processes the registration.
2. Parents are responsible for all tuition and fees outlined below and applicable to their account.
3. Payment:
Auto-Payment by e-check or credit card. When registering online, parents input their checking account information OR enter their credit card information. The account is automatically deducted each month on the 1st (or next business day).
-Parents are responsible for keeping information current. In the event account information changes, parents must update the online
-Should a payment decline or return insufficient, the parent is responsible for the Returned Transaction Fee charges in addition to late payment fees.
4. Registration Fee and/or Supply Fee: all accounts, regardless of choice of payment are charged an initial fee. The initial fee is non-refundable/non-transferable. The fee is posted on our website and may change at any time.
5. Discounts: Discounts cannot be combined.
a. Discounts: 10% off for siblings or Military/Veteran. Documentation must be sent to info@zenithlearning.org upon completion registration.
b. Staff discount: School employees may have a special discount according to the agreement with the campus/district. Documentation to verify employment must be sent to info@zenithlearning.org upon completion registration.
6. Late Pick-Up Fee: $1/minute per child will be charged if a child is picked up late, regardless of the reason (e.g. weather, traffic, emergency, etc.). Charges begin after a four-minute grace period according to the school clock. Late pick-up fees are added to the account and must be cleared by the next time tuition is due or late fees are applicable for the account holding a balance.

8. RTF (returned transaction fee) Charge: a $30 fee is charged for all non-sufficient funds/failed transactions/returned checks/credit card charge backs. In the event of multiple returned items, an account may be suspended.

10. I will complete all necessary forms and I will notify the main office of any changes in registration information (e.g. address, phone number, place of employment, etc.). I will pay all outstanding balances or fees from the previous semester or school year prior to my child's registration or my child's registration may be delayed in processing.
11. For before/after-school care: I will make other arrangements for emergency school closings, delays and dismissals (e.g., inclement weather, facility problems). I understand that ZL is not in session when schools are closed. I understand that if the schools are dismissed early due to emergency conditions the ZL is not in session, and my child will be dismissed according to the information I provide to the school office. I recognize tuition will NOT be prorated due to emergency closings, as enrollment in the program continues.
12. I understand that ZL programs and enrichment have scheduled activities and my child should not bring personal toys or special belongings. If my child has a personal item that interferes with scheduled activities or becomes a distraction to the program, staff members may retrieve the item and return it upon pick-up. I will NOT hold ZL responsible for lost, broken or stolen items. If my child is required to bring a device for a specific Learning program, I will not hold ZL responsible for broken or stolen items. Staff members will provide continuous supervision over all personal items.
13. I certify that I have read the above information regarding tuition and I will abide by all the requirements listed under Parent Agreement regarding Late Fees, Late Pick-up Fees, Non-Sufficient Funds Charge and Withdraw/Drop Services Policy.
14. LIABILITY WAIVER: Zenith Learning desires to provide safe and enjoyable programs for all students. Parents understand that there are risks involved with enrichment, after and before school programs and other associated activities. In consideration of your child being allowed to participate in this program, parents /guardians agree to assume responsibility for any risks associated with activities. I/We, on behalf of myself and my minor child, agree to release, hold harmless and indemnify Zenith Learning, its affiliates, agents and representatives, including volunteers, employees, officers and agents, from any loss, cost, damage and/or expense of any nature, including all attorneys' fees and costs and all claims arising, either directly or indirectly, from my child's participation in Zenith Learning Programs. I/we acknowledge that we have read and understand this document and accept the risk and responsibility of participation in Zenith Learning Programs.
15. TERMINATION FROM THE PROGRAM: I understand and accept that my child may be withdrawn from the program for the following reasons and that payment is due at the time my child is dismissed:
- If my child's behavior is severe, or if my child has persistent misbehavior and does not respond to intervention
- If my child's behavior is greater than can be met in the standard program ratios
- Failure to pay balances on accounts
- Consistently picking up my child late, regardless of the reason
If my child is dismissed from the program for any reason, I recognize my child may not be accepted in ZL Programs in the future.
I acknowledge I have read and will abide by all the guidelines and procedures listed above, as well as all guidelines in any print material or Parent Handbook. My account will be debited upon receipt of the enrollment for the initial fees and the first month's tuition.
SUMMER CAMP / ENRICHMENTS
The guidelines below apply to our Summer Camp and Enrichment Programs. Even if you are not registering for Summer Camp or Enrichment, please verify you have read and agree.
SUMMER CAMP Registrations for summer camp are non refundable and non transferrable for any reason after payment is made/due. Payments for summer camp will be posted and charged the Friday before each week of summer camp starts. I recognize tuition will not be prorated due to emergency closings. There are no discounts for summer.
ENRICHMENT I recognize tuition will not be prorated due to emergency closings. There are no discounts for enrichments.
-Make-Up Policy: In the event of a day of class cancellation, make-up sessions will be offered by extending two classes by 30 minutes each or by adding an additional day, depending on the instructor's decision.
-Transferring / Cancelling Enrollment: Please be aware that once you have registered for a class, we cannot accommodate changes. Please do not register for a class if you are not positive your student will want to attend. This prevents others for registering as there is limited space.
-Tuition: Tuition will be charged the following business day after registration. Once tuition is charged, your student can not switch classes and it is non refundable.
-Refunds: Zenith and the instructors do not offer refunds if a student dislikes the class or cannot attend for the full semester due to personal schedule changes or other instances that may occur.
Enrichment classes need a certain number of students registered in order to take place. If we do not reach this number by the week before the class starts, the class will be canceled.
If a class is canceled because we do not have enough students, any tuition payments made will be fully refunded on the day we make the decision. Please know that deciding to cancel classes based on enrollment is not something we can control.
