Registration/Tuition
There will be a $20 registration fee (each additional family member is $5) assessed at the time of enrollment. This fee reserves the student's place in class, and is non-refundable. Current tuition rates are posted on our website. Tuition is a 45-week fee divided into 10 equal monthly payments (August-May). The tuition is determined by the amount of classes taken by the entire family, not each individual student. Please note, tuition fees are due on the FIRST lesson of every month, and are the same, whether or not there are 3, 4 or 5 lessons in any given month. We DO NOT prorate tuition, or provide refunds for classes missed. We will, however, allow the student to attend a make-up lesson in a like class. If you decide to drop a class, please provide a two-week notice. If payment is not received by the 10th of each month, a $15 late fee will be added to the account.
Calendar
Please see the calendar of events on our website (http://www.studiosevenpg.com) for all important dates and events. If there is a reason that the studio needs to close with little warning, all efforts will be made to contact the parent/caregiver via cellular phone or email. It will then be posted on our website, followed by an update on our Facebook page (http://www.facebook.com/ studiosevenflorida.com).
Payment
Studio Seven Center for Creative Studies takes payment via cash, check or credit/debit card. Checks may be made payable to Studio Seven, and a $25 fee will be assessed for all returned checks. Receipts are available from the receptionist, or may be e-mailed to you. To avoid potential late fees, it is suggested that you set up an automatic payment, should your bank offer this service. The tuition fee is NOT inclusive of dance apparel, footwear, costume costs, art/music supplies, accessories, recital fees or tickets; these are additional expenses.
Attire
It is essential for students to be in proper dance attire when participating. Please see the class description page on our website for specific requirements for each class. Acceptable clothing includes: tights, leotards, dance skirts, dance pants, leggings, and non-baggy t-shirts and sweatpants. If the student is wearing a leotard, shorts, and/or a skirt, dance tights are required. Bare midriffs are NOT allowed. Hair must be pulled back and completely off of the face (for ballet, a bun is required). Aside from small stud earrings, no jewelry is permitted while dancing. Studio Seven Center for Creative Studies is NOT responsible for lost or stolen belongings, so please ensure that personal items are left at home. Proper footwear is critical both for technique, and in the prevention of injury. Suggested footwear and tights are available for purchase in studio. Satin slippers are NOT approved footwear.
Pick-up/Drop-off
Please ensure your student is dropped off with adequate time for drinks, restroom use, and for putting shoes on, as to minimize classroom disruption. Please also ensure that your student is picked up within 15 minutes of the end of his or her class. Students are required to stay inside the building until an authorized adult (as indicated on Authorized Pick-up/Drop-off form) comes inside to take them home. For student safety and traffic congestion purposes, it is REQUIRED that you park your car in a space, and come in to pick up your student.
Classroom Observation
Parents are not permitted inside of the dance room unless invited by the instructor. This helps the instructor to maintain the complete attention of the student, and negate any distraction that may be caused by family members and friends being in attendance. We do have observation windows, but please refrain from gathering at the window for extended periods of time. We understand the importance of seeing the progress that your student is making, so official observation days will be scheduled throughout the year. Please see the calendar of events on our website. If you need to speak with an instructor, please see the receptionist or Ms. Casey to schedule an appointment.
COVID-19
Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19 The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. STUDIO SEVEN LLC ("the Studio") has put in place preventative measures to reduce the spread of COVID-19; however, the Studio cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending the Studio could increase your risk and your child(ren)'s risk of contracting COVID-19.
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending the Studio and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at the Studio may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Studio employees, volunteers, and program participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)'s attendance at the Studio or participation in Studio programming ("Claims"). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless the Studio, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the Studio, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Studio program.
Liability
By submitting this registration form, I agree that I shall not hold Studio Seven, LLC, liable for any injury that the above child may incur while participating. I understand that Studio Seven, LLC does not carry a medical insurance policy for students, and agree that the student is covered by a family or individual medical policy. I agree that I have received, and will abide by the policies listed above. FOR PARTICIPANTS OF MINORITY AGE (UNDER AGE 18 AT THE TIME OF REGISTRATION)

This is to certify that I, as parent/guardian, with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/ward including the risks of presence and participation and his/her personal responsibilities for adhering to the rules and regulations for protection against communicable diseases. Furthermore, my child/ward understands and accepts these risks and responsibilities. I for myself, my spouse, and child/ward do consent and agree to his/her release provided above for all the Releasees and myself, my spouse, and child/ward do release and agree to indemnify and hold harmless the Releasees for any and all liabilities incident to my minor child's/ward's presence or participation in these activities as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent provided by law.
