COVID-19 Risk Acceptance
By entering the facility, you are aware that you agree to fully accept all known and unknown risk, including the potential risk of exposure to respiratory illnesses such as COVID-19. COVID-19 is primarily transmitted via exhaled respiratory droplets, most often through coughing and sneezing. These droplets can travel up to six feet and are more commonly transmitted between persons rather than from equipment to persons.
Although we regularly disinfect our equipment and presently are using enhanced cleaning methods plus enforce social distancing in our facility, you understand that you may be exposed to COVID-19 or its symptoms through no fault of our own. Known COVID-19 symptoms include fever, cough, shortness of breath or difficulty breathing, muscle pain, sore throat, new loss of taste or smell. More severe cases can lead to pneumonia, kidney failure, stroke, or even death. You understand and agree that you will hold us harmless and you will not hold us liable for any real or perceived symptoms of COVID-19 or any other disease, illness, or condition, nor for exacerbating any symptoms. You are fully aware and agree to accept all risks of entering the facility, using the equipment, working with coaches and instructors, attending classes, and/or interacting or being exposed to other members.
Assumption of Risk
I recognize that the activities of fitness, gymnastics, tumbling, and cheer are physical activities that can cause injuries that are catastrophic in nature. Although steps are taken to prevent injury, they can occur. There are risks and dangers associated with participation in gymnastics or cheer, and activities associated with them, including travel and events held outside the building that can cause partial or total disability, paralysis or even death. To help us maintain our high safety standards, please discuss with your child the need for obedience and respect of the coach and facility at all times. This also pertains to parents, siblings, and visitors.
Release of Liability/Medical Emergencies
I hereby release, waive, discharge and covenant not to sue USA Gymnastics, event hosts, Mid-Columbia Gymnastics Inc., Elite Force Cheer, it's owners, next of kin, employees, volunteers, other participants, and building owners.
I authorize the necessary steps regarding medical attention (i.e. first aid, calling ambulance services or other transportation to a hospital) and will allow authorization to hospital faculty and staff to treat my child in the event of an illness or injury when deemed necessary. I understand that all medical expenses are my responsibility as the parent or guardian for the child and Mid-Columbia Gymnastics Inc. is not responsible for these expenses.
Media Waiver
Occasionally Mid-Columbia Gymnastics uses photos or video of its students in print ads, on its website, its Facebook page, or other marketing mediums. I understand that my child's likeness may be used in such advertising. These images will be used for Mid-Columbia Gymnastics purposes only and will not be given or sold to outside companies or individuals.
Gym Policies: Email/Online Policies
It is imperative that we have your current email address and that you are opening/reading our emails. For documentation and time efficiency emailing is our preferred form of communication. Your email address will allow us to inform you of important billing information, facility closings, upcoming events, special activities, and pro shop specials.
Your online portal allows you to update contact info, make payments, view your account history, enroll in classes, reserve birthday parties, register for events, and much more. Anytime there is an update to policies they will show when you login to your customer portal.
Gym Policies: First Visit
*An online registration must be completed by a parent or legal guardian before a child may participate in class and most events.
*Prorated tuition for the month and the Annual Registration fee will be posted to account at time of registration but not due until the week after trial. If you decide that you are not going to continue you MUST notify the office before the next class so you will not be charged anything. Without notice your child will be considered enrolled and fees will be due at next week's class.
Gym Policies: Annual Registration Fee
After your first class and you are enrolled a $50 Yearly Registration is charged to every family and recharged annually. The Annual Registration Fee is non-refundable and non-transferable.
We do not pro-rate or discount the Annual Registration Fee if you are planning on enrolling less than a year.
The Annual Family Registration is for immediate family only. Example: no extended family, other children that you pay tuition for, etc.
Gym Policies: Payment Policies
**As of January 1, 2019 all new enrolls and re-enrolls will be required to have a working debit/credit card on file that is set on an automatic payment schedule. If you wish to use a different payment method you must do so before your scheduled payment date. No refunds will be issued after auto pay is run. If your payment is declined an email will be sent to notify you that it was declined and we need you to let us know if you will be using an alternate payment method or if you would like us to run the card again. This must be done before your child's next class.
*If no auto withdrawal date is chosen, it will default to 1st of month. You can change date at anytime.
*Class tuition is due for the upcoming month no later than the 5th of the month. Payments received after the 5th are subject to $10 late fee.
*Tuition is paid for the upcoming month so if you need to take a break or discontinue for any reason we must receive a withdrawal form no later than the 20th prior to the month you will be discontinuing.
*In the event of 2 parties paying bill they must communicate between each other. Mid-Columbia Gymnastics will not mediate between parties, be responsible for parenting plans, etc.
*If your account becomes 20 days past due, your child may lose their spot in class. **Gymnastics or Cheer Team tuition policies are stated in team handbook and override general gym policies.
*Anyone that has previously had their account sent to collections must show proof collection account has been paid in full and enroll in auto pay to re-enroll.
*Tuition is based on 4 weeks per month. We will not prorate for shorter months and we will not charge extra for longer months. There is no pro-rating or deductions for holidays or classes missed
*If you prefer to use a different payment method than your card on file you must do so BEFORE your scheduled payment date. MCGA accepts online payments, credit cards, checks, or cash. A $35 Fee will be charged for all checks returned from bank for non-sufficient funds.
*Payments are accepted online, at both gym locations, or over the phone.
*In special circumstances where a refund is approved and issued, it is subject to processing fee.
Team Liability (other gyms)
We cannot offer classes to athletes that are enrolled on competitive teams at other gymnastics or cheer gyms due to liability issues.
MCGA/EFC Team Policies
Please refer to your team handbook given at the beginning of season for details on all payment information as well as rates,policies, and procedures as they may differ from regular classes. Once your child is placed on team you will be held responsible for the policies. We highly encourage you to read your team acknowledgement, however, not doing so does not absolve you from team policies.
Gym Policies: Discontinuing Lessons
*A Withdrawal Form must be submitted no later than the 20th prior to the month you will be dropping. You can find this on our website under "Forms" It goes straight to us, no printing required.
*No drops will be accepted over the phone, neither will verbal notice to office staff or the coach.
*If no notice is given, your spot is being reserved whether it is utilized or not and you will be responsible for that month's payment.
*Make ups are only for currently enrolled students, once you withdraw all make ups are forfeited.
*If you leave a balance when you withdraw we will charge it to card on file. If not paid within 30 days we will turn it over to an outside collection agency.
Gym Policies: Make Up Lessons
**During the COVID-19 Crisis Phases make-up lesson policies may be revised due to smaller class ratios and contact tracing. Do not come to class if your child or anyone in household is not feeling well, is sick, or has any symptoms of illness.
*Class space is very limited, please make every effort to maintain good attendance.
*If your child will be absent please call or email the office so we can make note of make up eligibility in your account.
*MCGA allows students to make up classes due to illness or family emergency. Make ups must be scheduled through the office within 30 days of the missed class.
*Please schedule your make up at least 24 hours in advance so it will be on the schedule. Make ups can only be scheduled in classes that have space available. No drop in make ups.
*If your child cannot make it to their scheduled make up, please contact the office. Without notice the make up is lost.
*Make ups are only for currently enrolled students.
Gym Policies: Viewing Area
*During the COVID-19 Crisis there will be limited seating in our viewing area, we advise dropping your child off.
-Limit is 1 parent or guardian per family. Parents with students under 4 yrs or students with special needs will have priority in viewing area
-Phase 2 states that high risk clients are not permitted in the facility. This includes anyone over the age of 65, people with serious underlying medical conditions like chronic lung disease, moderate to severe asthma and individuals with compromised immune systems.
-Water fountains and vending machines will be blocked off no food or drink is permitted. Students with classes over 2 hours may bring a water bottle with their name clearly visible on front.
*We are not responsible for any lost or stolen items left in the gym.
Gym Policies: Gym Closures
*Throughout the year, each student will receive an average of 4 classes per month. (Several have 5!)
*There is no pro-rating or make up classes allowed for holiday closures.
*We do not follow school closures for bad weather. If in the event we do need to close we will notify everyone via email and post on Facebook and Twitter. Make sure to download our app for instant notifications.
