Medical Emergency
The undersigned gives permission to Epic Dance Studios, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health.
Release of Liability
As the legal parent or guardian, I release and hold harmless Epic Dance Studios, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Epic Dance Studios/Concord Dance Club, its owners and operators or in route to or from any of said premises or off-site events.
COVID-19 Guidance
By signing this agreement, I acknowledge the contagious nature of Covid-19 and voluntarily assume the risk that I and/or my child may be exposed to or infected by Covid-19 by attending the studio and that such exposure or infection may result in personal injury, illness, personal disability, and death.
I understand that the risk of becoming exposed to or infected by COVID-19 at the studio may result from the actions, omissions, or negligence of myself and others, included but not limited to, staff and program participants and their families.
Epic Dance Studios is doing everything we can to protect our students and the public as well as our workers and volunteers. To this extent, we will be following the Center for Disease Control and the local health department's guidelines with regard to social distancing practices in order to reduce the spread of the novel coronavirus or COVID-19. This will require staff and volunteers to maintain distance between ourselves and the client as much as possible. We will also be requiring the same procedure for client to client contact in order to limit exposure to all individuals. If the state of Indiana requires a business shutdown or quarantine order, I understand I will be responsible for continued monthly payments for Epic Dance Studios' online dance classes.

Photography Release
Permission is granted to Epic Dance Studios. to use and publish for editorial, trade, advertising, or other promotional purposes all photographic, video, and digital images of my child/ren, including my Child's First Name Only, in conjunction with Epic Dance Studios. All parents/guardians who do not wish their child's image or name to be used in this manner must declare so in writing and submit the signed paper to the Dance Director.
****CREDIT CARD POLICY****
A credit card on file is required in order to register. Tuition will be pulled from this card on the 1st of each month unless online payment, cash, or check is received before the 1st. Within a week of registering for classes, May's tuition will be pulled from this card and then proceed to being pulled on the 1st of Sept-April.
