I understand that summer tuition payments must be made at the start of each week of the summer session - as outlined in the paragraph below, in full, to allow participation. I understand that Summer 2018 tuition payment due dates are Thursday, May 31 for all June 2018 classes, Monday, July 9 for any class that runs July 9, 2018 - July 12, 2018, Monday, July 16 for any class that runs July 16, 2018 - July 19, 2018, Monday, July 23 for any classes running July 23, 2018 - July 26, 2018 and Monday, July 30, 2018 for any classes running July 30, 2018 - August 2, 2018. I understand that a valid credit card, debit card or bank account is required to be on file with the studio, using our secure "Parent Portal." I understand and authorize summer tuition, or any other associated studio fees to be automatically withdrawn, using the payment method on file, if they are not paid by the above mentioned summer tuition deadlines. Additionally, I understand that if tuition is not paid, in full, by the end of each summer week, a $25.00 late fee will be added to my account. I understand that a $10 fee will be added and withdrawn from any account for a declined credit card payment. I understand that tuition is available to be viewed using our online system, the "Parent Portal" at www.csdanceacademy.com, to make secure online payments. Customer can check their balance at any time by logging into their account at www.csdanceacademy.com. For customers who still wish to pay by check, a $25.00 service fee will be charged for all returned checks. Online payments are suggested and encouraged.
I understand that there are no tuition refunds for classes - this includes classes your dancer is scheduled to attend but is absent from, classes that are cancelled due inclement weather, and classes that your dancer does not participate in due to injury, illness or any other condition. There are no refunds for registration fees. I understand that there will be a $10 fee for any dancer that drops or transfers into another summer class within 7 days of that class. To avoid this, please notify the studio at least 7 days prior to the class if you know your dancer will not be attending a class that they have signed up for. I understand that no credits or refunds will be issued for a missed class. I understand that classes canceled by Centerstage Dance Academy will not be charged. Any exceptions to the refund rule can only by made by the Director of Centerstage Dance Academy.
I understand that tuition is available to be viewed and and processed using our online system, the "Parent Portal" at www.csdanceacademy.com, and that online payments are suggested and encouraged. Customers can check their balance at any time by logging into their account at www.csdanceacademy.com but no partial payments are allowed through the portal. I also understand that additional fees such as costumes and dancewear will be added to my account when applicable.
I understand that costume deposits for the spring recital performance are due by November 15 and the remaining costume balance is due by January 15. I understand that paying the initial costume deposits, I am mandated to pay for all remaining costume balances in full.
I understand that tuition for the months of April and May will be posted simultaneously and must be paid before recital ticket day in order to purchase recital tickets. I understand that I have the option to "opt in" for auto-pay which will automatically take my tuition out using my card on file on the 5th of the month.
I understand that we require any dancer participating in recital to pay a one-time $25 rehearsal fee per dancer. If families have one or more siblings, you only pay $10 for each additional dancer. This covers the cost our on-stage dress rehearsal/performance and helps to keep our recital ticket prices from increasing substantially. I understand that the rehearsal fee is due on March 15, 2018.
Centerstage Dance Academy reserves the right to discontinue the account of any customer with an outstanding payment balance of 60 days or more. Accounts and unpaid balances that are more than 60 days past due will be submitted to a collections agency. I understand that Centerstage Dance Academy will not be responsible for any attorney or collection fees arising from our efforts to collect unpaid balances.
I understand that dance classes may include, without limitation, dancing with props, stretching, barre work, across the floor combinations, dance routines in the center, and other related activities. I further understand that all of the activities of the dance class involve some degree of risk of strain or bodily injury. Centerstage Dance Academy strives to reduce this risk through proper dance training and technique. Each student may decline to participate in any activity which she/he feels may be harmful and is responsible to inform the instructor of any physical limitations which may prevent full participation in class.Centerstage Dance Academy and its employees, instructors, subcontractors, volunteers and participants are not responsible or liable for personal injuries sustained or loss or damage to physical property.
As the parent/legal guardian, I have read the rules and general information on this site and agree to abide by them. I release and hold harmless Centerstage Dance Academy and its employees, owners, participants, subcontractors and volunteers from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Centerstage Dance Academy and its employees, owners, participants, subcontractors and volunteers or in route to or from any of said premises.
