I understand that tuition will be determined by the total number of hours my dancer(s) attends class, each week. I understand that tuition must be paid monthly, in eight equal payments, from September, 2018 until April, 2019. I understand that tuition is also able to be paid on paid on a semester (bi-annual) or annual basis with permission granted by Centerstage Dance Academy. I understand that tuition can be viewed and and paid using our online system, the "Parent Portal" at www.csdanceacademy.com, and that online payments are suggested and encouraged. Customers can check their balance at any time by logging into their account at www.csdanceacademy.com. I also understand that additional fees such as costumes and recital rehearsal fees will be added to my account when applicable. I understand that costume deposits for the spring recital performance are due on November 15 and the remaining costume balance are due on January 15. I understand that paying the initial costume deposits, I am mandated to pay for all remaining costume balances in full. I understand that my tuition and any applicable studio fees must be paid in full before recital ticket day in order to purchase recital tickets. I understand that I have the option to "opt in" for auto-pay which will automatically withdraw tuition, using my credit card on file, on the 5th of the month.
I understand that we require any dancer participating in recital to pay a one-time $25 rehearsal fee per dancer. If families have one or additional siblings, you only pay $10 for each additional dancer. This covers the cost our on-stage dress rehearsal/performance and helps to keep our recital ticket prices from increasing substantially. I understand that the rehearsal fee is due on March 15, 2019.
Centerstage Dance Academy reserves the right to discontinue the account of any customer with an outstanding payment balance of 60 days or more. Accounts and unpaid balances that are more than 60 days past due will be submitted to a collections agency. I understand that Centerstage Dance Academy will not be responsible for any attorney or collection fees arising from our efforts to collect unpaid balances.
I understand that tuition payments must be made monthly and are due by my dancers first class of the month. I understand that a valid credit card or debit card is required to be on file with the studio at all times and will be kept within our secure online portal. I understand that if my credit card becomes invalid, for any reason, I must provide the studio a new or updated card within 48 hours. If a new credit or debit card is not given within that timeframe, I understand that my dancer may be unable to participate in classes until payment has been made in full and that a $25 late fee will also be added to my account. I understand and authorize monthly tuition, costume fees and any other applicable studio fees to be automatically withdrawn on the 16th of each month, using the payment method on file, if they are not paid on or before the 15th of the month. I understand that a $25.00 late fee will be added to my account after the 15th of the month for any late payments occurred. I understand that a $10 fee will be added and withdrawn from my account for any declined credit card payment. For customers who still wish to pay by check, a $25.00 fee will be charged for all returned checks. I understand that any late fees, declined credit card fees or any other processing fees must be paid, in-full, and that Centerstage Dance Academy can not and will not remove any of these fees ,for any reason, once they have been posted. I understand that there are no tuition refunds for classes - this includes classes your dancer is scheduled to attend but is absent from, classes that are cancelled due inclement weather, and classes that your dancer does not participate in due to injury, illness or any other condition. There are no refunds for registration fees. Any exceptions to the refund rule can only by made by the Director of Centerstage Dance Academy, Kathy Coughlin.
I understand that dance classes may include, without limitation, strength and conditioning, dancing with props, stretching, barre work, across the floor combinations, dance routines in the center, and other related activities. I further understand that all of the activities of the dance class involve some degree of risk of strain or bodily injury. Centerstage Dance Academy strives to reduce this risk through proper dance training and technique. Each student may decline to participate in any activity which she/he feels may be harmful and is responsible to inform the instructor of any physical limitations which may prevent full participation in class. Centerstage Dance Academy and its employees, instructors, subcontractors, volunteers and participants are not responsible or liable for personal injuries or damage to physical property.
As the parent/legal guardian of the registered dancer, I have read the rules and general information on this site and agree to abide by them. I release and hold harmless Centerstage Dance Academy and its employees, owners, participants, subcontractors and volunteers from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Centerstage Dance Academy and its employees, owners, participants, subcontractors and volunteers or in route to or from any of said premises.
