Release of Liability
As the legal parent or guardian, I release and hold harmless Corona Stars Cheer, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Corona Stars Cheer, its owners and operators or in route to or from any of said premises.
Medical Emergency
The undersigned gives permission to Corona Stars Cheer, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health.
Paperwork
Prior to attending your 1st class, you must sign the following forms which will be emailed to you; Participant Release and Waiver Form, Fees and Payment Agreement, and the Mandatory Official Account Authorization Form.
Payment Information
The monthly fee is due on the 1st of each month. If a payment is declined, the credit card company automatically attaches a $25 late fee, and continues to bill the card. After 7 days without payment, the account will referred to collections. If a payment isn't received or declined, after 7 days, your account is placed on hold and the athlete will be unable to attend the class until payment / late fees are paid and the account is current.
Dropping a Class
You must give notice when dropping a class. The notice must be given 7 days before the end of the month, prior to the last month attended. ( For example, if you gave your notice on May12th, June would be your last month. If you gave your notice on May 30th, July would be your last month, since it was not 7 days prior to the end of the month the notice was given.) All drop notices must requested through the Parent Portal by the "Contact Us" link. Once requested, you will be sent a discontinue form that will need to be digitally signed by you. Only the original parent(s) / legal guardian, who originally registered the student, may give the drop notice.
Gym Rules
1. Registration fees are due annually from the date of your registration. Registration only applies to classes you register in.

4. If gym attire, paid by check, is returned as non-sufficient funds, there will be a $25.00 Service Charge that must be received within 3 days. Corona Stars Cheer reserves the right to forward the issue of non-payment items to the Riverside County District Attorney Bad Check Restitution Program and your child might be dismissed from the program.
5. All classes are on a monthly basis, from the first to the first. If your child would like to CHANGE or DISCONTINUE taking a class, you must notify us through the parent portal. If notification is not received you will be charged for the additional months that a space was held in your child's name.
6. Your child's monthly fees are due regardless of how many classes your child attends and/ or holidays that Corona Stars observes in any given month. Our class fees are based on a 4 week month, while some months have 3 weeks and other months have 5 weeks.
7. Your child can be suspended from class for displaying inappropriate behavior and refusal to follow gym rules and coaches instruction. You are still responsible for your child's monthly fees and account during the suspension period.
8. If you choose to participate in private lessons, your instructor reserves the right to set up a personal contract with you and your child. All private lessons must register and pay prior to each and every lesson.
9. Every participant must purchase and wear the properly designated cheer shoes while in the gym. They are not to be worn outside the gym at any time. This is to ensure that our floors remain in the best condition.
10. Hair must be worn in a high ponytail to all classes. If hair is longer than your shoulders, hair must be placed in a bun.
11. No jewelry, etc. (We ask that athletes do not bring jewelry to practice) This means no necklaces, bracelets, rings, etc. All earrings, nose rings, belly button rings need to be taped or covered in a band aid. It is preferred that all piercings are removed to due safety concerns.
12. I release CSC the right to take pictures or video the progress of my child. Parents & athletes are unable to videotape in the waiting room or gym. If you would like to video your child, arrangements can be made at the owners discretion.
13. CSC can place current accounts on hold for 60 days, however after 60 days, you will need to re-register.
Cleaning and Gym Protocols
Cleaning and Gym Protocols
• Any athlete that feels sick or has a sick family member should stay home. There will be absolutely no penalization for missing practices due to fever of the athlete or anyone in the athlete's household. Play it safe - stay home, rest up, and notify gym. You will need a doctor's note to return.
• If we have an exposure in our building, per OSHA, CDC & specific county guidelines, the gym will remain closed for the duration required per that municipality to thoroughly clean the facility. Be cognizant of your health and do not leave your home if you (or anyone in your household) are not feeling well.
• Each athlete must wear a mask when outside of their vehicles. Ensure it is properly fitted and you can work out with the straps. We will follow county guidelines for masks - this is a moving target. There are different mask options that offer differing levels of room to breathe - find what works best for you.
• Please, do not touch your face. If you have an itch or must remove sweat, try to use your forearm.
• Only athletes and working coaches are allowed outside of vehicles at our gym location. No siblings, no friends (even if on other Corona Stars teams or classes), and no non-working coaches allowed in the gym at any time.
• A water bottle with their name printed on it and car keys are the only personal items allowed in the facilities. All other items, including phones, need to be left at home or in a vehicle.
• No spotting of tumbling until further notice to adhere to social distancing.
• CSC will utilize separate entrance and exits. Parents, your athletes may be released from an alternate side of the building- please note where the new entry and exit locations are.
• Athletes will line up on each marked space to wait for temperature read and hand sanitizing station and will adhere to strict social distancing guidelines of at least 6'.
• We miss hugs, too! But now is not the time, and we cannot allow contact between any athlete or coach during this time.
• Athlete temperature will be taken with an infrared thermometer (no touch) by one identified staff member only who will employ PPE to prevent potential exposure. The Centers for Disease Control and Prevention considers a reading of 100.4 degrees Fahrenheit a fever. While an actual temperature reading is the best diagnostic tool for fevers, the CDC also says it considers a fever to be present when a person feels hot to the touch, feels feverish (possibly with chills), or looks flushed or glassy-eyed. Any athlete or staff with these signs need to immediately leave and monitor their health from their home.
• Please bring your own water labeled with your name. Any kind of sharing is prohibited.
• We will be utilizing a limited set of equipment and will be disinfected after each use. Athletes will also disinfect any equipment they use. All Tumble Traks are closed until further notice. Please ensure you do not touch anything in the gym, including walls.
• All doors will be opened and secured in that position by staff- athletes, ask staff to open doors that may have accidentally closed.
• All gym offices are closed - payments or other business must be online or over the phone.
• Bathrooms for athletes and staff are not available except for emergency situations and must be cleaned by staff with the correct EPA-approved disinfectant immediately after use.
• Staff will have their temperature taken each workday, prior to entry into the building, and will comply with all handwashing, cleaning, and strict social distancing guidelines.
• Surfaces, entry ways, and mats will be sanitized between each session with the appropriate EPA- approved cleaner as identified on this list: https://www.epa.gov/pesticide-registration/ list-n- disinfectants-use-against-sars-cov-2
We will have sanitizing stations around the gym. Athletes will break every 30 minutes to sanitize.
