By agreeing to our policies online, your child(ren) will be eligible to participate at MAG.
For your convenience, MAG electronically stores your credit card number in accordance with legislated procedures. Please expect an emailed receipt with the total amount charged to your credit card.
COVID-19 Release/Assumption of Risk/Waiver
I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending Maine Academy of Gymnastics and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at Maine Academy of Gymnastics' may result from the actions, omissions, or negligence of myself and others, including, but not limited to, employees, volunteers, program participants and their families.
I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)'s attendance at Maine Academy of Gymnastics' or participation in Maine Academy of Gymnastics' programming ("Claims"). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless Maine Academy of Gymnastics', its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Maine Academy of Gymnastics', its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Maine Academy of Gymnastic 's program.
Photo & Video Release
MAG coaches take and use headshots of students for our internal database to help with attendance, skill tracking, and guest coaches. MAG also uses photos/videos from time to time for purposes of publicity, illustration, advertising or website content. These images will be used for MAG purposes only and will not be given or sold to outside companies or individuals. If you have any questions or concerns please contact the Office staff directly.
Membership Fee
The membership fee of $45 is paid when you initially enroll and will be added to your tuition on the anniversary month of your original enrollment. This fee covers administrative costs and student/athlete secondary insurance. This fee is non-refundable and will not be prorated. The membership fee will be waived for camp/clinic-only enrollments.
Monthly Tuition with Automatic Payment Plan
Tuition is billed and paid monthly using a credit card or debit card. Tuition is deducted from your account on the 25th of each month for the following month. A billing statement will be emailed to you on the 20th of each month as a reminder regarding your automatic payment. If there are changes to your credit card or debit card, the information must be received prior to the 25th online or in the office. Tuition once paid, is non-refundable.
If your credit card is declined, the issue must be resolved and payment received by the end of the current month to continue in the program.
If you choose to become inactive, a withdrawal form located in the office or emailed upon request must be received by the 20th of the month prior to withdrawal in order to be removed from the Automatic Payment Plan. Please note that your child will be dropped from the class and your child's spot will become available to another student. If the Office does not receive a completed withdrawal form by the 20th, then your child will be enrolled and charged for the next month.
If you need to discontinue due to injury/illness at any time, you must notify the office within one week of injury/illness in order for any adjustments to be made to your account.
Payments can also be made in the office prior to the 25th of the month by check or cash if you do not want your credit card on file charged. Otherwise, your credit or debit card will automatically be charged the monthly tuition.
Returned Checks
There will be a $25 fee charged for returned checks. Returned checks and fees must be paid with cash, money order or credit card.
Late Fees
A late fee of $25 will be charged if payment is not received by the 25th of each month for recreational accounts.
A late fee of 13% will be charged if payment is not received by the 25th of each month for Team Membership accounts.
Refunds/Credits
All tuition, once paid, is non-refundable. All account credits will expire after one year and are non-transferrable. Account credits may be used for other classes, gym apparel, open gym passes, tumble passes, gift certificates or birthday parties.
Illnesses
If your child has been sick in the past 24 hours, please do not bring them to the gym. If your child has had a fever, yellow-greenish mucus from their nose, vomited or had diarrhea in the past 24 hours then they need to stay away from other children. Children must be free from fever, without the aid of medication for a minimum of 24 hours before participating in class. As with any children's program, keeping the children and staff healthy keeps us all happy!
Emergency Medical Care Authorization
In the event Student Participant should become accidentally injured or ill while at ("MAG") or any related activity in which Student Participant may be involved, we hereby authorize MAG or its agents to arrange for whatever emergency medical care is deemed necessary and reasonable at the time, including transport to a local hospital Emergency Department and agree to be solely responsible for all expenses and costs related to such emergency treatment and agree to indemnify and hold harmless MAG, its shareholders, directors, officers, employees, coaches, agents and Parents Association for any expenses and costs it may incur related to such treatment.
Release/Assumption of Risk/Waiver
In consideration of Maine Academy of Gymnastics ("MAG") accepting and permitting Student Participant named above to participate in its gymnastics program and other athletic events, directly or indirectly related to MAG and MAG activities, respectively, Student Participant named above and parent(s) and/or legal guardian(s) of minor Student Participant (collectively the "Parents") understand, acknowledge and agree that:
- Student Participant named above may be involved in gymnastics and any related activity provided by MAG which may include travel, both local and away and in consideration of MAG allowing Student Participant to participate in its gymnastics programs and related activities, as more fully spelled out in the attached Release, Waiver of Liability, Assumption of Risk, Indemnification, and Arbitration Agreement, we unconditionally release, hold harmless, waive, discharge and covenant not to sue MAG, its shareholders, directors, officers, employees, coaches, agents and Parents Association from any and all liability for damages and costs and any claim or demand therefore, directly or indirectly arising out of or related to any injuries that above-named Student Participant might sustain while engaged in the practice or performance of any gymnastics routine or activity while at MAG or during participation at any event in which Student Participant, MAG, it's coaches, agents or Parent Association are involved or that we, the parents/guardians might sustain as a direct or indirect result of Student Participant's or our participation in, association with, or attendance at any gymnastics program or related activity of MAG.
- There are risks and dangers, direct and indirect, that are inherently associated with participation in and attendance at gymnastics or athletic events and activities such as those encompassed in MAG activities that can result in bodily injury, partial and/or total disability, paralysis and death. The social and economic losses and/or damages that can result from these risks and dangers can be severe. These risks and dangers, as well as other risks not currently known and/or not reasonably foreseeable at this time, may be caused by the action, inaction or negligence of Student Participant or Parents or the action, inaction, rescue attempts or procedures, or negligence of others, including, but not limited to, Releasees named below. If Student Participant and Parents of minor Student Participant believe that class/event conditions are unsafe, they shall immediately advise MAG of such conditions and refuse to participate or, if Student Participant is a minor, Parents shall withdraw minor Student Participant from participation.
- Recognizing the risks inherent in MAG activities and the right of Student Participant to refuse to participate, and, if Student Participant is a minor, the Parents' responsibility to withdraw minor Student Participant from participation, Student Participant and we the Parents, hereby fully accept and assume the risks and responsibility for any and all losses and/or damages directly or indirectly arising from or related to any injury, disability, paralysis or death, however caused and whether caused in whole or in part by the negligence of Releasees named below and accordingly HEREBY RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE MAG, its shareholders, directors, officers, managers, coaches, employees and agents, (herein "Releasees") FROM ALL LIABILITY TO STUDENT PARTICIPANT AND PARENTS, as well as to their respective personal representatives, assigns, executors, heirs and next of kin for any and all claims, demands, losses or damages and any claims or demands arising directly or indirectly from or related to any injury to Student Participant or any of the Parents, including but not limited to partial or total paralysis or death, or damage to property, directly or indirectly arising out of or relating to MAG activities caused or alleged to be caused in whole or in part by the negligence of Releasees or otherwise.
WE HAVE READ THIS CONSENT, RELEASE, WAIVER OF LIABILITY, AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT WE HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND HAVE SIGNED IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO US AND INTEND OUR SIGNATURES TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.
