Absolute Dance Company Parental Permission and Release of Liability
Parent/Guardian/Dancer over 18 will herby be referred to as the Releasor.
In consideration of being permitted to enroll in dance class instruction given by Absolute Dance Company, Releasor, on behalf of herself/himself and his/her minor child and his/ her personal representatives, heirs, and next of kin, releases, waives, discharges and covenants not to sue Absolute Dance Company, its officers, members, and all referred to a Releasees, from all liability to the Releasor, his/her personal representatives, assigns, heirs and next of kin for all loss or damage, and any claim or damage, on account of injury to the person or property of resulting in death of the Releasor or his or her minor child, whether caused by the negligence of Releasees or otherwise while the Releasor is participating in any activity sponsored by Releasee.
Releasor agrees to indemnify the Releasees and each of them from any loss, liability, damage or cost Releasees may incur due to the presence of Releasor in or on 690 Avon Belden Road, Avon Lake, Ohio 44012 and/or while competing, or participating in activities sponsored by Releasee.
Releasor agrees that this release, waiver, and indemnity agreement is intended to be as broad an inclusive as permitted by the laws of the state of Ohio and that if any portion of the agreement is held invalid, it is agreed that the balance will, notwithstanding, continue in full legal force and effect.
Releasor, being of lawful age, in consideration of having his/her child be permitted to participate in activities sponsored by Releasee, does for himself or herself, his or her heirs, executors, administrators, and assigns, releases and forever discharges Absolute Dance Company, its employees, heirs, administrators, and executors of and from any and every claim, demand, action or right of action, of whatsoever kind or nature, either in law or in equity arising from or by reason of any bodily injury or personal injuries known or unknown, death and/or property damage resulting or to result from any accident which may occur as a result of participation in activities sponsored by release or any activities in connection with the dance classes, whether by negligence or not.
Releasor further states that he/she has carefully read the above release and knows the contents of the release and signs this release as his/her own free act.
Releasor further releases all employees and officers of Absolute Dance Company from any claim whatsoever on account of first aid, treatment or service rendered him or her during participation in dance activities.
This release contains the entire agreement between the parties to this agreement and the terms of this release are contractual and not a mere recital.
Absolute Dance Company Tuition Information
You will be paying for the entire school year of 36 weeks of classes held over 10 months September - June. This is an average of 3.6 classes per month. You are not paying for scheduled school vacations or holidays. Please see our published calendar in this booklet. The Registration Fee is due with the September tuition. All other months are due by the first day of the calendar month. To avoid late fees pay in advance. For example, October's tuition is due October 1st, please send tuition payment in during the final week of September. Absolute Dance Company accept cash checks, Visa and Mastercard.
The calendar in each month will vary depending upon which day you have your class. Some months you may have 2 or 3 classes, others 4 or 5. Everyone will have had 36 classes over the ten month dance session. If for any reason you decide to discontinue your classes, you are not obligated to pay for the balance of the school year.
We have a very fair policy regarding our payment for tuition and subsequent assessment of late fees. Payments are due on the first of each calendar month. There is no late fee assessed until after 5th of the month. Any account unpaid after the fifth of the month will include a $10.00 late fee. Payments may be made at class or by mail. No credit is given for student absences, missed snow cancellation makeup classes. If you make a payment late, please include your late fee or you will be billed. If you make your payments by mail, be certain to allow adequate time for mail delivery. Thank you for your co-operation.
Returned check fee is $33.00
Any student with an overdue balance of more than 45 days will not be allowed into class until payment in full is received. Overdue balances must be paid by credit card, money order, or bank check.
If you change your class style, add or delete a class or have any questions concerning the monthly charge, please inform someone in the office or by calling 440.315.5678.
Please make up any missed class within one month of your absence if possible; no credit will be given for missed classes.
The quality of our facility and faculty depends upon a system of mutual respect and cooperation. We understand that anyone may make a late payment from time to time, but please be respectful of our late fee policy and when applicable, graciously include your late fee in your next payment. Absolute Dance Company Studio Policies
Please note the following guidelines regarding all classes:
1. We request that students carefully observe our dress code outlined in this booklet.
2. Students should arrive 5 minutes prior to their scheduled class time. Please do not disturb the class in progress. Please wait in the waiting room until your scheduled class time.
3. School age children and teens: please practice respect. Students are not to be socializing in a disruptive manner during the class with other students. Please come early or stay late to enjoy friends in the waiting area. Please honor your teacher and fellow students by giving full attention and co- operation. You will be advised if we feel any student is not well adjusted to the classroom environment for any reason over time.
4. Cell phone use during class is prohibited. Any cell phones brought into the studio must be set to silent.
5. Parents and guardians are permitted to remain in the waiting room throughout class. No parent or guardian is permitted to observe class. The instructor requires the full attention of each student. Parents are invited back into the classroom for the final 5 minutes of class.
6. Students must adhere to the dress code, code of ethics, studio policies, and attendance agreement. Please contact Absolute Dance Company owner/director Brandie Andrews with any questions or concerns.
7. If any parent/student wishes to have a private conference concerning the student's individual dance goals, please do not hesitate to ask! We are happy to work with you to ensure that your dance year is one of pleasure and growth!
8. Inclement Weather Days: In the event that Lorain County is at a Level 2 warning or higher, classes will be canceled.
Dance class provides education and enjoyment, however, an important part of class work is developing an attitude of discipline and respect for teachers and fellow students. We trust that following our classroom etiquette will provide an environment whereby classes at Absolute Dance Company will be a pleasure for all enrolled.

To train and empower students to a pre- professional level with a competitive edge who may consider dance as a major or minor in college. To make possible a young dancer's dream to pursue a career in dance performance or as a dance educator.
