Covid 19 Safety Precautions
Student and Parents
1. Zoom classes will be offered for those not comfortable returning to the studio at this time.
2. Students must stay home if they are sick or have a fever. They are welcome to Zoom into class.
3. Students must be able to follow social distancing procedures posted in the Dance Studio.
4. Students must wear masks, except when actively participating (does not apply to under children under 12).
5. Dancers should remain in their cars until a staff member greets them at their car and escorts them into the building.
6. Dancers should arrive in dance clothes and will not be permitted to change in the restrooms.
7. Dancers will only bring in the shoes needed for class, as well as water for the night.. ALL SHOES, BAGS, and WATER BOTTLES MUST BE LABELED WITH THE DANCER'S NAME.
8. Dancers will be required to hand sanitize upon entering and exiting the building, as well as after using the restroom.
9. There will be no eating in the lobby or dance studios except for students who have multiple classes in the same night. If food is needed, it must be brought from home in a sealed bag, labeled with your child's name. PARENTS MAY NOT DELIVER FOOD, NOR MAY STUDENTS HAVE FOOD DELIVERIES BROUGHT IN FROM OTHER SOURCES. Any food items brought to the studio must be PEANUT FREE.
10. Dancers who have multiple classes in the same evening will wait in designated socially distanced spaces while classrooms are cleaned between classes.
For Faculty:
1. Returning to the studio is optional. Any staff member who feels uncomfortable returning to the studio can continue to teach via Zoom from home with no loss in pay.
2. Instructors and staff will stay home if they have a cough, fever, or if a member of their household has been sick or shows signs of Covid19 symptoms.
3. No physical contact is permitted at any time with anyone (students, other teachers, parents, other staff members).
4. Instructors and staff will wear a face masks when in the studio. For teachers who physically dance with the students, face masks are optional pursuant to the Covid19 guidelines for exercise facilities. In addition, teachers will stay socially distanced while in class by at least 6 feet at all times.
5. Instructors and staff will sanitize studios between classes, including barres, floors, and teacher area (stereo/etc.).
6. Instructors will provide students with hand sanitizer both upon entering and exiting the building.
Building Use and Cleaning
1. Fire Code Capacity: Fire Code Capacity is 155 people. To meet the 30% capacity requirement, no more than 46 people will be permitted in the building at any one time.
2. Lobbies will be closed. Only students, teachers, and DLBS staff are permitted inside of the building.
3. Classrooms will be disinfected between classes with steam mops as well as with a pH neutral cleaner.
4. Classrooms, restrooms, barres, lobby, and hardware within the building (door knobs, etc.) will be disinfected between each class.
5. Classes will be limited based on 6 to 10 feet of social distancing, by classroom.
6. Office will be closed to students and instructors.
7. Students will be given hand sanitizer upon entry to the building, and again upon exiting.
8. Studios will be taped off in social distancing squares to ensure students maintain distance at all times.
9. No communal supplies will be used - dancers must provide their own Therabands, water bottles, dance shoes, and dance bags.
Additional Details
-The number of students in the studio will not exceed 30% of our fire occupancy.
-The office will be closed outside of answering phone calls and assisting with faculty needs.
NO TUITION PAYMENTS WILL BE ACCEPTED AT THE STUDIO.

At all times, we will be observing social distancing guidelines. All persons entering and exiting the dance studio will be required to maintain social distance.
Commitment
I understand that I am registering myself or child(ren) for The Dancing Lights Ballet School. I am fiscally responsible for the monthly fee associated with the attendance of classes on a monthly basis. I am responsible for a non refundable annual registration fee ($25 per family). I understand if it is necessary to discontinue classes for any reason The Dancing Lights Ballet School requires two weeks notice. I understand two weeks tuition will be forfeited. If the suspension or absence of classes is for a medical reason,The Dancing Lights Ballet School requires a written medical explanation from a treating physician, and upon return may require a release to resume.
Registration and Tuition
Upon registration online I will receive a confirmation email. This is an automated email and does not confirm correct placement. If I am a new student and have not received direction, the director or staff will email with placement confirmation. I understand once I've received a confirmation email first months tuition and registration fee is required.
I give permission for the card on file to be charged the MONTHLY FEE due and Registration fee when applicable unless I otherwise notifiy the director via email.
Payments are based on the total number of hours of class enrollment, with each class meeting 36 times per year. As each month has the potential for a different number of classes, the tuition for 36 weeks is divided into 9 equal payments. The session begins on Monday, September 19th and is completed on Monday, June 17th.
Tuition | Due Dates | Late Fees
I understand all tuition payments are due by the 5th of each month from August (for Sept/ June) through April (for May). If tuition payments are received after the grace period (after the 5th of each month), a late fee of $15.00 will be applied to your account for which you are responsible. If monthly payments are in arrears for more than 30 days, the late fee will be adjusted to $25.00. If the account is in arrears for more than 60 days, students may be withheld from all classes until the account is made current. Please note in the case of financial or medical hardship financial arrangements for payment are considered by Miss Kristin regularly. Returned check fee is $25.
Payment Methods
I understand acceptable methods of payment include cash (only when given to the director), personal checks made payable to The Dancing Lights Ballet School, Visa, MasterCard and American Express by the due date. Please note, there is a 3% processing fee for credit card payments.
Automatic recurring monthly payments for credit cards are available. My card on file will be charged if I have not logged in and made a payment independently.
Should a credit card transaction be declined for any reason a late fee $15.00 will be assessed.
Release of Liability and parent code of conduct
As the legal parent or guardian, I release and hold harmless The Dancing Lights Ballet School, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of The Dancing Lights Ballet School, its owners and operators or in route to or from any of said premises.
Parent code of conduct on premises or any premises under management of The Dancing Lights staff requires a consistent message of the proper role of a parent in support of the environment, classroom, waiting room. I will be a positive role model for my child by encouraging respect and courtesy in demonstrating positive support for all students, teachers and parents. In showing respect and courtesy to all, I will privately discuss any discord.
Medical Emergency
The undersigned gives permission to The Dancing Lights Ballet School, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health. I request that our doctor/physician be called and that my child be transported to the nearest hospital.
Photographs and Social Media
I understand that Dancing Lights Ballet School may from time to time take photographs and/or video of the dancers enrolled on this form.
I, the parent/guardian authorize Dancing Lights to use such photographs and/or video for school use, social media and class pictures. Please contact us via email if you have specific concerns concerning this waiver. IN WITNESS WHEREOF, I have read and understand this waiver.
