Risk and Waiver of Liability
I am fully aware that any activity involving motion or height creates the possibility of serious injury and I further agree to hold AOG and its agents and affiliates harmless from any injury to my child/children. As the legal parent or guardian, I release and hold harmless AOG , its affiliates, agents, owners and operators and each of their respective agents and owners from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control, supervision, and/or use of AOG , its affiliates, agents, owners or operators or any of their respective agents or owners or in route to or from any of said premises.
The undersigned gives permission to Alpha Omega Gymnastics , its affiliates, agents, owners and operators and each of their respective agents and owners to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare that I will disclose any physical/mental problems, restrictions or condition and/or declare the participant to be in good physical and mental health.
AOG sometimes photographs or films its students. These images may be used on the gym's website, offered for sale in formats such as photographs or DVD's, displayed in the gym, or used in other ways. I understand and agree that AOG may photograph and videotape my child/children and use his/her name and photographic likeness in all forms and media for any lawful purposes.
Payment Policies
I understand that upon my initial registration, the 1st charge will include: Enrollment fees, prorated tuition for current month and/or next month's tuition if enrolled after the 15th of the month, and required uniform fees. Remember that tuition is charged in advance on the 15th of the month. IE. We will charge Dec. tuition on Nov. 15.
I understand that all Tuition is billed/paid electronically by Credit Card Charge. The electronic payment will be charged on the 15th of the month for the following month. There will be no refunds once tuition has been charged.
I understand that in order to cancel or freeze my membership, the membership change form (links located at the bottom of our website: https://www.alphaomegagymnastics.com) must be submitted prior to the 10th day of the month in order to avoid a charge for the next month's tuition.
I understand that I will be responsible for all tuition until written notice of cancellation is given.
I understand that tuition is based on a 46 week year, and there will not be classes the weeks of Spring Break, 4th of July, 1st week of school, Thanksgiving, Christmas, and New Years. Membership fees will not be prorated during these months.
I understand that there will be no prorating of tuition for missed classes. I am responsible for full tuition regardless of class attendance.
Where applicable, I understand that there is a $25 late fee per 15 minute block for late pick-ups. The first 15 minute block is a a free grace period.
We generally do not accept checks. In the event that a check is accepted and returned as NSF; a $30 return check fee will be applied.
Should I dispute a charge through my financial institution this will constitute a breach of contract possibly resulting in, but not limited to penalties, additional fees, collection, legal action, and/or termination of any/or all current and future agreements.
Once payment has been charged for any program, except for inside of the first 30 days, I understand that no refunds will be issued.
I understand that all policies are listed on the AOG website under, Our Center/Membership Details. I agree to read, review, and follow all AOG Policies.
Other Policies
I understand that I must schedule gymnastic make-up classes by filling out the class make-up form located on the AOG website, and must have approval in order to attend.
Most AOG students are required to wear uniforms corresponding to their class level. These charges/payments will be done so at the time of registration, prior to their 1st attended class. Parents should make arrangements to pick up uniforms on the day of their child's first class.
Olympic Showcase
Alpha Omega produces an annual "Olympic Showcase", where all students participate and perform their skills/class routines for parents, family, & friends. The Olympic Showcase is typically scheduled over a weekend in April or May of each year. I understand that upon enrollment, my children are automatically confirmed to participate in the Olympic Showcase.
I understand that I will be charged $99 spread out over a three month period, which covers the showcase uniform, awards, etc. I understand and agree to have $33 charged on the following dates: February 1st, March 1st, & April 1st.
I also understand that I will have the opportunity to opt-out of the Olympics Showcase, in the event of travel, etc. I understand that in order to do so, I must complete and submit the Olympic Showcase Opt-Out form (located on the Alpha Omega website) no later than January 25th. I understand that charges will not be refunded once they have been charged.
Camps, Open Gyms, PNO, & Other Events
As a member and where applicable, I understand that I must pre-register for special events such as Open Gym, Gymnastics Skills Clinic, Hip Hop Dance Clinic, DJ Dance Party, etc. A spot is not guaranteed. A no-show admin fee of $10 applies to all membership events. Non-members must pre-register for special events as well.
Some events are included in monthly membership. For those events that require a new charge/payment; I understand that charges/payments will done so when registering for the event. Once payment has been made, no refunds will be issued.

SUMMER CAMP/KID'S CAMP
In addition to all policies above I understand the following policies pertain to Summer Camp/Kid's Camp:
There is a $35 Enrollment Fee for each camper (Waived for AOG Members).
A $35 Non-refundable deposit is required for each week of camp. This is required when registering and reserves each camper's spot.
Each week of Camp is charged on the previous Friday, if not already paid in full with any applicable discounts. No refunds will be given once payment has been made.
