Contract and Consent
As the legal parent or guardian, I release and hold harmless Learn Beyond The Book, LLC, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Learn Beyond The Book, its owners and operators or in route to or from any of said premises, except due to negligence or willful dangerous acts by staff.
The undersigned gives permission to Learn Beyond The Book, LLC , its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health. I request that our doctor/physician listed on file be called and that my child be transported to requested hospital. (Please provide physicians' phone number.)
I have no objection to my child being included in photographs, slides or movies taken at Learn Beyond The Book, LLC that might be used for purposes of interpreting the program or for program publicity. (If you have an objection, please fill out the form in our Santa Clarita office or email LearnBeyondTheBook@gmail.com for a copy to submit from a different location.
I understand that my child may be discharged from any program should I, or my child, fail to comply with the requirements or the policies, or the appropriate requirements, of the management personnel of Learn Beyond The Book, LLC.
I understand that if a class isn't dropped by me in writing by the end of the second week of classes, I am committed to pay for the entire semester.
