Assumption of Risk
In consideration of participating in San Francisco Gymnastics recreational/competitive classes or events, I represent that I understand the nature of this Activity and that I am qualified, in good health and in proper physical condition to participate in such Activity. I acknowledge that if I believe event conditions are unsafe, I will immediately discontinue participation in the activity. I fully understand that this Activity involves risks of serious bodily injury, including permanent disability, paralysis and death, which may be caused by my own actions, or inactions, those of others participating in the event, the conditions in which the event takes place, or the negligence of the "releasees" named below; and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume all such risks and all responsibility for losses, costs, and damages I incur as a result of my participation in the Activity.
Release of Liability
I hereby release, discharge, and covenant not to sue San Francisco Gymnastics, its respective administrators, directors, agents, officers, volunteers, and employees, other participants, any sponsors, advertisers, and, if applicable, owners, and lessors of premises on which the Activity takes place, (each considered one of the "RELEASEES" herein) from all liability, claims, demands, losses, or damages, on my account caused or alleged to be caused in whole or in part by the negligence of the "releasees" or otherwise, including negligent rescue operations and future agree that if, despite this release, waiver of liability, and assumption of risk I, or anyone on my behalf, makes a claim against any of the Releasees, I will indemnify, save and hold harmless each of the Releasees from any loss, liability, damage, or cost, which any may incur as the result of such claim.
Indemnity Agreement
I, the minor's parent and/or legal guardian, understand the nature of the above referenced activities and the Minor's experience and capabilities and believe the minor to be qualified to participate in such activity. I hereby Release, discharge, covenant not to sue and AGREE TO INDEMNIFY AND SAVE AND HOLD HARMLESS each of the Releasees from all liability, claims, demands, losses, or damages on the minor's account caused or alleged to have been caused in whole or in part by the negligence of the Releasees or otherwise, including negligent rescue operations, and further agree that if despite this release, I, the minor, or anyone on the minor's behalf makes a claim against any of the above Releasees, I WILL INDEMNIFY, SAVE AND HOLD HARMLESS each of the Releasess from any litigation expenses, attorney fees, loss liability, damage, or cost any Releasee may incur as the result of any such claim.
Membership
This is a registration charge that includes a class uniform (leotard or T-shirt). Membership only needs to be renewed if the students takes a break from classes for longer then four months. Membership is non-transferable. San Francisco Gymnastics reserves the right to cancel a student's membership at any time.
Insurance Fee
Our policy requires us to insure every student. An annual insurance charge of $40 is due for each student. This is a secondary insurance policy with a $100 deductible. This is required so that every student is covered by the gym's insurance. Insurance is not transferable. (Littlebug classes have the insurance fee included in their monthly tuition.)
Make Up Policies
Any student will be allowed to make up to a maximum of two (2) missed classes per Term for each class a student is enrolled in per week. The class has to be missed first in order schedule a make up. Make up classes need to be scheduled with the SFG Office at least 24 hours in advance. Once scheduled, all make ups are recorded and counted, unless cancelled 24 hours prior to the start of the scheduled make up. Classes must be made up in the same term as the absence. REFUNDS AND CREDITS WILL NOT BE GIVEN FOR MISSED CLASSES.
Parent Responsibility
DRESS CODE - Students are required to wear a leotard (without attached skirts) or a non-baggy T-shirts and sweatpants or shorts. NO skirts, denim, pockets, buttons, clasps or zippers. Long hair must be tied back, no watches or jewelry. Bare feet are required for class. Loaner clothing is available to borrow if you forget your uniform. If loaner clothing is not returned within 30 days, your account will be charged $15.
WARM UPS - All classes begin with a warm-up, which is very important for your child's safety! Please try to have your child here on time. If you have missed the warm-up, please check with the front desk to see if you are still able to join class or if you need to schedule a makeup. Coaches cannot interrupt a class in progress to catch up late students.
PARENT VIEWING - Parents are welcome to view classes at any time. Please view from the lobby area only. Parent's are required to be in the building at all times during the Littlebug, Busy Bee and Grasshopper classes.
SAFETY - Students are expected to wait in the lobby before or after class. Parents are not allowed on the equipment at any time - only the registered student is allowed on the equipment with an instructor present. Parents should walk their children in and out of the building.
BEFORE & AFTER CLASSES - Please arrive in time to park and come into the lobby for drop off and pick up. If you are more than 5 minutes late picking up your child, please sign out at the front counter before leaving. Because we are not staffed for supervising students after class, students who are picked up more than 5 minutes late will be charged a $20.00 emergency babysitting fee - to be charged at the discretion of the office. At no time are children to be left unattended in the facility.
DIGITAL IMAGING- Photographs and Video recording by spectators are not allowed on premises without expressed permission from the manager on site. This is to protect the privacy of all our students.
Skill Session
All members, currently enrolled in class, have access to Skill Practice on the third Saturday of the month from 3:00-4:00 p.m. (check our website for closure dates). Students should wear their class leotard/t-shirt and stick to only the skills worked on in his/her class. Parents must sign their children in and out and refer to our skill session policy. The Littlebugs and BusyBees (ages 1 up to 4) must have a parent on the floor with them. Parent's are not allowed on the floor during older kids skill session (ages 5 and up). Please read the Skill Session Policies before participating.
Image Release
From time to time we take images of our kids in action and like to share them in social media and print/electronic marketing. By clicking this box, you agree to give San Francisco Gymnastics (SFG), the right to take photographs of me and my family while on site for classes. I authorize SFG, its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that SFG may use such photographs of me with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content. Please talk with us specifically if you object, and we will keep your name and info on a separate file.
SFG Gym Safety Rules & Policies
1. No running or horseplay.
2. No disrespectful words or actions towards others.
3. Only Athletes or Gymnasts are allowed on the floor and he/she must be accompanied by an SFG instructor or coach.
4. All release forms must be signed by a parent or legal guardian and payments made before any lessons begin.
5. Hair that can fall into participants face must be tied back and no jewelry allowed to be worn.
6. Follow the dress code - Students must wear movable, form-fitting clothing with no skirts, buttons, zippers, or snaps.
7. No glass containers are permitted on premises. No food or drink allowed on the gymnastics/carpeted areas.
8. No spectators are allowed on the gymnastics carpeted or matted areas.
9. All children and minor spectators MUST have adult supervision at all times.
10. Athletes are to wait quietly while waiting for classes and no horseplay, gymnastics, or dance allowed in waiting areas.
11. No screaming, yelling or loud outbursts from athletes, spectators or students.
12. Parents may NOT coach or teach their child while students are in classes and under the supervision of SFG staff.
13. Children/students may not leave premises or go outside without a parent or guardian.
14. Visitors for students, camps, field trips or birthday parties may only view from observation area and are not allowed on the gymnastics, carpeted or matted areas.
15. Athletes/Students wearing shoes must be sure shoes are clean and have white soled shoes only. Littlebug parents will remove shoes.
16. Students misbehaving or acting in a unsafe or disrespectful manner, will be asked to sit out or asked to go home.
17. Students must be dropped-off no earlier 10 minutes prior and picked up within 5 minutes from the end of the lesson. Any student remaining after that time will be charged a $20 emergency babysitting fee (at the discretion of the office).

19. SFG reserves the right to refuse service or cancel a student membership at any time.
Refunds
SFG offers no refunds unless a class is cancelled due to under enrollment. In the case of cancellation, students may transfer to another appropriate class. If your schedule doesn't permit a transfer from a cancelled class, you may request a refund from the front office. When you register for a class, it is yours for the term; it is not possible to substitute siblings or friends in place of a registered student.
Credit for the balance of tuition fees is occasionally issued (less a $25.00 processing fee) if a child is having difficulty adjusting to a class (and has made an effort to participate for at least 2-3 classes), or is suffering from an extended illness or injury. We do not offer credit if a class needs to be dropped due to a change in a child's or parent's schedule.
Camp Refund: If you notify us that you need to drop from a Camp week prior to two weeks before the start date, we will refund your tuition minus a $25 processing fee. No Refunds if you are within two weeks of the first day of camp.

Payment Policies
Payments are due PRIOR to the start of the new Session. Tuition statements will only be send out by e-mail. There will be Discount Tuition Deadline of $6.00 for pre-enrolling and pre-paying per student. There is a $20.00 late fee per child, or $30 per family if payment are paid AFTER the previous session ends. See session calendar for discount tuition deadline dates.
