Supplemental COVID-19 and any Contagious Disease Waiver/Liab
As a participant or as a parent/guardian of a participant of Dance Theatre of Jacksonville dance programs, I recognize and acknowledge there are certain risks of physical injury and I agree to assume full risk of any injuries, including damages or loss which may be sustained as a result of participation in activities associated with Dance Theatre of Jacksonville. I agree to waive and relinquish all claims against Dance Theatre of Jacksonville and its faculty members from any and all claims resulting from participation in the programs offered. I acknowledge the contagious nature of COVID-19 and any other contagious diseases and viruses and voluntarily assume the risk that I and/or my children may be exposed to or infected by COVID-19 by attending and participating and that such exposure or infection may result in personal injury, illness, disability, and/or death. I understand that the risk of becoming exposed or infected by COVID-19 and other contagious diseases and viruses, as always, may result from the actions, omissions, or negligence of myself and/or others, including, but not limited to, employees, volunteers, and program participants and their families. I have read and understand this policy and agree to assume these risks and liabilities.
Tuition Policy
Your total monthly tuition rate is due for the months of Aug-May, regardless of the number of classes in those months. August tuition will be charged at ½ your regular rate. Payment is due on the first of each month and is considered late after the 15th (even if the 15th falls on a weekend or holiday). There are NO EXCEPTIONS to this policy. There will be a $10 late fee charged on any balance not paid by the 15th of each month. Tuition more than 45 days overdue will be cause for dismissal from class. There is a $15 fee for returned checks. Tuition will continue to be charged until a drop notice (available at the front desk) is filled out and signed by a parent or guardian.
Attendance Policy
Please notify your teacher or the secretary if your child will be absent for several weeks. If a student misses 3 or more classes without notifying the studio and has a delinquent account, the student will be dropped from the class. Make up times are available for CERTAIN classes; please see your child's teacher to find out if this is a possibility.
Dress Code
*HAIR MUST BE SECURED OFF THE FACE FOR ALL CLASSES*
Rhyme, Rhythm & Motion - Play Clothes.
Baby Ballet - Black leotard and pink tights. Pink leather ballet shoes. Skirts and tutus are permitted.
Tap/Ballet Combination - Black leotard and pink tights, pink leather ballet shoes and black tap shoes. Please put elastic fasteners in tap shoes and your child's name in each of the tap and ballet shoes. Skirts and tutus are permitted.
Ballet - Black leotard and pink tights. Pink canvas ballet shoes. Hair always in a bun for ballet, please.
Lyrical - Any appropriate dance attire
Jazz - Black leotard and tan tights. Tan slip on jazz shoes. Dance shorts and skirts are permitted.
Tap - Black leotard and tan tights. Tan tap lace up shoes. Dance shorts and skirts are permitted.
Hip Hop - Loose comfortable clothing. No jeans, leotards or booty shorts. Tennis shoes or dance sneakers.
Musical Theatre - Any appropriate dance attire.
Salsa - Any appropriate dance attire and black jazz shoes.
Waiver
We the staff of Dance Theatre of Jacksonville (DTOJ) recognize our obligation to make our students and their parents aware of the risk and hazards associated with the sport of dance. Students may suffer injuries, possibly minor, serious or catastrophic in nature. DTOJ staff members will not accept responsibility for injuries sustained by any student during the course of dance instruction, open workouts, or in the course of any exhibition, competition, or clinic in which he or she may participate or while traveling to or from the event.
I fully understand that staff members of DTOJ are not physicians or medical practitioners of any kind. With the above in mind, I hereby release DTOJ staff to seek medical help, including transportation by a DTOJ staff member and/or its representatives, whether paid or volunteer, to any health care facility or hospital, or the calling of an ambulance for the child named below, should DTOJ deem this to be necessary.
With the above in mind and being fully aware of the risks and possibility of injury involved, I consent to have my child participate in the program offered by DTOJ. I, my executors or other representatives, waive and release all rights and claims for damages that I or my child may have against DTOJ and/or its representatives whether paid or volunteer.
With this signature I also acknowledge the tuition and attendance policies of DTOJ.
I give my permission for DTOJ to use any photos/video taken in conjunction with our classes or performances
I am aware that DTOJ does report to the Credit Bureau and understand that if my account is not paid as expected or arrangements made to pay are not made then I will have a negative filing on my credit report from DTOJ.
