Tuition
All class tuition, recital and costume fees are non-refundable.
Release of Liability
I understand and agree that in participating in any dance class, workshop, rehearsal or performance, there is a possibility of physical injury or death. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me or my child during any of The Davy Dance Academy classes, rehearsals, performances, or activities. I also exempt, release, and indemnify The Davy Dance Academy, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my children, or property which may arise out of or in connection with participation in any classes or activities conducted by The Davy Dance Academy. I further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold The Davy Dance Academy, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students liable for such damage, loss, injury, or death. I understand that I should be aware of my physical limitations and agree not to exceed them. If I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights.
Permission is granted The Davy Dance Academy to use photographs of students for publicity purposes.

Withdrawl Policy
Withdrawal requests must be made in writing by the 15th of the month in order to stop tuition payment for the following month. If we do not receive a notice in writing by the 15th, you will be responsible for the next month's tuition. There are no exceptions to this policy.

As the legal parent or guardian, I release and hold harmless Davy Dance Academy, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Davy Dance Academy, its owners and operators or in route to or from any of said premises.
Medical Emergency
The undersigned gives permission to Davy Dance Academy, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restictions, or condition and/or declare the paricipant to be in good physical and mental health. I request that our doctor/physician ________________ be called and that my child be transported to ______________________ hospital. Please include physicians' phone number _______________.
Student Handbook
I understand that dance classes may include, without limitation, dancing with props, stretching, barre work, across the floor combinations, dance routines in the center, and other related activities. I further understand that all of the activities of the dance class involve some degree of risk of strain or bodily injury. Davy Dance Academy is not responsible for personal property.

I agree to be responsible for reading studio correspondence and respecting deadlines, if applicable.
I hereby acknowledge that I have read the statements above and agree to participate accordingly.
Payment Information
Tuition is due by the first of each month. If accounts are paid after the 5th of the month, there will be a 10% late fee applied to the account balance. There is a $25.00 returned check charge for any checks returned by the bank. Auto-bill is available for monthly tuition and recital/costume fee payments. To sign-up for Auto-bill please complete the form you receive you in your registration packet and turn it into the studio.
Covid-19 Policy
At Davy Dance Academy, we are following strict social distancing and cleaning protocol.
As any other public place, we cannot guarantee that your child will not be exposed
(whether at Davy Dance Academy or elsewhere) or if exposed, that your child will not
contact the disease.
By signing below, you agree to waive any claims of liability against Davy Dance
Academy and to hold Davy Dance Academy harmless if your child contracts COVID-19
despite our best efforts to prevent it.
Additionally. I agree to notify Davy Dance Academy if my child or any member of our
household contracts COVID-19 or has come into contact with someone who tested
positive for COVID-19. This action will help ensure the utmost safety of our dancers and
staff.
Class Tuition Policy
You are registering for a studio dance class. In the event of a required Federal/State/Local stay at home order due to a pandemic or other natural disaster we will run your class via Zoom (or other online streaming platform) and there will be no refunds or tuition prorating.
