Inconsideration of participating at MIDWEST TRAINING & ICE CENTER ("MTIC") I represent that I understand the nature of this activity and that my child is qualified, in good health and in proper physical condition to participate in such activity. I acknowledge that if I believe event conditions are unsafe, I will immediately discontinue participation in the activity. I fully understand that this activity involves risk of serious bodily injury, including permanent disability, paralysis and death, which may be caused by the students own actions or inactions, those of others participating in the event, the conditions in which the event takes place or the negligence of the "releasees" named below; and that there may be other risks either known or not readily foreseeable at this time. I fully accept and assume all such risks and all responsibility for losses, costs and damages incurred as a result of student participation in the activity.
I hereby release, discharge and convenant not to sue MIDWEST TRAINING & ICE CENTER, it's respective administrators, directors, agents, officers, volunteers, employees, other participants, any sponsors, advertisers, and if applicable owners a lessors of premises on which the activity takes place, ( each considered on the "RELEASEES" herein) from all liability, claims, demands, losses, or damages, on students account caused or alleged to be caused in whole or in part by negligence of the "releasees" or otherwise, including negligent rescue operations and future agree that if, despite this release, waiver of liability and assumption of risk, student, or anyone on student's behalf makes a claim against any of the releasees, students/parent/guardian will indemnify, save and hold harmless each of the releasees from any loss, liability, damage or cost, which make occur as the result of such claim.
I have read this RELEASE AND WAIVER of LIABILITY, ASSUMPTION of RISK AGREEMENT, I understand that I have given up substantial rights by signing it and have signed it freely without inducement or assurance of any nature and intend it to be complete and unconditional release of ALL liability to greatest extenet allowed by law and agree that if any portion of this agreement is held to be invalid the balance, not withstanding, shall continue in full force and effect.
I, the minor's parent/legal guardian, understand the nature of the above referenced activities, the minor's experience, capabilities, and believe the minor to be qualified to participate in such activity. I herby release, discharge, convenant not to sue and agree to indemnify, save and hold harmless each of the releasees from all liability, claims, demands, losses, or damages on the minor's account caused or alleged to have been caused in the whole or in part by the negligence of the releasees or otherwise. This includes negligent rescue operations and further agree that if, dispite this release, I , the minor or anyone on the minor's behalf makes a claim against any of the above releasees, I will indemnify, save and hold harmless each of the releasees from any litigation expenses, attorney fees, loss liability, damage or cost any releasee may incur as a result of any such claim.
I, hereby, give my consent to MIDWEST TRAINING & ICE CENTER to provide, through a medical staff of it's choice, customary medical/athletic training attention, transportation, and emergency medical services as warranted in the course of my chid's participation or mine.

By entering this facility, you are aware that you agree to fully accept all known and unknown risks, including the potential risk of exposure to respiratory illnesses such as the coronavirus (COVID-19). The coronavirus is primarily transmitted via exhaled respiratory droplets, most often through coughing and sneezing. These droplets can travel up to six feet and are more commonly transmitted between persons rather than from equipment to persons.
Although we regularly sanitize our equipment and presently are using enhanced cleaning methods and enforcing social distancing in our facility, you understand that you may be exposed to the coronavirus or its symptoms through no fault of our own. Known coronavirus symptoms include fever, coughing, shortness of breath, pneumonia, kidney failure, and may include other symptoms, stroke or even death (collectively "Symptoms"). You understand and agree that you will hold us harmless and you will not hold us liable for any real or perceived Symptoms of COVID-19 or any other disease, illness, or condition, nor for exacerbating any existing symptoms, and you fully agree to accept all risks of entering the facility, using the equipment, working with personal trainers, attending classes, and/or interacting or being exposed to other members.
There is an ANNUAL REGISTRATION FEE of $25.00 for the first child, and any additional children will be charged at $15.00 each, so long as the first child remains enrolled. The registration fee is NON-REFUNDABLE/NON-TRANSFERABLE and must be paid at the time of enrollement along with the first month's tuition. Midwest Training does not profit from registration fees. They cover the administrative and insurance costs associated with new athletes.
TUITION - is paid monthly and is due on the FIRST DAY OF EACH MONTH. A $30.00 late fee will be added, if tuition is NOT received by the 10th of the month. Tuition can be paid at the front desk, by mail, by phone, or online. We accept cash, checks (Payable to MTIC), as well as MC and VISA. MTIC also offers an automatic tuition payment plan. Any unpaid balances including tuition, late fees, and/or other unpaid charges not paid by the 10th of the month will be charged to the Credit Card on file.
Midwest does not mail statements, and it is the Parent/Guardian's responsibility to keep their account in good standing.
RETURNED CHECKS are subject to a $50.00 insufficient funds charge which will be added to your account.
DECLINED CREDIT CARDS are subject to a $15 insufficient funds/declined transaction fee will be added to your account.
CLASSES BEGIN PROMPTLY at the scheduled time. It is very important that you understand for the safety of the child that they receive proper warm up and stretching activity that is taught in the first 15 minutes of each class. Safety will ALWAYS come first.
VISITORS and parents are required to watch from the parent viewing area. Your child has a coach, please do not disturb class by waving or calling your child over to talk. This is very disruptive and unsafe. Distracting the students can result in serious accidents. Visitors will be asked to leave the facility if the disruption continues.
BEHAVIOR of the students will be in an orderly manner and for them to conduct themselves with respect to their classmates, instructors, and facility. Misbehaving students will receive a warning and then a meeting with the parents. If future instances occur the student may be asked to sit out of the class, and finally be asked NOT TO RETURN.
MEMBERSHIP - MTIC has the right to cancel a class IF THERE ARE NOT ENOUGH STUDENTS ENROLLED IN THAT PARTICULAR CLASS.
DRESS CODES - are for the safety of the child, we require our students to wear leotards and tight fitting shorts in the gym and proper clothing for the ice. This assures the coaching staff to be able to properly spot the student during situation. NO JEAN SHORTS, belts, jewelry, bobby pins or other accessories will be allowed for the safety of the child. Boys may wear shorts and t-shirts in the gym, and on the ice proper clothing should be worn at all times. Socks, Gymnastics Slippers, or Clean, Indoor Only Parkour/Cheer Shoes (for 2 Hour Tumbling Class- may be worn on Cheer Floor Only) MUST be worn to class. If your child attends class without proper attire they will NOT BE ALLOWED to participate and you will be notified.
VALUABLES - should not be brought to gym. Midwest Training & Ice will not be held responsible for lost or stolen items.
