Release of Liability
The adult Student or Parent, acknowledges they/their child are/is taking class at their own risk and agrees to hold Backstage Dance Studio, its instructors, and/or assignees harmless in the event of any physical injury to student or loss of personal property. Signer is aware that there are inherent risks associated and accompanied with the activity registered for and that the child may be injured as a result of an accident arising out of participation in the activity. All students are required to be covered by their own insurance policies. If any injury occurs, it is understood that the student's own policy is the only source of reimbursement.
Communicable Disease Release of Liability
Backstage Dance Studio is following the guidelines provided by our state and local authorities, and we have put in measures and protocols in order to reduce the spread of COVID-19. But we cannot guarantee that any of our dancers, parents, or staff will not come into contact with, or become infected with, COVID-19. You acknowledge that by participating in classes and/or activities at Backstage Dance Studio you are voluntarily assuming the risk of infection.
You also understand and agree that by participating in any Backstage Dance Studio classes and/or activities (or allowing your child to participate), you are releasing and waiving Backstage Dance Studio from any and all claims, demands, suits, causes of action, losses, and liability of any kind whatsoever, whether in law or equity, arising out of or related to any illness, injury, disability, death, or other damages incurred due to or in connection with any communicable diseases (including COVID-19) arising out of or related to Backstage Dance Studio classes and/or activities, WHETHER ARISING FROM THE ACTIONS, OMISSIONS, OR NEGLIGENCE OF THE RELEASED PARTIES OR OTHERWISE, to the fullest extent permitted by law.
The undersigned hereby agrees, represents and warrants that neither the undersigned nor such participating children shall visit or utilize the facilities, services or programs of Backstage Dance Studio if he or she (1) experiences symptoms of COVID-19, including, without limitation, fever, cough or shortness of breath, or (2) has a suspected or diagnosed/confirmed case of COVID-19. The undersigned agrees to notify Backstage Dance Studio immediately if he or she believes that any of the foregoing access/use restrictions apply.
Concussion Policy
Any dancer suspected of suffering a concussion should be removed from the class or performance immediately. No dancer may return to activity after an apparent head injury or concussion, regardless of how mild it seems or how quickly symptoms clear, without medical clearance. Close observation of the dancer should continue for several hours. The new "Zackery Lystedt Law" in Washington requires the consistent and uniform implementation of long and well-established return to play concussion guidelines that have been recommended for several years: "A youth athlete who is suspected of sustaining a concussion or head injury in a practice or game shall be removed from competition at that time" and"...may not return to play until the athlete is evaluated by a licensed health care provider trained in the evaluation and management of concussion and received written clearance to return to play from that health care provider".
You should also inform your child's instructor if you think that your child may have a concussion. Remember, it's better to miss one event than miss the whole season. And, when in doubt, the dancer sits out. For current information on concussions, see: http://www.cdc.gov/ConcussionInYouthSports/
Medical Emergency
In the event of medical emergency, every effort will be made to immediately reach the primary account contact. If unable to make contact, Parent's acknowledgement here authorizes Backstage Dance Studio to call 911.
Publicity Release
Signer is aware that Backstage Dance Studio may take photos/videos of students for the purpose of promoting the Studio, recording performances or documenting progress.
Payment Information
Tuition is based on STUDENT classes per week and is paid by the Month. Discounts apply (per class) for third class enrolled in (5%); fourth class enrolled in (10%); fifth class enrolled in (20%); sixth+ class enrolled in (35%). These discounts are applied on a per-student basis, not per-family. There is an annual Family Registration Fee of $35 due at time of registration. All accounts being billed on a monthly basis will automatically be set to auto-pay, using the credit card provided upon registration. Auto-pay accounts are processed on the first business day of every month. Tuition is considered late after the 10th of the month, and a $20.00 late fee per family will be charged to your account if past-due. WE DO NOT BILL UNLESS AN ACCOUNT BECOMES DELINQUENT.
Non-refundable registration fee and first month's tuition is due upon registration for the 2020-2021 School-Year Session.
There is a $25.00 fee for any checks returned by the bank.
Cancellation/Drop Policy
Written notice of intent to cancel enrollment is required two weeks in advance of intended end date. There is no refund for tuition due, or paid, in the month (or mini-session) the class is dropped. Drop forms are available at the studio front desk, or you may email us at dancewithus@backstagedance.org.
If your account becomes 61 days past-due, your dancer(s) will be automatically dropped from all classes. Dancers with four consecutive absences in an individual class may be automatically withdrawn. Regardless of dancer(s) attendance, tuition for the months enrolled is still owed, NO exceptions.
Dress Code
Proper dance attire enables the student to fully participate in class and to maintain a disciplined attitude towards dance studies. The Backstage Dance Studio Dress Code is posted online at http://backstagedance.org/about-us/dress-code/, and dancers not dressed in accordance to these requirements will not be allowed to participate in class or performances.
