Little Scholars Handbook Confirmation
I have received a copy of the Little Scholars Montessori Program Handbook. I have read all of the information provided in Little Scholars Montessori Program Handbook and agree to abide by all of the policies, rules and regulations of Little Scholars Montessori.
Licensing Summary Confirmation
I have received a copy of the Licensing Summary provided by the State of Tennessee. A hard copy is provided in the Little Scholars Montessori welcome packet during scheduled tours. Additionally, a downloadable version of the Licensing Summary is available on the Little Scholars Montessori website at http://www.littlescholarstn.com/information-for-parents.html.
Abuse and Neglect Reporting
I, the undersigned, understand that the Little Scholars staff is required to report any suspected child abuse, neglect or exploitation to Child Protective Services (CPS) or our local law enforcement agency immediately. When any licensed or certified child care provider or their employees has reasonable cause to believe a child has suffered abuse or neglect, he or she is required to report such incident, or cause a report to be made to the proper law enforcement agency. A complete documentation of abuse incidents will be reported and remain confidential. Little Scholars School is not required to inform the parents of this reporting.
Little Scholars Montessori is required by the state of Tennessee to provide Abuse and Neglect training for parents and staff. Little Scholars Montessori has made this training available on the Little Scholars website or at http://lms.csw.utk.edu/login.aspx. I, the undersigned, agree that Little Scholars Montessori has provided to me, the undersigned, the ability to participate in the above referenced training for reporting of abuse and neglect.
Infectious Disease
I further acknowledge, understand, appreciate, and agree that attendance at Little Scholars Montessori may result in possible exposure to and illness from infectious diseases, including, but not limited to, MRSA, Influenza, and COVID-19. While health policies and personal discipline may reduce this risk, the risk of serious illness and death does exist. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releases or others, and assume full responsibility for my enrolled student(s) and exposure.
Authorization for Medical Care
I, the undersigned, understand that Little Scholars Montessori will provide notification of any accident or illness to my child, and will make arrangements for medical care of my child with the physician or hospital of my choice. I authorize Little Scholars Montessori to administer minor first aid procedures as needed for the above listed child. I understand that if either parent or emergency contacts for my student cannot be reached to make the necessary arrangements, or in a critical emergency requiring medical care, I hereby authorize Little Scholars Montessori to call local Emergency Management Services (911). I, authorize Little Scholars Montessori to obtain emergency transportation to and/or emergency medical care for the above listed child at the nearest hospital's emergency room, or at the emergency room the EMS/Ambulance Service is required to transport patients to at the time of emergency. I agree to update the emergency contact information for my student when contact listing changes occur or every six months at a minimum.
Payment Policies
I, the parent/guardian of the above listed child, hereby acknowledge that I am entering into a Contract for Child Care Services/Fee Agreement with Little Scholars Montessori. The Little Scholars Montessori program agrees to provide child care services for my child in accordance with the mission, philosophy and policies outlined in the Little Scholars Program Handbook. I agree to compensate Little Scholars Montessori for the above listed services as follows: Monthly tuition paid in full prior to the month of child care services rendered.
Tuition is due on the first of each month prior to attendance (post-dated checks are accepted.) If the 1st falls on a weekend or holiday, tuition is due before the 1st of the month. Accounts receiving 3rd party funds applied to monthly tuition including, but not limited to CCAoA, Essential Employees Funding, Fold of Honor, ect must maintain a zero balance by the 1st of each month. The primary billing contact will be responsible for ensuring any remaining account balance is paid in full by the 1st of each month.
Little Scholars does not prorate tuition and does not give credits for any absences, extended breaks, vacations, or emergency closures during the enrollment period.
Late Fees:
Accounts with an outstanding balance as of the 2nd of each month will be assessed a $25 late fee. Students with an outstanding account balance of 10 days or more will not be permitted to attend Little Scholars Montessori until the balance is paid in full. Interest will be charged on all unpaid balances.
Return Payment Charge:
A $25 Return Charge will be assessed for all returned checks or ACH bank drafts.
Tardy Fee:
A drop-in rate of $15.00, per occurrence, will be added to the student's billing account if they arrive earlier than 8:15am or stay later than 3:00pm and are not currently enrolled in the applicable Extended Care Program. Students enrolled in the after school extended care program must be picked-up by an authorized person no later than 5:30pm. If pick-up occurs after 5:30pm for students enrolled in After School Care, a late pick up fee of $1.00 per minute will be billed to the parents/guardian's account.
Enrollment Schedule Changes:
I understand that any requests for changes to the enrollment schedule listed herein must be made in writing and submitted to the Little Scholars Administrative Office. Approved enrollment schedule changes will require the execution of a new Contract for Child Care Services/Fee Agreement and payment of any associated account charges. Little Scholars reserves the right to deny any request for schedule change for any reason within its sole discretion.
Summer Fees:
Students not enrolled in the summer program, but plan on returning to Little Scholars for the next academic year, will be required to pay a $150 monthly hold fee (June and July) per student to guarantee enrollment for the upcoming school year. Students who have not remitted summer hold fees for June and July will be placed at the bottom of the waitlist, and school semester enrollment will not be guaranteed by Little Scholars.
Withdrawal Procedure:
A formal notice of intent to withdraw must be provided to the Little Scholars administration two weeks prior to the effective termination of services date. Accounts will be billed for services through the notification period, regardless of student attendance. If formal notice of intent to withdraw is not provided to Little Scholars, the associated billing account will continue to accrue monthly tuition charges until notification is received or the student is administratively removed from the Little Scholars program. If proper notice of intent to withdraw is received, any tuition amount exceeding the cost of services through the effective date will be refunded within 30 days of the withdrawal. Formal notice of intent to withdraw must be submitted to office@littlescholarstn.com.
This Contract for Child Care Services/Fee Agreement may be cancelled by Little Scholars Montessori at any time with or without notice, in its sole discretion. Any tuition amount exceeding the cost of services through two weeks from the effective termination of services date will be refunded within 30 days of the withdrawal.
Registration, curriculum and field trip fees are non-refundable.
I confirm that I have had the opportunity to discuss the information contained herein with a representative of Little Scholars Montessori, that my questions have been answered fully and to my satisfaction and that I agree to abide by the conditions set forth herein as well as the policies contained herein by reference from the Little Scholars Montessori Program Handbook. I/we agree to update this agreement whenever changes occur or every six months at a minimum.
Photo Release
Occasionally Little Scholars Montessori uses photos or video of students in print ads, on official LSM social media outlets, the LSM website, commercials, or other marketing mediums. I understand that my child's likeness may be used in such advertising. These images will not be given or sold to outside companies or individuals. The LSM Administrative Office must be notified of requests for exceptions to this policy.
Personal Safety Curriculum
Every state licensed school is required to teach a personal safety curriculum to all enrolled students. Little Scholars Montessori utilizes the state approved "Keeping Kids Safe" program to meet this requirement. The purpose of the program curriculum is to improve the knowledge, self-confidence and assertiveness skills of the children. The instructional content includes: self-esteem, feelings, problem solving, personal safety, and safety around me.
Little Scholars Montessori has provided the opportunity for content review, means of implementation, purpose, and goals of the personal safety program available on the Little Scholars Montessori website. I, the undersigned, acknowledge that Little Scholars has provided the opportunity for reviewing the aforementioned personal safety curriculum information, and GRANT permission for my child(ren) to participate in the personal safety curriculum implemented at Little Scholars Montessori.
The LSM Administrative Office must be notified via email office@littlescholarstn.com with denial of permission for student participation in the personal safety curriculum provided by Little Scholars Montessori.
Consent for Technology Use
Little Scholars Montessori uses a variety of technology and digital resources to enable and enhance instruction. With parent consent as indicated below, students may use physical devices, including but not limited to, computers, tablets, cameras, and television (all of which allow some degree of Internet access). Students are expected to use technology and digital resources in accordance with the Little Scholars Technology policy. Parental permission is required in order for your student to access technology and digital resources at school. I, the undersigned, GRANT permission for my student to use all technology and digital resources, both devices and web-based applications.
The LSM Administrative Office must be notified via email office@littlescholarstn.com with denial of permission for student technology usage.
