Release of Liability
I, the minor's parent and/or legal guardian, understand the nature of these activities and the minor's experience and capabilities and believe the minor to be qualified, in good health, and in proper physical condition to participate in such activity. I hereby release, discharge, covenant not to sue, and agree to indemnify and save and hold harmless each of the releasee's from all liability, claims, demands, losses, or damages on the minor's account, including negligent rescue operations, I further agree that is, despite this release, I, the minor, or anyone on the minor's behalf makes a claim against any of the releasees named above, I will indemnify, save, and hold harmless each of the releasees from any litigation expensed, attorney fees, loss liability, damage, or cost any may incur as the result of such claim.
Exposure to infectious disease
I acknowledge, understand, appreciate and agree that my participation may result in possible exposure to and illness from infectious diseases, including, but not limited to, MRSA, Influenza, an Covid-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist. I knowingly and freely assume full responsibility for my participation and exposure.
Membership Fee
Each student is charged a $46 membership fee to enroll in classes. The membership fee is due annually thereafter based on the students registration date.
Year Round Program
We are a year round program with a monthly tuition based on an average of four classes per month. If there are five classes in a month, we do not increase tuition. If there are three, we do not credit.
Enrollment Defined
Upon enrollment your space is reserved in the class(s). Enrollment is continuous and must be cancelled when you want to withdraw. Tuition reserves your space in the class(s); it does not guarantee a certain number of classes.
Payment Policies
Tuition is due by the 25th of each month to reserve your child's spot in the class for the following month. Tuition is accepted through our web site, by mail, by phone, or in person. We accept payment in the form of cash, check, bank draft, debit or credit card. A debit card, credit card or bank account is required to enroll and will be charged automatically if no payment is received before the 25th of the month. All returned checks or failed bank drafts will carry a fee of $25.00. Once we have taken a bounced check or declined bank draft all tuition must be paid from then on by cash or debit/credit card.
Late Fees
A late fee of $10 will be assessed for all accounts paid after the 25th of the month. The late fee will also apply to all declined debit card, credit card and bank account transactions set up for auto pay.
Written Notice to Withdraw
Written notice to withdraw from classes is due before the 25th of your last month. Notice is required each time your child decides to take a break from classes. If no withdrawal notice is given, your debit card, credit card or bank account on file will be charged automatically for the next month's tuition.
Students may be automatically withdrawn from class for the next month if tuition is not received by the last day of the month prior to classes starting. Students may also be automatically withdrawn if they have 4 consecutive absences with no contact with the front desk.
Make-up Policy
If your child will be absent, we will allow a make-up in a similar class if you call us no later than 9:00 AM the day of his/her class. If you call us after 9:00 AM or not at all, the student may attend Open Gym at no charge to make up for the missed class. All make-ups are required within 30 days of the missed classes, must be pre-scheduled with the office, cannot be scheduled before the class is missed and are not to be taken before the missed class. Make-ups are void once a student has withdrawn from classes. Account credits are not given for missed classes. I have been given a copy of the make-up policy for my records.
Photo Release
I authorize Gymnastics Nevada to use any photographs that contain images of myself or any of my gymnast children for any purpose for furtherance of the business of Gymnastics Nevada, including, but not limited to, inclusion of any such images in advertisements, on websites, or on posters. I further understand that no personal information will be associated with any such images, including, but not limited to, name, age, address, school affilition or any other identifying information, without my prior written consent.
Holiday Closures
Gymnastics Nevada observes the following holidays and does not hold classes: New Years Day, Memorial Day, Independence Day, Labor Day, Halloween, Thanksgiving and Family Day, and Christmas Break ( 2 weeks). We do not prorate or credit tuition, or offer make up classes for these closures.
No Refunds
Gymnastics Nevada does not offer refunds or credits.
