By signing this agreement I hereby consent to have my child(ren) participate in sports or activities, including but not limited to cheerleading, dance, tumbling, fitness, private lessons, birthday parties, open gyms, camps, special events, and any and all other activities offered by United Talent. The risks involved in respect to such activity are fully understood. It is hereby agreed that I, my child(ren)/adopted or otherwise, my executors, waive and release all rights and claims for damages that I or my child(ren) may have at any time against United Talent and their representatives whether paid or volunteer, for any injury, permanent disability, or death in connection with all of United Talent's programs, or other activities related to United Talent.
1. I understand I must have a valid credit or debit card on file for monthly tuition payments. Once enrolled, I understand my credit or debit card on file will be charged for tuition in equal monthly installments for the duration of the season. My card will be charged for tuition automatically on the 1st or 15th of each month depending on what I choose to set up with the front office. Because tuition payments will be made automatically on the due date, I will never incur Late Fees on my account, however if my card on file declines for any reason, I am subject to a $25.00 Declined Card fee. I am responsible for keeping a valid credit or debit card on file at all times. I further understand that if tuition is not paid before the 16th of the month, my child(ren) will NOT be able to participate in class or practice until my balance is paid. I will have a period of 10 days to bring my account current before my child(ren) will be dropped from their class or team. If dropped, I am subjected to the cancelation policies stated below.
2. I understand class tuition cannot be prorated due to my child being absent from class or practice for any reason. There are no credits or refunds for missed classes or practices. All sales are final. Any credit on my account must be used within one year of the issued date. I understand Class & Team tuition is based on the number of months required to participate in my child(ren)'s program (See cancelation policy below.) Tuition for classes is averaged on four (4) classes or per month. United Talent does not charge more for months that include 5 weeks, and will not prorate for months that include less than 4 weeks. Holidays or closings have been equated in my child(ren)'s monthly tuition cost (Please see the holiday/closing schedule provided each season.) Make-up classes will only be available for closings due to extreme inclement weather .
3. I understand this agreement only depicts charges related to tuition and annual membership fees. I am fully aware that other cost may be associated with various programs United Talent offers, including but not limited to competition fees, uniforms, costumes, choreography, coach's fees, practice attire, shoes, and any and all other expenses related to a specific program. (See program handbooks/welcome packets for complete details.) I agree to pay all cost associated with the program. If I do not pay the various other charges related to the program by the specified due dates, United Talent will charge my credit or debit card on file. If at any point additional costs are not being paid on my account, my child(ren) will be dropped from their class or team. If dropped, I am subject to the cancelation policies stated below. Note: Additional cost can be paid with cash, check, and credit or debit cards, but must be paid on specified due dates to avoid your card on file from being charged.)
