Required Policies and Agreements
As the legal parent or guardian, I release and hold harmless Charlottesville Ballet, its Board of Directors and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Charlottesville Ballet, its Board of Directors and operators, or in route to or from any of said premises.

Regular, punctual attendance is important to a dancer's progress!
*Parents/Guardians must report all absences prior to class time. Parents may call the studio at 434.973.2555 or email info@charlottesvilleballet.org.
*Students are urged to make up any classes that are missed in another age-appropriate class of the same level. Make-up classes must be completed within 60 days of missed class. No refund or credit is given for missed classes.
*If the student is ill or contagious, they should remain home and rest. If the student is injured and cannot actively participate in class, they should sit in class to observe. Much can be learned from observation and the student keeps up with his/her classmates.

*First Steps & Children's Division:
All children under age 11 must be accompanied by a parent or guardian on the elevator at all times.
*Student Division:
Dancers over age 11 may be dropped off at the elevators. Please ensure that your child is on the elevator before departing. During pick up after class, students age 11 & up may wait in the upstairs hall for a parent or guardian or in the downstairs lobby.
*It is very important that parent/guardians pick up children in a timely manner. Students must be picked-up within 10 minutes after their last class. If you are running late, please call the office immediately.

In cases of inclement weather:
*The Academy telephone message (434.973.2555) will be changed to reflect any changes in class schedules due to weather and an email will be sent.

Charlottesville Ballet is hereby granted permission to take photos and videos of the students to use in brochures, studio website, posters, social media advertisements, or other promotional material. Permission is also hereby granted for Charlottesville Ballet to copyright such photos and videos.

The undersigned gives permission to Charlottesville Ballet, its owners and staff to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health.

ACADEMIC YEAR 2019-2020 Charlottesville Classes Only: The Academic Year Session is September 3, 2019 to May 30, 2020. December 15, 2019 is the last day to drop or transfer classes for Children's Division and Up. A drop fee of $100 will be charged for each class dropped after this date. No refunds will be made. April 1st is the last day to enroll or transfer classes for First Steps Division.
The first month's tuition & an annual registration fee of $20 is due upon enrollment. Monthly tuition is due on the 15th of the month prior (ex - Oct tuition is due Sept 15). A costume rental fee of $35 per class (except First Steps Division & Adult Classes) will be charged on November 1st (if registering after November 1st, this fee will be charged with the first month's tuition). This fee is put towards the rental of the student's costumes for the Spring Performance. No refunds will be made. A Spring Performance Fee of $50 per student ($25 per sibling) will be assessed for each student at CBA that participates in the Spring Performance (due March 1). Tickets will be FREE for the Spring Performance.

SHORT SESSION CLASSES & SATELLITE CLASSES: Please see dates listed for each individual class. These classes pay 1 tuition fee and an annual registration fee upon enrolling.

*Charlottesville Ballet Academy accepts cash, checks, and credit cards (Visa, MasterCard, Discover). Checks can be made payable to "Charlottesville Ballet."
*Credit Card payment is required when registering online. Tuition will be charged to the card at time of enrollment (after applicable discounts are applied). An email receipt will be sent after payment is processed.
*A late fee of $10.00 will be assessed if tuition is not paid within five days of the listed due date; any returned checks will carry a fee of $25.00.
*Please note that all Trial Classes are FREE and credit cards will not be charged for the single-time placement Trial Class.

I authorize the use of the below listed credit card to pay any and all fees due Charlottesville Ballet. I understand that the credit card transaction is for the amount indicated in Payment Policies above. I understand that I may be charged a penalty fee if the credit card company denies my credit card. I understand that a facsimile or photocopy of this form with my signature on it is the same as an original. My signed authorization will remain in effect until I revoke in writing this authorization.
