The participant and/or the parent/guardian of the participant (if a minor) also hereby agrees to release, discharge and hold harmless from any and all liability Starfish Aquatic Club, Inc., its coaches, instructors, officers, directors, agents and employees and Fitness International, LLC, its affiliates, and their respective successors, related entities, directors, officers, employees, and agents for any damages or losses incurred arising from any claims, demand, action or cause of action by the participant resulting from any injury, including death, that may occur to the participant regardless of whether any such damage or loss is caused by the active or passive negligence of the aforementioned parties or otherwise and regardless of whether any such liability arises in tort, contract, strict liability or otherwise, to the fullest extent allowed by law. The participant authorizes any representative of Starfish Aquatic Club, Inc. to have the participant treated in any medical emergency during their participation in Starfish Aquatic Club, Inc. swim lessons. Further, the participant and/or parent/guardian agrees to pay all costs associated with medical care and transportation for the participant. I have noted any medical/health problems of the participant of which the staff should be aware. I have carefully read the above liability release and signed it with full knowledge of its contents and significance.
Starfish Aquatics, Inc. requires payment in full before the beginning of any session. By signing off on this agreement, the guardian or participant is agreeing to full payment of swimming lessons. If there is a request for a makeup or reschedule and is not included in the 4-6 month package or 8 week session of PRIVATE LESSONS ONLY, there will be a $15.00 fee charged to your account. Any makeups from a session must be done within 2 weeks of the missed class, and cannot be carried over into a new sessionStarfish Aquatics, Inc. does not give refunds at any time.
If you are unsure if your student will be happy taking lessons, we suggest trying a Pay as You Go Private Lesson or scheduling a skill assessment to determine the right type of class for your student. If you sign your student up for a full session of lessons, there are NO REFUNDS
if they do not participate in the classes.

Instructors changes sometimes occur within the session due to situations that are out of our control. If an instructor is out for their lessons, a substitute will be teaching their lessons for that day. If they cannot find a substitute to provide the lessons that day, then the management will contact you individually to reschedule your lessons for an alternate day. In the event that an instructor leaves the school for any reason, there will be a new instructor to take over their lessons. We do not issue refunds due to instructor changes. Instructor departures sometimes happen due to college students graduating, getting new jobs, moving, etc.
