HOLD HARMLESS AGREEMENT
I understand that there are risks of physical injury associated with, arising out of and inherent to the activity of dance. I agree to indemnify and hold Cornerstone Studio, its instructors, employees and owner harmless from any and all liability for injuries, property damage, or loss in connection with my/my child's participation in dance or fitness activities. I also hereby authorize Cornerstone Studio to act for me according to their best judgment in any emergency requiring medical attention and hereby waive and release them from any liability from injuries, illness or loss incurred.
COMMUNICABLE DISEASES RELEASE, INCLUDING COVID-19
Participation in dance, movement, or fitness activities at Cornerstone Studio includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and,
I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,
I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest staff member immediately; and,
I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Cornerstone Studio, their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event ("RELEASEES"), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY AGREEING TO IT, AND SIGN IF FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. If the participant is under the age of 18, I agree on their behalf as their parent/legal guardian.
PAYMENT POLICIES
I hereby assume all financial responsibility for myself or the student I have enrolled at Cornerstone Studio. I understand that all Registration Fees are non-refundable & will be automatically charged to my card on file upon enrollment and that monthly tuition is due by the 15th of each month, from September through June, or I will be charged a $20 Late Fee. I understand that yearly tuition is calculated based on a 10-month calendar, broken up into 10 monthly installments for my convenience.
I also understand that Cornerstone Studio does not send out regular invoices, so I will be responsible for timely payment authorization of tuition or will authorize Cornerstone Studio to automatically charge tuition to my credit/debit card listed on my account, on the 1st or 15th of each month. I understand that cash and checks are no longer accepted as forms of payment.
ABSENCE/WITHDRAWAL POLICIES
Regular attendance of class is encouraged to ensure student progress. If a class is missed, students can view a recording of the missed class by accessing "Resources" in their Parent Portal.
If taking a leave of absence (4 weeks or more), return placement in class is not guaranteed. Tuition must be paid during the extended absence in order to guarantee placement is held for you/your student in class. We do not offer tuition proration for missed classes. Pre-arranged absences can now be scheduled within your Parent Portal to notify your/your student's instructor in advance. Class participation can be virtual via a live-stream or on-demand for the duration of the absence.
To withdraw from a class, notification must be made via email by the 15th of the current month prior to dropping a class for any reason. There is no refund for tuition due or paid in the month a class is dropped. We continue to hold a place for you/your student in class until a formal withdrawal is received, and you will continue to have access to all class recordings for on-demand playback. You will be responsible for payment of tuition until formal notification is given. A survey about the reason for withdrawal will be requested.
HYBRID CLASS POLICIES
I/My student will be required to wear a mask at all times when attending a Hybrid Class. This includes while entering and exiting the studio and for the entire duration of participation in dance, movement, or fitness activities. I will not enroll myself/my student for a Hybrid Class if I know I am/she/he is unable to keep a mask on.
I/My student will receive a contactless temperature check and will be asked a series of health screening questions before entering the studio. I/He/She will not be allowed into the studio until less than 5 minutes before class and will be asked to exit promptly when class ends.
A hand sanitizing station will be at the front entrance, and participating students will only be allowed to bring in their dancefloor shoes to change into, along with a disposable water bottle. I/My student will need to come dressed for dance/fitness class, with hair up if necessary, etc. Cubbies will not be available for the storage of items during class time.
Hybrid Classes will run for a maximum of 45 minutes per session because we know that exposure time and ventilation are critical factors in limiting the spread of COVID-19, based on dance-specific research and guidelines from national organizations in our field. I/My student will be assigned a designated area of space on the dance floor or barre that will be thoroughly sterilized before and after their participation.
Our public office will remain closed, and parents/relatives/guests will not be allowed to enter the studio. There will be a maximum of 4 students and 1 teacher in the building at all times and sometimes 1 additional staff member stationed at our entrance with the door open.
CLASS CANCELATION POLICIES
A class or program will not commence until a minimum number of students are enrolled in it. All classes are subject to possible cancellation if enrollment drops too low, but this is not commonplace. In the event that your class or program is canceled, we will make every attempt to find a comparable class to transfer you/your student into. If we are unable to provide you with a suitable alternative, you will have the option of either a refund or credit, pro-rated based on the number of classes already received.
In the event of an unexpected studio closure due to a Stay-at-home Order, inclement weather, or other emergencies, you will be notified as soon as possible and we will do our best to continue with virtual class offerings until we are able to resume as normal. DANCER DRESS CODE POLICIES
I understand that proper dance attire, hairstyle, and appropriate dance shoes enable me/my dancer to fully participate in class and to maintain a disciplined attitude towards dance training. If I/my student is not dressed in required or appropriate dance attire, I/he/she may not be allowed to participate in my/his/her in-person class. I understand that proper dance shoes are specific to each style of dance and are required in each class. Bare or exposed feet are now prohibited, so half-soles, footUndeez, etc. are no longer allowed. I also agree to learn how to put my/my dancer's hair into a proper bun if I am/he/she is enrolled for ballet.
Dress Code is also expected for virtual participation while live-streaming but footwear should be chosen based on what is safest for movement in your particular space.
PHOTO/VIDEO PERMISSION
I expressly consent, release, and transfer to Cornerstone Studio, in advance, any and all rights to photographs and/or videos of myself/my student in class, on a live-stream, at performances, at all Cornerstone Studio-related events, and from professional Picture Days. I understand that Cornerstone Studio may use such photographs or video recordings for promotional purposes, on various media platforms; such assignment may survive the termination of this agreement. I also understand that Cornerstone Studio will never use my full name or the full name of my student in association with any photography or video I am/he/she is depicted in and that the context of such depictions will also be displayed in a positive and professional manner, in alignment with our core values.
