THERE IS NO REGISTRATION FEE FOR SUMMER CLASSES. REGISTRATION: There is a NON-REFUNDABLE, annual registration fee of $35.00 per student. First month's tuition and registration fee are required to register, except for summer classes.
TUITION POLICIES: Monthly tuition has been set to average holidays, performance dates and four/five week months and is therefore charged equally each month. Tuition is not prorated due to student absences, but you may schedule a make-up class. Summer classes are only 4-6 weeks and paid in full upon registration. SUMMER TUITION IS NON-REFUNDABLE. Camp tuition will be billed in two payments. Half will be processed at the time of registration and the other half will be due on the first day of class. Camp tuition is Non-Refundable.
TUITION PAYMENT OPTIONS:
1) EFT: Monthly Draft by a Visa Check Card (Band Card), Visa Credit Card, or Master Card. (In order to select this option, choose 1st of the month from the drop down box below titled "e-payment schedule").
2) PIF: Payment in full for the full Term or 2 payments for each half-term. DDI Schedule begins Mid-August and ends in June. (In order to select this option, choose "PIF" from the drop down box below titled "e-payment schedule").
PRODUCTION FEE: Every recreational class that begins in the fall semester ages 3-up is included in the June Production. Each recreational class will have a $40.00 production fee per family. Each family will receive two free tickets to a recital.
COSTUMES: Students are required to purchase a costume for each class they perform with in the June Production (there are no costumes for summer classes) Costumes are $75.00 for ages 6-up and additional accessories may also be required. A NON-REFUNDABLE $37.50 deposit for each of these costumes is required on October 15 for each student. Costumes are $65 for ages 3-5 years old. A NON-REFUNDABLE $32.50 deposit for each of these costumes is also due on October 15. The balance for costumes is due Nov 15th and will be drafted on this day if you pay by monthly draft. If you choose to pay per semester, your entire costume purchase must be included with your first semester payment. DDI does not pay for alterations. Fees are NON-REFUNDABLE. DDI requires a parent to attend class the day we measure students for costumes, so they can make the final decision on what size is ordered for their child. DDI does not pay for alterations or exchanges.
There is a $15.00 service fee as a result of any payment returned to Destiny Dance Institute for insufficient funds.
I am the participating adult or the consenting adult, parent, or legal guardian of the child or children I am registering and allow my child or children to participate in classes at Destiny Dance Institute. Participation in dance does involve a certain amount of risk of injury. By permitting myself or this student to participate, I acknowledge that I am fully aware of that risk and hereby release Destiny Dance Institute, LLC and The Factory from all claims, expenses, injury and any legal action arising from my, or my child's participation in the classes offered by DDI, LLC or any activity on DDI's premises. I also release DDI from all claims, expenses, injury and any legal action incurred to any child being worn in a class.
This form also serves as a release for for the use of pictures, video presentations, promotions, media, television and the likes. You release all rights for the use of our material. We do not list names with photos.
