Release Agreement
As the guardian of the participant listed above, I acknowledge that I understand the nature of the virtual activities my child may be participating in and that my child is in the proper physical condition and capable of participating in the related activities, understanding that MYM/Make Your Move Performing Arts is not in any way responsible for making such a determination.
In consideration of my child's participation in virtual activities and instruction offered online in any dance, acrobatic and/or tumble instruction program, I understand and agree on behalf of myself and my child, to release, hold harmless, and discharge MYM/Make Your Move Performing Arts, it's employees and families, from all claims, costs, liabilities, expenses or judgments, including attorneys' fees and court costs for any occurrences in connection with any dance, acrobatic, and/or tumble instruction.
I assume all risks to my child in connection with any instruction and further release MYM/Make Your Move Performing Arts and its owners, employees, and volunteers from liability for any injury sustained by my child while he or she is participating in any dance, acrobatic, and/or tumble virtual instruction program, including all risks reasonably connected with such activity whether foreseen or unforeseen.
I understand that MYM/Make Your Move Performing Arts is not responsible for personal property that is damaged while I or my child is participating in virtual classes offered by MYM/Make Your Move Performing Arts.
I acknowledge and agree that it is my responsibility to maintain my own accident and health insurance coverage that provides adequate coverage for myself and my child participating in MYM/Make Your Move Performing Arts virtual classes and that MYM/Make Your Move Performing Arts does not provide accident or health insurance for those participating in its instruction, activities or programs.
By checking the box below, I acknowledge this is proof of my intention to execute a complete and unconditional waiver and release of all liability pursuant to the terms herein, and agreement as to all terms and conditions contained above.
I am of lawful age and competent to sign this affirmation.
Payment Responsibilities
Parent/Guardian agrees to assume all financial responsibilities (monthly tuition, late fees, returned check fees, dancewear, shoes, concert fees and costume fees) unless Written Notice of Cancellation of said agreement by parent/guardian and "Make Your Move" is received. PLEASE NOTE: All Students are considered enrolled and monthly charges/late fees are incurred unless Written Notification of Withdrawal is received, all fees and deposits paid are non refundable. If an account is 60 days past due dancer will not be allowed in class until account is current.
Withdrawals
There is a two month minimum for all lessons. One month notice from the first of the month is required to discontinue any classes. Withdrawal must be done in person and will not be accepted over the phone.
Withdrawal must be done at the school office and not with a teacher.
Withdrawal must occur within the first seven days of the month.
No withdrawals will be accepted after March 1st. To withdraw from classes a parent or student must:
-Inform school administration in person
-Complete and sign a withdrawal form provided by the school office
All Automatic bank draft charges will stop after the one month notice period. Make Your Move Performing Arts reserves the right to terminate lessons to any students without notice. In such case a refund for unused lessons will be given.
Payment of Fees
A registration fee of $50 per Student/$65 per Family for returning Students will be charged to your account upon Registration. A $25 "Tutu Twirl" Program fee for all Twinkle Tot , Twinkle Star, and Kinder Star classes will be charged at registration. Tuition must be paid by automatic bank draft (Debit or Credit).
Tuition will be debited from your bank account on the 5th of each month, Aug/Sept-May (tuition date in August may vary according to class start date. Any tuition payment made after the 10th will incur a $10 late charge.
The first month is non-refundable. Costume Fees for the year end concert for Starlet Academy will be withdrawn on Oct 25th. Every Dancer will pay a concert fee in April according to the concert package that you choose (Silver, Gold or Platinum) this fee will withdraw on the 25th of April.
All Competition Team members please refer to your team packet concerning Costume Fee withdrawal dates.
Tees, Costumes, etc
All Dancers are required to purchase a t shirt upon registration to be worn for most community events this fee is included in your registration fee. .
Costume fees for the year end concert will be charged to your auto bank draft on Oct 25th.
No costumes fees will be refunded after October even if student withdraws from the school. The costume fee that was paid will be used for shipping and restocking fees.
Costumes for competitive classes require an additional fee.
Costume Fees
Costume Fees for Starlet Academy will be withdrawn on Oct 25th. *please refer to "Overview of Dance Fees"
All Competition Team members please refer to your team packet concerning Costume Fee withdrawal dates.
By agreeing to this form you authorize make Your Move Performing Arts to withdraw all fees that are due.
Parent/Guardian Responsibilities
It is the responsibility of the guardian or adult student:
1. To be aware of ALL school activities (Performances, rehearsals, concerts, picture day, extra classes).
The school will post all such notices on the website, bulletin board, Facebook, in Newsletters and through Email to insure that you are informed. However, with the number of students in our school we CANNOT call each person on an individual basis to remind you of an event, that is why it is VERY IMPORTANT for you to come INSIDE the studio and check out the bulletin board and information table/to make sure you check your emails.
2. To Inform the school of any address change, personal information or changes to your auto draft account.
3. To make sure that your account is UP TO DATE.
Our TV monitors are placed in the waiting area for you to enjoy and view your dancer during class. They are not there to critique the staff or talk about other students. We ask if you have any concerns you will set up a meeting with your child's instructor through the office. Please do not Gossip in the Waiting area for this sets the stage for a NEGATIVE atmosphere and we always want to have a POSITIVE atmosphere at Make Your Move.
Assumption of Risk/Injury
Parents, Legal guardians, minor students and adult students waive the right to any legal action for any injury sustained on the school property or traveling to/from a MYM event, resulting from normal class activity or any other activity conducted by the students before, during or after class times.
Release of Liability
The Parent/Guardian agrees and acknowledges that the entire risk of personal injury to the student is assumed by the parent/guardian. Any and all claim, demand, and liability is waived by the student and parent/guardian for any personal injury or losses incurred during travel to and from any "Make Your Move" functions, competitions, workshops, performances, etc on or off the premises of "Make Your Move Performing Arts Studio", city of Valley Sportsplex, owner and staff associated with said studios and facilities
Extreme Weather and Substitutes
If the School must cancel class due to extreme weather conditions or events beyond our control such as power outages, the missed lessons will be made up during the season or added on the end of the teaching year. No refund for lessons missed due to these reasons will be given.
The School reserves the right to provide a substitute teacher if the regularly scheduled teacher is ill or otherwise unable to teach classes. If a teacher is ill and the school cannot arrange a substitute, any missed classes will be made up.
Photo Release
The school is hereby granted permission to take photographs of the students to use in brochures, web sites, posters, advertisements, and other promotional materials the school creates. Permission is also hereby granted for the school to copyright such photographs in it's name.
Dress Code
Required Dance Wear and dance shoes must be worn to ALL classes. Failure to wear required dance wear to class could result in students being asked to sit out of class or sent home. Repeated failure to wear required dance wear could result in termination of lessons. Students ARE NOT permitted to wear jeans or shoes worn on the street or outdoors to any dance class. For safety reasons dancers are not permitted to wear jewelry. All hair must be NEATLY SECURED in a ponytail or bun and out of the face and eyes.
Attendance and Tardines
The school reserves the right to ask a student who is late to sit out of class. Repeated tardiness may result in termination of lessons due to the potential harm to the student who misses the warm up segment.
A minimum attendance level is required. If a student misses more than 4 classes without written notices the school reserves the right to terminate lessons or deny the student an opportunity to perform/compete. Students missing more than 4 classes after January will not be allowed to participate in the year-end concert, festivals or competitions.
