NO MEMBERSHIP FEES FOR SUMMER 2020.
ANNUAL MEMBERSHIP fee will be due upon registration for FALL 2020/SPRING 2021 ONLY and is non-refundable in order to hold your dancer's spot in class(es)

PREMIUM MEMBERSHIP: $90 / DANCER or $175 / FAMILY (3 or more siblings)
BASIC MEMBERSHIP: $55 / DANCER or $105/ FAMILY
TUITION
• Tuition rates are for the full season (September-May, including studio holidays), not by the number of classes in the month. 1st month's tuition is due AUGUST 15th, last month's APRIL 15th. A full season of dance includes a minimum of 32 lessons. Tuition fees are paid monthly or annually.
• After registering and paying the membership fee, monthly tuition must be paid by account debit or automatic credit card payment. A valid debit or credit card is required on file at all times. Tuition will be debited from your account (or charged to your credit card) on the 15th day of each month (August-April.) To avoid running your card, please pay by cash, check or debit card by the 1st of each month.
• Accounts not paid within terms are subject to an accruing 5% monthly finance charge until paid. Accounts more than 60 days past due may be submitted to collection services.
• Recital costumes deposit of $40 / costume is due November 15th will be paid by automatic bank account debit or automatic credit card. Final payment is due on January 5.
• NSF on paper checks, auto-debit, or auto-charge payments will incur a $20 NSF fee which will be paid at the front desk along with the tuition.
I hereby authorize Elevation Dance Studio, LLC to electronically debit my bank account / credit card for the amount of any NSF authorization.
TRIAL CLASSES
One trial class may be available at the single class drop-in rate of $25 before signing up for a monthly package. The trial class fee can be applied to a Monthly Package if registration is completed within 30 days of the trial class. Trial classes are only available in classes that have space and must be approved by the School Director via email prior to attending: alyssa@elevationdancestudio.com.
RESCHEDULING & SUBSTITUTION
Elevation Dance Studio reserves the right to provide a substitute teacher if the regular teacher is ill or is otherwise unable to teach. Elevation reserves the right to reschedule, combine classes, cancel due to low enrollment, and move to a distance learning platform in the event the studio has to close.
Schedules are subject to changes based on enrollment.
WITHDRAWAL FROM CLASS
There is a two month minimum for all lessons. One-month notice from the first of the month is required to discontinue any payments. To withdraw a parent must email the artistic director: renee@elevationdancestudio.com. Automatic bank debiting or credit card charges will stop after the one-month notice period.
• Refunds are only issued when Elevation cancels a class due to low enrollment.
• Account credit will be issued when the student initiates the schedule change.
Credit will be applied to any Elevation tuition or fees due within one year.
ATTENDANCE
Regular attendance is vital to student progress and group choreography. Please make every effort to attend each class. Premium members may make up missed classes until March 1 and Basic members may make up missed classes until February 1. If you miss more than two of the final eight classes before recital you may be dismissed from participating in the recital routine at the discretion of the teacher. Please report all absences via website link. If your dancer arrives 15 minutes after class begins, they may be marked absent unless they check in with the front desk upon arrival.
DRESS CODE & MERCHANDISE
Appropriate dance apparel must be worn in class in order to participate. Check class descriptions on the website for class dress codes. All students have until the first week of October to obtain all required attire and shoes. The teacher reserves the right to not allow a student to participate in class if a student is not dressed in the proper dress code attire.
All required dance attire is available for purchase at Elevation.
A merchandise transaction sheet is located at the desk. Students who wish to purchase any items will be expected to pay at time of purchase. If a student signs in a transaction on the sheet at the desk, the amount will be posted to the student family account and the credit card on file will be charged at that time unless the student pays with cash or a check on site.
A valid credit card on file is required at all times.
Elevation is not responsible for keeping track of students not permitted to purchase merchandise from Elevation. Please be sure to educate your dancer about this policy.
LOST & FOUND & SECURITY
A lost and found basket is located in the dressing room. Please check the lost and found basket regularly as all lost and found items are donated to Lift Up on the 1st and 15th of each month.
Security cameras are located in public areas for safety and security.

STUDIO POLICIES
Students should use lockers for all personal belongings they will not need in class.
All walkways should be free and clear of belongings. Elevation reserves the right to place any items left on the floor unattended into a locker or the lost and found.
Seasonal locker rentals are available, please inquire at the front desk.
PARENT OBSERVATION:
The last 10 minutes of the last lesson of each month is open to observation at the discretion of the instructor. During our Fall and Spring Parent Observation Week, you are invited to be our guests in the classroom for the entire lesson. Other observation is at the teacher's discretion. We ask that you observe quietly, that cell phones be turned off, and that siblings are monitored and quiet during observations. Unless invited into the studio, please observe all classes from the lobby area only via the tv monitors and/or the one way glass.
PARKING DOWNTOWN:
Please park on Oak or 4th Street. Please do not park or Idle in the back alley of the studio. Please do not park in the spaces in the back of the studio at any time. You may park in the Wells Fargo lot after business hours to avoid being ticketed or towed.
Dancers are not allowed in the studio until an instructor invites them in.
No street shoes are allowed with the exception of clean, streak free sneakers for hip-hop.
Please bring water, as it is permitted in the studio.
No running, yelling or horseplay in lobby or dressing rooms.
No gum, food, candy or cell phones in the studio classroom.
ONLY Elevation staff is permitted behind the front desk.
NO students are allowed behind the front desk at any time.
NO CHILD should ever be left unattended at the studio in the lobby area.
SPRING RECITAL & COSTUMES
• Each May, we end our season with our fully-staged, professional Spring Recital at the Steamboat Springs High School. We encourage, but do not require, participation.
• Spring Recital dates and mandatory dress rehearsal dates are published in the weekly newsletters and Welcome Packet. Specific show assignments are published in the late March. Specific rehearsal assignments are published in April.
• Special requests to adjust rehearsal and recital times are accepted until February 1st. We attempt to accommodate every school's activities while meeting the technical demands of the show. Our priority is to prepare your children for the performance experience of a lifetime!
All classes have a costume unless noted on the schedule.
Spring Recital costumes deposit will be paid by automatic bank account debit or automatic credit card payment on November 5th with the balance of the costume due on January 5th and are not refundable after December 21st. Costumes are ordered over the Holiday Break. Costumes will not be ordered unless the deposit of $40/costume has been received.
Students are measured for costumes in classes and they will be delivered in Jan.-April. If a costume does not fit, we are able to exchange within one week of receiving it but there may be additional shipping/handling fees and/or limited availability.
IMAGE & VIDEO USE
Elevation Dance Studio is hereby granted permission to take photographs and video of the students to use for promotional purposes. Permission is also hereby granted for Elevation Dance Studio to copyright such photographs and video in its name.
RELEASE OF LIABILITY & MEDICAL TREATMENT
As the legal parent or guardian, I release and hold harmless Elevation Dance Studio, its owners, administrators, and instructors from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, sickness, illness, including death, that may be sustained by the student and/or the undersigned, while in or upon the premises or any premises, including online classes, under the control and supervision of Elevation Dance Studio, its owners, administrators, and instructors or in route to or from any of said premises.
The undersigned gives permission to Elevation Dance Studio, its owners, administrators, and instructors to seek medical treatment for the student in the event they are not able to reach a parent or guardian. Elevation Dance Studio cannot dispense any form of medication.
I acknowledge that my participation in in-person/online live instruction in dance, yoga, tumbling, and instruction in activities as an individual in a group or via online (live zoom or other live online platform) known and unknown, and unanticipated risks that could result in physical or emotional injury, paralysis, death, or damage to myself, to property, or to third parties. I understand that such risks simply cannot be eliminated without jeopardizing the essential qualities of the activity.
SAFER STUDIO POLICY
EDS requires all staff and students to stay home when they are ill. In order to reduce community spread of Coronavirus, we follow best practices and recommendations from our local health department and from our industry association, More Than Just Great Dancing!® and Youth Protection Advocates in Dance. EDS understands that unlike older children and adults, young children cannot be expected to maintain social distancing at all times. Therefore EDS focuses on a hierarchy of measures beginning keeping ill people home, followed by frequent hand cleaning and good hygiene practices, amplified cleaning, and curricular and facility adaptations to minimize contact and mingling where possible. All studio classes are backed up on an online learning platform. In the event a class is unable to be held at the studio classes will convert to an online learning platform until in-studio classes can resume.
ELECTRONIC AGREEMENT
Electronic Authorization
You are attesting that you are the parent or legal guardian of the above student or students, or are at least 18 years of age or older. By checking the boxes, you are electronically signing Elevation Dance Studio's liability waiver and agreeing to abide by all policies and procedures in this agreement. Your electronic signature has the same legal effect as a written signature by hand.
