Required dance wear and dance shoes must be worn to all classes. Failure to wear required dance wear to class could result in students being asked to sit out the class. Repeated failure to wear required dance wear could result in termination of lessons. Students are not permitted to wear jeans or street shoes to any dance class. For safety reasons dancers are not permitted to wear jewelry. Long hair must be tied back. Lost or stolen items are not the studio's responsibility.
Ballet Dress Code:
Princess Ballerinas /Pre Ballet.............. Pink tights, pink leotard, pink ballet shoes
Beginning/Intermediate Ballet............ Pink tights, light blue cap-sleeved leotard, pink ballet shoes
Advanced Ballet................................... Pink tights, black leotard, pink ballet shoes
DA Dance Center reserves the right to provide a substitute teacher if the regularly scheduled teacher is ill or otherwise unable to teach classes. If a teacher is ill and the studio cannot arrange a substitute any missed classes will be made up.
No refunds will be given for missed classes. School activities such as field trips, school camps, spring breaks that differ from our studio's spring break, sports games, etc. do not qualify for make-up lessons. If you miss a class, a make-up class is available but must be made up in the same month as the class that was missed. Only two (2) make-up classes will be allowed per month.
Class Attendance and Lateness:
The studio reserves the right to have students who come late to class, sit out the class. Repeated lateness may result in termination of lessons. A minimum attendance level will be required. If a student misses more than 4 classes without written notices, the studio reserves the right to terminate lessons. Students missing more than 4 classes after April may not be allowed to participate in the year-end show.
It is the responsibility of the parents or adult students to be aware of all students activities, such as shows, extra classes, and dates the studio is open or closed. The studio will post all such notices in the studio as well as emails. It is the parents/guardians responsibility to regularly check the studio and emails to ensure they are informed. It is the responsibility of the parent/guardian or adult student to inform the studio of any address or telephone number changes. All class changes must be recorded at the front desk to ensure you are charged the correct tuition.
Care of Students:
The studio is not responsible for providing before or after class care for students. Parents with students under the age of 5 are to remain in the studio during classes. Students are not to be left at the studio for excessive time before or after class. Students must wait inside the studio for pick-up.
I, the undersigned and/or being the legal and acting guardian representing a minor participant, hereby release, waive and discharge DA Dance Center, its directors, officers, employees, agents, independent contractors and volunteers from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises, or in route to or from any of said premises, or any premises under the control and supervision of DA Dance Center. I understand that dance activities as conducted and taught at DA Dance Center have inherent risks of injury. I recognize that the participant is exposing himself/herself to such risks when undertaking dance activities. I, the undersigned and/or being the legal and acting guardian representing a minor participant assume and accept all risks of injury or damages resulting from such dance activities. I understand that appropriate physical contact is required during the instruction of dance, and I give permission for instructors to make appropriate physical contact with me or my child for such instruction.
I, the undersigned give permission to DA Dance Center, its directors, officers, employees, agents, independent contractors and volunteers to seek medical treatment for the participant in the event they are not able to reach a parent, guardian or emergency contact. I also agree that I will be responsible for any financial debt incurred by said action. I have declared on this form any physical/mental disabilities, limitations, restrictions, or condition and/or declare the participant to be in good physical and mental health.
