GYMNASTICS CLASS - Tuition Information
Each student will be charged a $40 registration fee at the time of registration. Additional children of the same family will be charged $20 for registration.
One class per week: $75/month
Two classes per week (same child): $135/month
Additional children of the same family receive 10% off of tuition as well as there being a 10% discount for multiple classes. If you are currently a Boost member you will also receive 10% off.
We are a year-round program with a monthly tuition based on an average of four classes per month. If there are five classes in a month, we do not increase tuition. If there are three, we do not give credit.

If you choose to pay by cash or physical check, tuition is still due on the 15th of the month. If tuition is not paid by the 20th of the month a $15 late fee will be added to your account.

To be eligible for a makeup, the absence must come during a regularly scheduled class time. Holidays or days when Boost Gymnastics is closed, are not eligible make-up days. A student is eligible to make up for FREE during an Open Gym time within 4 weeks of the studentS absence. Tap - See leotard color below per class level, black leggings or shorts. *Combo and Intro to Tap will use Maryjane matte-finish tap shoes. Tap 8+ will use a split sole tap shoe in a matte finish.
Hip-Hop - black leotard (if you take another class just wear that leo, as you don't need to purchase two), black leggings, black sweat pants or shorts and a pair of clean sneakers used only for class (*no street shoes). *T-Shirts can be worn over leotards.
Acro Dance - You can wear your Jazz or Ballet leo, shorts and bare feet.
BOYS - plain white t-shirt with black pants and then appropriate shoes for style of class.

In the event that only one child is present for class, that child will receive 30 minutes of private instruction in lieu of the full class.
CAMP/OUTTA SCHOOL SPOT/RLS - Policy
A 50% deposit is due at the time of enrollment to reserve your spot. Your 50% deposit is non-refundable. No refunds or credits are given. The remainder of your balance is due the week before camp. Cancel your reservation before the Monday the week before the camp you are enrolled in and your remaining balance will no longer be due. However, if you do not cancel on or before the Monday the week before the camp you are enrolled in you will still be responsible for the remaining balance. All cancelations MUST be submitted in writing via email to info@boostgymnastics.com. Our camps fill up quickly and we schedule all staff and events according to enrollment which is why we are not able to be flexible with this.
CAMP & GYMNASTICS CLASS - Information
If you are new to Boost please visit www.KidCheck.com to create your KidCheck account before your first day of camp or class. KidCheck is our security check-in system for the safety of all of our participants. When creating your account be sure to add ALL approved guardians that may be picking up your child from camp. No gymnast or camper will be released to a person picking up without either the Guardian Receipt (received upon check-in) or a photo I.D. All persons picking up with a photo I.D. must be listed on the KidCheck account as an approved guardian. Also, no gymnast or camper will be released to a sibling unless they are at least 18 years of age.
Camp hours are from 9 am - 3 pm for Big Kids, and 9am - 1 pm for Little Kids (3-4). Before Care is offered daily beginning at 7:30 am and running through 9 am. Arriving prior to 8:45 am will result in a Before Care charge. After Care is also available daily for Big Kids. That is from 3 pm-5:30 pm. Picking up your child late from camp will result in an After Care charge for the day plus a $15 late fee. We appreciate your cooperation and punctuality! Also, note that at the time of drop-off Little Campers DO NOT take their shoes off for the day so please leave their shoes on when dropping off.
We fully understand and are sensitive to the fact that separation can be stressful for both parents and children. We are here to help in this sometimes difficult yet common process. Experience indicates that the best approach for guardians to take when dropping off their child is to hug the child and reassure him/her that you will be back later to pick them up after they've had a super fun day. If your child consistently has trouble separating, our coaches and I will continue to work with you and your child to make the transition time as smooth as possible. It takes a little longer for some children to adjust; be patient, and don't give up by lingering, entering the gym area, or "peeking in" on your child. If you would like us to give you a call to give you a report about how they are doing, just let the front desk know and we are more than happy to do so.
Our Big Kid (ages 5-12) campers will be swimming at camp. Everyone will be required to pass a swim test administered by our staff lifeguard, to be allowed to swim without a flotation device. The swim test consists of an unassisted 25 meter swim. It is totally fine if they cannot swim or do not pass the test as we have flotation devices for them to use. They are also welcome to bring their own that they are comfortable swimming in if you would like. Please give your kiddos a heads up that this will take place so they know it is coming. This is not something that needs to be stressful or overwhelming as we definitely will do our very best to not make it that way. They also do not have to swim if they do not want to. They are welcome to sit on the side of the pool and watch (there are always plenty of campers who choose to sit out).
Upon arrival at Boost the morning of camp, you will enter through the Gymnastics Entrance double doors. Once in the gym you will proceed to the white table to your left to retrieve your already printed KidCheck stickers (guardian receipt & name tag(s)). If you do not have tags on the table this means that we are still needing something from you (Medical Authorization Form or Payment) and you will need to see the front desk staff before your child can be dropped off. Once you have your name tag and guardian receipt you will locate your child's name and basket. Place all of their belongings into the basket. Coaches will be there to help you find your correct group, but Little campers will meet in the Preschool area of the Rec gym (where the front desk is located) and Bigs will meet in the Team gym (where the restrooms are located).

Please make note that the Kona Ice truck accepts cash only so please remember to place cash ($3-$5 depending on size preference) in an envelope with your child's name on it in their bag on Thursday. We require that a new Medical Authorization Form be filled out yearly. This form is located on our website under Forms. This MUST be filled out before your camper(s) can attend camp.
We will be charging your card on file for the remaining balance for camp tuition the week before camp. In the event, that you do not have a card on file you must pay the remaining balance before dropping off on Monday. Upon arrival please see the front desk to make your payment.

Late Pick Up Policy
Parents are required to pick up children promptly at the end of their program. All programs are different durations, so be sure to check the times for your specific program. Classes are an hour long with the exception of Parent & Tot and Preschool classes, which are 45 minutes. A child who is picked up late can get anxious, and coaches need to prepare for their next class or begin their closing duties. We completely understand that things happen and we will have a staff member stay with your child until you arrive. There is a $10 fee for anyone who is 15 minutes late picking up their child and a $1 fee for every minute after until you arrive.
Electronic Payment & Auto-Draft Policy
I AGREE TO PAY FOR ALL FEES ASSOCIATED WITH MY CHILD'S/CHILDREN'S PARTICIPATION AT BOOST GYMNASTICS, BOOST DANCE ACADEMY, AND BOOST FITCLUB. I further understand and agree that if I fail to make timely payments on my account, I will be responsible for any and all reasonable costs of collection, including filing fees, as well as attorney's fees.
I authorize my bank or credit card company to make monthly payments by method indicated below and post it on my bank account or credit card for electronic funds transfer (EFT) drafts. If my EFT is discontinued, a $5.00 direct billing fee will be charged on each month's payment. If my EFT is not honored for any reason, discontinued or changed without notice to Boost, a $20.00 service fee will be charged by Boost. Automatic Teller Machine cards not acceptable. I am in full control of my payments. If I decide to discontinue payment(s) or change the account information by EFT (checking/savings/credit card) for any other reason and go to credit card EFT or direct billing, I will give Boost Gymnastics and Boost Dance Academy written notice by email of such change before the 15th of the month. All cancellation emails must be sent to info@boostgymnastics.com or info@boostdanceacademy.com. Boost and/or its agent(s) are authorized to revert my account to a statement billing rate limited to the election made in the Note and exclusive of any other charges a Member may incur if the EFT Authorization is cancelled by me or I fail to meet payment obligations as stipulated herein within a reasonable time period, set at Boost discretion.
I further understand and agree that all credit card and bank drafts will show up as FITGROUP, LLC on my statement.
Photography Consent & Release
PHOTOGRAPHY CONSENT/RELEASE - parent/guardian grants permission to Boost Gymnastics, Boost Dance Academy, and Boost FitClub to take and use: photographs and/or digital images of me and/or my children for use in news releases and other marketing materials. These materials might include printed of electronic publications, web sites and other electronic communications. Parent/guardian further understands that child's (children's) and/or parent's name and identity may be revealed in descriptive text or commentary in connection with the image(s). I authorize the use of these images without compensation. All negatives, prints, digital reproductions shall be property of Boost Gymnastics, Boost Dance Academy, and Boost FitClub.
Assumption of Risk & Release
It is my desire as the Participant referred to herein to utilize the gymnasium, facilities, and tumbling equipment (hereafter the "Facilities") located at Boost, 11 Vaughns Gap Road, Nashville, Davidson County, Tennessee 37205. In connection with my use of such Facilities, I understand that activities associated with tumbling, gymnastics, dancing, and cheerleading, involve risks that could cause injuries to any part my body. I likewise understand that my use of athletic equipment at the Facilities, which may include trampolines, double-minis, tumble tracks, and rope climbing equipment involve special risks that increase the risks of serious injuries due to the altitude or height of the activity and the harm associated with a fall from an elevated height (with activities associated with tumbling, gymnastics, dancing, and cheerleading, and the use of the athletic equipment at the Facilities being collectively referred to hereinafter as the "activities"). I understand that my participation in the activities within the Facilities involves submitting myself to a particularly high risk of sprains, strains, fractures, breaks, ruptures, and dislocations of the bones, muscles, ligaments, and tendons in the ankles, feet, knees, thighs, fingers, hands, arms, shoulders, back, wrists, and face, as well as other types of serious injuries described in paragraph 2 below. In exchange for my being allowed to participate in the activities at the Facilities owned and managed by Boost, I agree (and if I am not 18 years old, my parent or legal guardian, agrees) to be bound by each of the following terms contained within this RELEASE OF LIABILITY, ASSUMPTION OF RISK, HOLD HARMLESS AND INDEMNIFICATION AGREEMENT (hereafter referred to as the "Agreement"):
1. Voluntary Participation. I understand and confirm that my participation in the activities within the Facilities mentioned above is voluntary. I attest that I am physically fit and do not have any medical conditions or health ailments that could be aggravated by my participation in the above-mentioned activities.
2. Identification of Risks. I understand that my participation in the activities within the Facilities may involve serious risk of injury to myself. I also understand that the risk of injury may include the possibility of permanent disability, paralysis, brain injury, illness, disease, and death. I recognize that other serious risks include heat and over-exertion injuries (such as heat stroke, cardiac arrest, and respiratory arrest), broken bones, nerve or spine damages, and joint dislocations. I understand that this Agreement is intended to address all of the risks associated with my participation in the above-mentioned activities undertaken within the Facilities, including, particularly, such risks created by action, inactions, or negligence on the part of Boost and its shareholders, officers, managers, employees, agents, and volunteers (with such other persons possibly rendering services to Boost in connection with the undersigned Participant's use of the Premises), including, but not limited to, risks created by the following: (a) my physical limitations and/or discomfort; (b) my use of the Facilities and/or the condition of the above-mentioned athletic equipment within the Facilities where such activities occur; (c) lack or inadequacy of policies, rules, or regulations with respect to use of such athletic equipment or Facilities; (d) the failure of Boost to protect me from the actions, inactions, negligence, recklessness, or intentional or criminal misconduct of persons other than those affiliated with Boost; (e) the inadequacy or unavailability of medical facilities or treatment; or (f) the lack of or inadequacy of supervision on the part of Boost or its shareholders, officers, managers, employees, agents, and volunteers in respect to my use of the Facilities.
3. Assumption of Risk. I assume all risks that are connected with my participation in the activities within the Facilities. I accept personal responsibility for any liability, injury, loss, or damage in any way connected with my participation in the activities within the Facilities.
4. Release and Waiver. I release Boost and its shareholders, officers, managers, employees, agents, and volunteers from any and all liability for and waive any and all claims for injury, loss, or damage, including attorneys' fees, in any way connected with my participation in the activities within the Facilities (a "Claim"), whether or not caused in whole or in part by the negligence or other misconduct of Boost or any of the individuals mentioned above.
5. Hold Harmless and Indemnification. I agree to hold harmless and indemnify (in other words, to reimburse and to be responsible for) Boost and its shareholders, officers, managers, employees, agents, and volunteers from all claims for liability, injury, loss, damage, or expense, including attorneys' fees (including the cost of defending any Claim I might make, or that might be made on my behalf, that is released or waived by this instrument), in any way connected with or arising out of my participation in the activities within the Facilities.
6. Binding Effect. This Agreement shall be binding upon myself and my relatives, personal representatives, insurers, members, heirs, beneficiaries, next of kin, or assigns and shall insure to the benefit of Boost and its shareholders, officers, managers, employees, agents, and volunteers (with such shareholders, officers, managers, employees, agents, and volunteers being an intended beneficiary of this Agreement due to their involvement with Boost's business).
7. Consent to Medical Treatment. I authorize Boost and its managers, officers, employees, or volunteers to provide to me, through medical personnel of their choice, customary medical assistance, transportation, and emergency medical services should I require such assistance, transportation, or services as a result of injury or damage related to any participation in the activities within the Facilities. This consent does not impose a duty upon Boost or its managers, officers, employees, or volunteers to provide such assistance, transportation, or services.
8. Applicable Law. Because Boost and the Facilities are located in the State of Tennessee, and in order to provide certainty in the law to be applied to the construction of this instrument, this Agreement shall be governed, constructed, and enforced in accordance with the law of Tennessee.
THIS IS A RELEASE OF LIABILITY AND WAIVER. I HAVE READ THIS ASSUMPTION OF RISK, RELEASE OF LIABILITY, HOLD HARMLESS AND INDEMNIFICATION AGREEMENT AND CONSENT. I UNDERSTAND THAT I AM GIVING UP SUBSTANTIAL RIGHTS BY AGREEING TO IT. I AM AGREEING TO THIS ASSUMPTION OF RISK, RELEASE OF LIABILITY, HOLD HARMLESS AND INDEMNIFICATION AGREEMENT VOLUNTARILY.
