RELEASE: No liability is assumed by the school and staff for the purposes of the child or children or siblings or guests for accidents caused by acts of said child or children, student and the person hereinafter signing the contract on behalf of said child or children or said self assumes responsibility thereof. I also understand that I am to pick my child up by the end of class time and that he/she will not be supervised outside of the classroom, therefore the school cannot be held responsible for the student's welfare. I understand that I should be in the reception area 5 or 10 minutes before the end of my child's class so that my child will have my immediate attention and supervision.
I agree that if my dependent or I (all heretofore known as "I") engage in any physical exercise, class, or activity, or facility on the premises or any venue where I participate as representatives of Chambers Performing Arts, I do so at my own risk. I agree that I am voluntarily participating in activities and use of said facilities, premises (including the parking lot) and designated Chambers Performing Arts venues. I assume all risk of injury, illness, damage, or loss to me or my property that might result, including, without limitation, any loss or theft of any personal property. I agree that this consent and assumption of risk statement covers each and every event or activity sponsored by Chambers Performing Arts. I agree to release and discharge you (and your affiliates, employees, agents, representatives, successors, and assigns) from any and all claims or cause of action (known or unknown) arising out of your negligence. I acknowledge that I have carefully read this Waiver and Release and fully understand that it is a release of liability. I am waiving any right that I may have to bring legal action to assert a claim against you for your negligence.

My child (if under 18) is in good health and has my permission and physician's approval to participate in dance, movement, fitness, acting, any event, or other performing arts related activities. My child has no known health issues to me or to their physician, which would inhibit or cause any detriment to my student. If 18 years or older: I agree that I am in good health and have my physician's approval to participate in dance, movement, fitness, and acting activities. I have no known health issues told to me by my physician which would inhibit or cause any detriment to my participation. I also understand that I (or my child) is participating on a voluntary basis.
I give my permission for Susan Chambers (owner), Studio Receptionist, Studio Teacher, or Studio Parent to take my child to a medical/dental facility, if necessary. In case of emergency, if no guardian of child can be contacted, I hereby give my consent for emergency medical care prescribed by a duly licensed Doctor of Medicine or Doctor of Dentistry. This care may be given under whatever conditions are necessary to preserve the life, limb or well-being of my dependent. I accept full responsibility for all costs of said medical care and any emergency treatments. Chambers Performing Arts will not be responsible for the cost of any medical care or emergency treatments. I hereby waive all claims whatsoever in connection with such medical treatments.
For Summer Students:
All fees are due in full at time of registration for Summer Camp. All fees are non-refundable. Summer students may make up classes at no charge in an appropriate-level camp.
For Annual Students: Annual Required Fees:
NOTE: All fees due by 1st of Month. NO checks or cash accepted after 1st of month. On the 5th of each month, payment is considered late: $15.00 late fee will be assessed and must be paid by credit/debit card only.
REMEMBER: For your convenience, all fees will be collected via autopay, where your credit/debit card will automatically be charged each month on the 5th of each month. If your account is not current before the 5th of the month, I understand that Chambers Performing Arts will pull fees from my debit/credit card on file.
TUITION FEES: Two Convenient Options:
10 PAY PLAN: 1st Payment and 10th Payment due at Registration, Registration Fee ($45 first student, $35 each additional student within the same family), and Production Fee ($75 per student) due at registration, followed by 1st of each month OR
FULL SEASON PAYMENT: 5% discount of tuition plus Production Fee and Registration Fee are paid in full at time of registration.
REGISTRATION FEE: $45 first student; $35 each additional student within family. Annual season and summer season included. DUE AT REGISTRATION.
ANNUAL SPRING PRODUCTION COSTUME FEE: You will be given a Costume Sheet with instructions on how to order and pay for your costume(s) online. You are required to order and pay for your costumes online before December 1, 2019. If you do not order by this time, your costume may not arrive on time. DUE DECEMBER 1, 2019.
If I register late in 2019, or early in 2020, I realize that I the costume company may not be able to order my costume for my performer depending on the time of order. Again, deadline is DECEMBER 1, 2019.
The following are approximate costs of costumes per class taken: Ages 3-6: $85.00 Ages 7-12: $90.00 Ages 13+: $95.00 Additional costume fees may apply-these are only approximate costs. I understand that I am financially responsible for any alterations that may need to be made and that costumes are not custom fit nor custom made.
PRODUCTION FEE: $75 per student. DUE AT REGISTRATION. Includes: Theatre rental and theatre staff, technical staff, dress rehearsals at the theatre, and lighting design. Does not include tickets to the shows. Actors who audition for a role and are cast, and additional $125 fee will be charged for all in studio rehearsals, additional dress rehearsal time in the theatre, microphone rental, etc. Ticket prices are as follows: $16, $19,,$23.
If I have two or more students in the Annual Spring Concert, I understand that there is no guarantee that they will perform in the same show. They may perform in different shows and I will be responsible for purchasing tickets to those shows. I also understand that if I, or my student, takes more than one class at Chambers Performing Arts, they may be performing in more than one show and must purchase tickets to those shows if I wish to attend.

A highlight of training at Chambers is participation in our Annual Spring Production, held in May of 2020. 99.9% of students participate. The 40th Annual Spring Concerts and Dress Rehearsals are as follows:

I understand that all children's classes ( All Combo classes: Combo Ballet/Tap, Combo Ballet/Jazz, Combo Musical Theatre/Tap, Fairytale Ballet, Broadway Babies), Level I, and Level I-II classes will be in one (or more) of the following shows: Friday, May 15, 2020 or Saturday, May 16, 2020. Times will be announced in the Spring of 2020.
Level II classes may be placed in the Matinee OR Evening shows depending on size of show.
Teen/Adult, Level II-III, Level III-IV and higher classes: will be in either Friday, May 15, 2020 OR Saturday, May 16, 2020 Shows. Standby for your exact show time for these classes. More info to come in the Fall of 2019.
I understand that if I have two or more students at Chambers in different level classes, that my students may be in two separate shows, with separate rehearsals, tickets, and casts.
I agree to the following policies, and have read the Chambers Performing Arts Handbook for this season. Here is a brief overview:
As part of their training, Chambers Performing Arts students are expected to behave in a responsible and disciplined manner and to adhere to the School's rules and regulations, both inside the facilities and outside in the greater community as representatives. Students are expected to treat Chambers staff with respect and maintain a positive attitude. Chambers strives to ensure that all rules are clear and reasonable, and students are expected to observe the established Code of Conduct. Chambers reserves the right to suspend or dismiss any student whose conduct or attitude is found to be unsatisfactory.
The following are absolutely forbidden at Chambers Performing Arts from Parents, Students, and Guests:
Physical intimidation or injury, verbal harassment
Theft, destruction of School property or property of others
Verbal abuse; lewd, vulgar conduct; profanity
Sexual harassment or coercion
Possession of drugs and/or alcohol
Smoking: Chambers is a non-smoking facility. State law prohibits anyone under the age of 18 from smoking. Students disregarding this policy will face suspension and/or expulsion from the School.
Firearms or weapons of any kind
Willful destruction or misuse of Chambers or another person's property
To film or photograph in the studios, locker rooms or bathrooms
Inappropriate use of the internet
Improper use of cell phones
Photography in the studios of fellow students or faculty is strictly prohibited.
Any of the above actions will be reported immediately to the Administrative Director, who will, in consultation with the Director of the School, decide the appropriate disciplinary action. Penalties may include (but are not limited to):
Dismissal from the School
Suspension from classes
Referral to counseling
Recording classes, or any person on site at Chambers Performing Arts, using tape recorders, cameras, cell phones or video recorders is strictly prohibited. Cameras and video recorders of any kind are also prohibited in the studios, dressing rooms, and bathrooms.
Students are expected to attend all scheduled classes. Students or their families should report all absences to the receptionist as soon as possible. Please do not attend if you are contagious, feverish, on medication, due to the safety of others. Students who are injured are expected to observe classes and take notes. Attendance is critical to consistent progress and advancement in the School. Poor attendance (for any reason) may jeopardize a student's placement and promotion. Classes are not transferable. Make-up classes are offered anytime throughout the current season.
It is essential that students come to class on time. Late arrival does not allow a student sufficient time to warm up, may cause injuries and disrupts class concentration. Students more than ten minutes late to any class will be asked to sit and observe.
Students are expected to be in proper dress code at all times. Check the web site for dresswear requirements. You must wear a cover-up to and from classes, and never wear dance shoes outside on the street or coming into the studio. Chambers dress code is designed to encourage unity and discipline among our students. Students not meeting the required dress code may be restricted from participation in class.
No shirts, legwarmers, skirts, etc., may be worn. Jewelry should be minimal: small earrings at the teachers' discretion. NO facial jewelry or multiple earrings (nose rings, multiple earrings in ears, etc.). Shoes should be neatly sewn with like color thread. NO safety pins. Dancewear is to be kept laundered and in good repair. Please mark clothing, shoes and dance bags with the student's name and class level. Extended curriculum dress code requirements are mandatory as announced. Please note: Do not use fabric softeners when laundering dance attire, as it damages the studio floors.
Parents are not allowed in the classrooms at any time during the season, unless it is Parent Observation Week. Please do not peer in to the windows while your student is learning. Be supportive and never correct your child in terms of technique--please trust the expertise of our professional Chambers instructors and staff. Do not let siblings become loud or disruptive in the reception rooms.
PARENT OBSERVATION WEEKS
Parents/guardians are invited to observe class only during designated observation weeks. Check the Calendar for dates-twice per year. The class observation schedule is subject to change. This observation is NOT open to the public (friends, etc.), but is for parents/guardians to observe their student's progress. Please do not bring siblings to Parent Observation week, as it is distracting to the students.
Trial Enrollment Parents, Students, and Guardians ONLY:
I understand that as a Trial Enrollment student, that the following provisions on this contract apply directly to myself, and/or my student(s):
1, 2, 3, 4, 9, 10, 11, 12, 13 (of this provision).
I understand that I am Trial Enrolled, and that includes 1 week of unlimited trial classes. 72 hours commencing the trial enrollment, if I have NOT contacted Chambers Performing Arts (via email-not via phone message or verbally) stating that my student(s) will NOT continue--I understand that my student(s) will become fully registered students and my credit card or debit card will be charged for:
1st Payment of classes
Once my students become fully registered students, all provisions of this contract now apply.
Again, if my student(s) do not wish to continue enrollment after the 7 day trial period, I understand that I must alert Chambers Performing Arts via email within 72 hours of the 7th day of Trial enrollment to end enrollment.
ALL FEES PAID TO CHAMBERS PERFORMING ARTS ARE NON-REFUNDABLE.
