I understand and acknowledge that the activities that I or my child engage in while on the premises or under the auspices of TAC pose known and unknown risks which could result in injury, paralysis, death, emotional distress or damage to me, my child, to property, or to third parties. The following describes some, but not all, of those risks:
Cheerleading, gymnastics and dance, including performances of stunts and use of height increasing apparatuses, entail certain risks that simply cannot be eliminated without jeopardizing the essential qualities of the activity. Without a certain degree of risk cheerleading students would not improve their skills and the enjoyment of the sport would be diminished. Cheerleading, gymnastics and dance expose participants to the usual risk of cuts and bruises, and other more serious risks as well. Participants often fall, sprain or break wrists and ankles, and can suffer more serious injuries. Traveling to and from shows, competitions and exhibitions raises the possibilities of any manner of transportation accidents. In any event, if you or your child is injure, medical assistance may be required which you pay for yourself.

In consideration of the services of Turners All Star Cheerleading, LLC., its owners, agents, officers, employees, and all other persons or entities acting in any capacity on their behalf (hereinafter collectively referred to as "TAC"), I hereby agree to release, discharge, and hold harmless TAC, on behalf of myself, my children, my parents, my heirs, assigns, personal representative and estate as follows.

I expressly agree and promise to accept and assume all of the risks, known and unknown, connected with TAC related activities, including but not limited to performances of stunts and use of height increasing apparatuses. My participation and that of my child is purely voluntary. No one has forced or coerced me or my child to participate. I elect for myself and my children to participate in such activities in spite of the risks. I hererby release TAC including its officers, shareholders, agents, and employees, from any liability to the athlete named below, of the person claiming through him/her, arising from injury to the person or property of the athlete occurring on the premise of TAC, including any event sponsored or sanctioned by TAC, and/or travel to and from such activities. This release includes but is not limited to any claims of negligence, dangerous condition, latent defect, premises liability, code violation, negligent security, failure to warn, vicarious liability, negligent hiring, negligent supervision, negligent maintenance or improper/dangerous equipment; it is intended to be as broad as permissible under PA Law.
I hereby voluntarily release, forever discharge, and agree to hold harmless and indemnify TAC from any and all liability, claims, demands, actions or rights of action, which are related to, arise out of, or are in any way connected with my child's participation in TAC related activities
I certify that my child has health, accident and liability insurance to cover bodily injury or property damage that may be caused or suffered while participating in this event or activity, or else I agree to bear the costs of such injury or created, directly or indirectly, though or by any such condition.
By signing this document, I acknowledge that if anyone is hurt or property is damaged during my participation or the participation of any of my children in this activity, I may be found by court of law to have waived my right to maintain a lawsuit against TAC on the basis of any claim from which I have released TAC by signing this Agreement.
By signing this document, I swear that my child has been cleared by a physician to participate and does not have any medical conditions that would limit their full participation.
I give TAC permission to film, photograph, or video the athlete mentioned below and use for anything for TAC. I waive rights to compensation or ownership thereto.
I have had sufficient opportunity to read this entire document.
I have read it and understand it. I agree to be bound by its terms.

COVID 19 Release Form
The novel corona virus, COVID-19 has been declared a world wide pandemic by the World Health Organization. COVID 19 IS EXTREMELY CONTAGIOUS and is belive to spread mainly from person to person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.
Turners All Star Cheerleading LLC, "The CLUB", has put in place preventative measures to reduce the spread of COVID-19: however, the club can not guarantee that you or your child/children will not become infected with COVID-19. Further, attending the club could increase your risk and your child/children's risk of contracting COVID-19.
By signing checking this box, I acknowledge the contagious nature of COVID-19 and VOLUNTARILY assume the risk that my child/children and I may be exposed to or infected by COVID-19 by attending the club and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand the risk of becoming exposed to or infected by COVID-19 at the club may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Club employees, volunteers, and program participants and their families.
I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child/children or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child/children may experience or incur in connection with my child/children's attendance at the club or participation in club programming ("Claims"). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless the club, its employees, agents, and representatives, of and from the claims, including all liabilities, claims, actions, damages, cost or expenses of any kind arising out or relating thereto. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of the club, its employees, agents, and representatives, whether a COVID-19 infection incurs before, during or after participation in any club program.
