Registration Agreement

Membership Fee:
A non-refundable fee of $35/$40 per returning/new student ($50/$60 per returning/new family) is required annually at time of registration (not applicable for workshop classes, 9 week programs or summer camps).
Annual Classes Fees (September - June):
Tuition fees are based on the entire season (including all holidays and studio breaks), not the number of classes per month. A full season of dance includes 32 weeks. Tuition fees are paid in monthly installments or annually. Last monthly installment (June) is non-refundable and payable at time of registration. Tuition must be paid by automatic credit card payment or electronic funds transfer (EFT) and will be charged to your credit card/bank account on the 1st day of each month (September - May). Full year payments can be made by Cash, Cheque or Credit Card. A 5% discount will be given for all full year payments made by Cash or Cheque by September 30th. Fees are subject to change.
Withdrawal Policy:
There is a one month minimum for all classes. BDA requires 30 days written notice from the 1st of the month to discontinue service. To withdraw from Bella Dance Academy you must complete and sign a withdrawal form. If notice of withdrawal from a class is received in the middle of the month, you will be required to pay the following month's tuition. If you do not give BDA written notice of your intent to withdraw from a class or classes, your account will continue to be charged until the end of the time period covered by this agreement (May, 2021). Withdrawals will not be accepted after February 1, 2021.
Refunds/Credits:
Tuition, registration fees and costume fees are non-refundable. Refunds will not be given for days missed due to absences, holidays, studio breaks, weather-related closings or power outages. Refunds will only be issued if BDA cancels a class due to insufficient enrolment. Refunds will not be given if a similar online class is provided in place of an in-studio class.
Account credit may be given in the event of a medical emergency and will be considered on a case-by-case basis. Any credit applied to an account, will be applied to any BDA tuition or fees due within one year.
NSF/Late Fees:
There is a $30 charge on all NSF cheques and/or returned EFT payments. Accounts not paid within terms are subject to an accruing 5% monthly finance charge and discontinued services until paid. Accounts more than 60 days past due may be submitted to collection services.
Workshop Classes (Including 9 Week Sessions and Summer Camps):
Workshop fees are non-refundable, non-transferable and due at the time of registration. Workshop classes, 9 Week Programs and Camps are not subject to membership fees.
Discounts:
Discounts will apply to students enrolled in more than 2 weekly hours of annual classes. Discounts are as follows:
X hours of class per week = X% tuition discount (5 hours of class = 5% discount on all annual class tuition).
Discounts are not offered for summer camps or classes, special events, examination fees, competition choreography, entry fees, costumes or membership fees.
Private Lessons: All private lesson fees are non-refundable and due at the time of booking. If cancellation notice is received by BDA (phone or email) with 24 hours notice, a credit will be applied to your account. If 24 hours notice has not been received, your fee will be forfeited.
I have read and agree to abide by Bella Dance Academy Inc., (herein BDA) registration agreement regarding tuition, membership fees, late charges, refunds, discounts, fee structure and withdrawal policy.
I authorize BDA to retain my credit card information on file and to automatically charge any tuition and supplies fees due on or after the 1st of the month in the event that I have not paid the amount due on or before the due date.
I agree that if my credit card information changes, I will notify BDA as soon as possible with the corrected information. I understand it is the policy of the studio to have a credit card on file even if payments are made by another method.
Year End Performance
We end our season with our Year-End Performance. All dance students, with the exception of Adult students and Workshop Classes (including 9 Week Session Classes, Camps) will have the opportunity to participate in our year-end performance. We encourage, but do not require participation.
If, for some reason you do not intend to perform, you MUST inform the office in writing by November 1, 2020.
Year-End Performance and mandatory dress rehearsal dates will be tentatively published in our February newsletter. Show assignments and rehearsal schedules will be tentatively published in our March newsletter. Finalized schedules will be released no later than April 15th, 2021.
Tickets will be available for purchase through a ticketing service in April (date TBA). Ticket prices typically range from $15-$30 depending on venue, and ticketing fees.
I take full responsibility for my child to take part in the year end performance.
It is my responsibility to make certain that my dancer attends all rehearsals as outlined in the BDA Season Schedule/Policy Agreement. I understand that there are additional fees for my child to take part.
Costume & Performer's Fee
Costume & Performer's Fees:
All Classes participating in the year end show must purchase a costume for year-end performance and will be sized by BDA staff. By signing below you authorize Bella Dance Academy to withdraw the appropriate Costume Fees on November 15th, 2020 and Performer's Fee on February 15th, 2021 from your EFT or Credit Card payment on file. A 3.5% processing fee will be added to all credit card or EFT payments. To avoid this fee, please pay by cash or cheque before November 14th (Costume Fees) or February 14th (Performer's Fees).
Costume & Performer's Fees are as follows:
Costume Fee:
$95 + GST (per class), Includes: one complete costume.
Performer's Fee:
$50 + GST (per student), Includes: garment bag, alterations, tights (if needed), performance t-shirt, & link to filming of Junior or Senior Year End Performance.
Costume & Performer's Fees are non-refundable and go towards fees associated with our performance (renting our theatre, sound, lighting crew & backstage crew, set and stage design, costumes, tights, alterations, accessories, performance t-shirts, videographer fees, NACC ticketing fees, and much more). If you withdraw from a class after November 30, 2020, your costume and performer's fee will be forfeited. All dancers with 3+ classes, or families with 3+ dancers will have the option to switch to a payment plan.
*Please note - Costume & Performer's Fees will only be collected for our 2020/2021 season if approval to move forward with a year-end performance has been approved.
I have read and agree to abide by BDA's policies regarding Year-End Performance, as well as Costume and Performer's Fees, and authorize all associated fees.
2020/2021 BDA Policies
5 Ways To Learn:
BDA offers a variety of service options including private instruction, small group instruction, traditional classes, online instruction and pre-recorded classes to meet the needs of students and staff as well as for the ability to maintain service continuity in any situation.
Rescheduling, Teacher and Class Substitutions:
Teacher assignments will be announced before classes begin. BDA reserves the right to reschedule or combine classes, or to provide a substitute teacher. BDA reserves the right to deliver class content via an online system in the event that classes are not able to be conducted live for any reason including, but not limited to: weather, teacher absence, or governmental advisory.
Safer Studio Policy:
BDA requires all staff and students to stay home when they are ill. In order to reduce community spread of Coronavirus, we follow best practices and recommendations from our local health department and from our industry associations, More Than Just Great Dancing!® and Youth Protection Advocates in Dance. BDA understands that unlike older children and adults, young children cannot be expected to maintain social distancing at all times. Therefore BDA focuses on a hierarchy of measures beginning with keeping ill people home, followed by frequent hand cleaning and good hygiene practices, amplified cleaning, and curricular and facility adaptations to minimize contact and mingling where possible. All studio classes are backed up on an online learning platform. In the event a class is unable to be held at the studio, classes will convert to an online learning platform until in-studio classes can resume.
Continuous Parent Communication Plan:
BDA uses a "green, yellow, red" system of continuous parent communicating regarding the status of classes which can be found in the top right-hand corner of our website and social media. Green indicates to attend face-to-face classes as usual. Yellow indicates a cautionary change to service delivery, so please check your email. Red indicates clients should stay home and attend class online.

Attendance:
Regular attendance is vital to student progress and group choreography. Please make every effort to attend each class. If you miss more than two of the final eight classes before our year-end performance you may be dismissed from participating in the performance routine at the discretion of the teacher. Please report all absences via website link.
Inter-Sessional Breaks:
BDA provides two inter-sessional breaks that provide flexibility during the dance season: one in the late fall and one in the late winter. Inter-sessional breaks allow for the potential makeup of classes in the event of breaks or a cessation of classes for any reason.
Dress Code:
Proper attire and hair etiquette are required for all classes. Special class attire including shoes and hair will be published in our August newsletter. For safety reasons, dancers are not permitted to wear jewelry. All required items will be available for purchase, in our Bella Boutique (with the exception of Pointe shoes).
Class Observation:
In an effort to maximize both instructional time and observational time, parent observation will be permitted for the entire class on scheduled dates (please see the BDA 2020/21 Season Calendar). Other observations are at the teacher's discretion. We ask that you observe quietly, that cell phones be turned off, and that siblings are monitored and quiet during observations.
Lost & Found:
A lost and found basket is located under the coat racks in the waiting room. Please check the lost and found basket regularly as all lost and found items are donated to the Salvation Army on the 15th of each month. BDA is not responsible for any lost or stolen items. Please do not leave belongings unattended. Dancers are encouraged to take dance bags into classrooms with them.
Exam Classes:
We are very pleased to be working towards offering Royal Academy of Dance (RAD) Ballet Exams and Canadian Dance Teachers' Association Exams in Jazz, Tap, Modern and Acro during our 2020/2021 season. Eligible classes and/or students will be selected early in the season, and will work towards June examination periods. Participation is not mandatory, but is encouraged. Extra fees apply and special attire is required. If your dancer has been selected to participate in an examination or class award session, you will receive notice (and all details) no later than October 15th.
I have read and agree to abide by BDA's policies regarding service delivery, attendance, and dance attire.
I understand that BDA reserves the right to reschedule or combine classes, to provide a substitute or cancel a semi- private class if enrollment falls below four students and to convert a traditional group class to a semi-private class if enrolment falls below eight students per class.
I understand that BDA is not responsible for lost/stolen items, or unclaimed merchandise.
Summer Camp Policies
*All students must wear comfortable and relatively tight fitting clothing.
*All students must wear dance shoes or socked feet.
*Jewelry is not permitted and hair must be tied back off of face.
*All students must bring a water bottle, and students enrolled in day camps, need to bring one sack lunch, and two healthy snacks.
*All food/snacks MUST be PEANUT FREE.
*Registration is complete when Registration Form is received with full payment.
*You will receive an email confirmation of your registration.
*Camps/Classes are limited in availability and are on first come first serve basis.
*Camp/Class registration fee is non-refundable and non-transferable, so please check your dates carefully.
*Cell phones are NOT permitted in the dance area. We suggest leaving all valuables at home.
*The school reserves the right to provide a substitute instructor if the regularly scheduled instructor is ill or otherwise unable to teach classes.
*All Day Camps run from Monday to Friday. Camps begin at 9:00 am (drop off 8:45 - 9:00 am) and run until 4:00 pm (pick up between 4:00 - 4:15 pm). Students are required to bring two snacks, a sack lunch and water bottle every day. A half an hour lunch and 2 fifteen minute breaks will be given.
*Injuries/ Illness: Parents, legal guardians of minor students and adult students waive the right to any legal action for any injury/illness sustained while attending summer classes/camps on and off school property resulting from normal dance activity or any other activity conducted by students before, during or after class time, or in connection with transportation to our outdoor performance.

*Minimizing mixing and mingling via social distancing practices
*Curricular and facility adjustments
*Gradual re-opening and continuous parent communications.
Please see below for the specific way BDA is applying these guidelines for the Summer Programs:
Wellness Checks for Staff
*BDA is a Youth Protection Advocates in Dance® Certified Studio. Additionally, all teachers and staff members will receive regular and continuing safety training as it relates to preventing community spread of COVID-19.
*All staff members are wellness checked before their shift.
*Parents are asked to not send their children to class if they, or a member of their household, have a fever or respiratory symptoms (checklist will be sent to you before camp commences).
*Parents are asked to not send their children to class if they, or a member of their household, have tested positive for COVID-19 in the last 14 days.
*Parents are asked to not send their children to class if they, or a member of their household has returned from travel outside of the NWT within the last 14 days.
Frequent Hand Hygiene & Good Respiratory Hygiene
*Employees will wash hands and/or sanitize before and after each class.
*Hand sanitizing stations will be available outside of all entrances, outside of every classroom, and inside the classrooms. Students will be encouraged to wash hands and/or sanitize before and after classes.
*Handwashing posters are posted within bathrooms and classrooms.
*Proper respiratory practises for staff and students will be encouraged: Coughing into elbow or into a tissue, Throwing the tissue directly in the garbage after use, then washing hands.
Enhanced Cleaning & Maintenance Procedures
*We have increased our cleaning frequencies and sanitation procedures in high traffic areas and high touch surfaces of the studio such as doors, viewing windows, barres, floors, counters, stereos, and bathrooms.
*We have installed all new HVAC filters throughout the facility to improve the air quality and reduce contamination. Intakes and returns will be cleaned daily.
Minimized Mixing and Mingling
*We have made facility, scheduling, and curricular changes to reduce mixing and mingling in support of social distancing recommendations. However, we understand that unlike older children and adults, young children cannot be expected to maintain social distancing at all times.
Facility & Curricular Adjustments
*All lobby waiting areas, kitchen area, children's play spaces, student study areas, locker rooms, business offices, staff room, etc will be closed unless specified.
*Please bring clearly labeled water bottles. Drinking fountains will not be available.
*Dancers will bring all dance bags into the studio for changing shoes. Locker rooms will be closed. Bathrooms will be available for toileting only, but not for changing clothes.
*No food will be allowed in the studio, except for day camps - where students require a packed lunch and two healthy snacks.
*Students will not participate in any food preparation.
*Food sharing is not permitted.
*Packed lunches must be clearly labelled and should stay with the student's belongings.
*Packed lunches will not be handled by staff.
*Microwave will not be accessible during camps - please plan accordingly.
*Curricular adjustments have been made to eliminate prop use and hand-holding and to reduce mixing and mingling in the classroom.
*Class sizes have been modified to reflect current territorial recommendations of no more than 25 in a room. (Our class sizes will be no larger than 13 plus an instructor and an assistant, and groups will remain consistent for the entire week of camp).
*Class sizes have been modified to reflect a 6-foot social distancing pattern and floors will have markers to help students maintain boundaries as much as possible.
*Classrooms will refrain from high fives, "ballerina hugs", fist bumps, stickers, and stamps.
Gradual Re-Opening & Continuous Parent Communication
*We are gradually reopening with roughly all day camps running at approximately 50% capacity.
*Please visit our website and join our BDA Parent & Student Community Private Facebook page beginning July 1 where you will find a stop and go light icon that clearly indicates the status of classes:
**Green indicates it is safe to attend face-to-face classes.
**Yellow indicates a cautionary change to service delivery.
**Red indicates clients should stay home and attend class online.
Arrival & Departure
*Please enter using the Old Airport Road main entrance for all arrivals. A staff member will be outside to greet your student and help them to their class. At this time, we ask that parents wait in their cars or outside during classes to help with social distancing in the building. At this time, we will not be accepting any students under the age of 6.
*Please limit parent drop off to one person whenever possible. Sibling visitors cannot be accommodated during this session.
*Students will be released from the Old Airport Road main entrance. Parents will be asked to wait outside for students to be released. A staff member will be outside to ensure your dancer gets back to you safely. At the end of class, parents must be by the door to receive their child. Students cannot walk across the parking lot to go to their parents for safety reasons.
Bathroom Use
*We encourage all students to use the restroom at home before coming to class.
*Bathrooms will be available for camp students, and will be cleaned hourly.
*Bathroom facility use will be limited to the one front unisex bathrooms for this session of classes.
*Bathrooms will not be available as change rooms - dancers need to arrive in dance/warm up apparel.
Masks
*Mask wearing for staff, students and parents is at personal/parental discretion.
*If students do choose to wear a mask - it must be clearly labelled, and will not be shared.
Email Authorization
Important studio and class information will be sent to all email accounts listed on file. If you do not wish to receive such email, you may opt out, or remove your email address at any time. However, since this is our primary form of communication with you, we urge you to keep at least one email address on file so you do not miss important and/or time-sensitive information.
Photo & Video Release
Photo & Video Release: Bella Dance Academy Inc., BDA, (hereinafter "Entities" irrevocable permission to use my child's image, name and visual likeness in all forms and media for advertising, trade, and any other lawful purpose. I agree that all right, title and interest in and to all such images and any reproductions or derivative works shall be the exclusive property of BDA. Additionally, from time to time parents and students may opt to share videos, photos or other information on social media pages that may be used, hosted, created or referenced by any of the above named Entities. The party posting takes responsibility for all sharing or posting of such information and waives any claim against the Entities for the release of any protected information. If my child(ren) is under the age of 18 at the time of this agreement, the party signing makes this waiver on behalf of their child(ren) . I hereby release BDA and the named Entities from all liability or legal responsibility that may arise from the acts that I have authorized or consented to herein. I agree that BDA does not owe me any compensation for the acts that I have consented to in this Section.

Today's Payment
Annual Classes (September - June):
Membership Fee and last month's installment (June) are non-refundable and will be charged to your Credit Card or EFT account when this Electronic Registration Form is received by BDA.
Workshop Classes/9 Week Session/Summer Camps/Private Lessons:
Workshop Fees and associated fees are non-refundable and will be charged to your Credit Card or EFT account when this Electronic Registration Form is received by BDA (in the next 72 hours).
A 3.5% processing fee and applicable GST will be added to all summer camp tuition fees. If you prefer to pay with cash, cheque or e-transfer, please email us at info@belladance.ca within 24 hour of submitting your registration form.

BDA Online Live Class Disclaimer
You are responsible for reading this message prior to participation in BDA Online Classes. By using our links to access our livestream classes, you agree to assume the risks involved in participating in instruction at your home or another non- studio location. You agree to indemnify and hold harmless to Bella Dance Academy, its owners, and its staff, from any and all liability, claims, demands, and causes of actions whatsoever, arising out of our related to any loss, damage, or injury, including death that may be sustained by the participant while using online instruction.
Release of Liability
I understand that participation in a dance program involves risk of possible injury and/or illness. I understand that BDA and its staff will not be held responsible for injuries or illness sustained in class, in rehearsals, while performing, traveling to or from its facilities, or participating in any associated events.
I understand that I assume full financial responsibility for risks of personal and bodily injury and/or illness that may be sustained in class, while performing, traveling to or its facilities or participating in any associated events.
I release BDA and its owners, managers, instructors, employees, agents, associations, related companies, volunteers, successors and assigns, from liability for all personal and bodily injury and/or illness that may be sustained in class, in rehearsals, while performing, traveling to or from its facilities, or participating in any associated events. I specifically intend this release to include all claims that my child and I might have based on actual or alleged negligent supervision, instruction, training, and or facility care and cleaning.
I authorize BDA to secure any emergency medical treatment my child might need. I understand that it is my responsibility to inform BDA of any medical conditions or concerns that they may need to be aware of.
I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation.
If I am a minor, my parent and/or legal guardian has signed this document releasing Bella Dance Academy Inc. from any and all such liability described above and has acknowledged that I am knowingly and voluntarily assuming all risks of injury inherent to this activity.
The participant has my permission to participate in BDA's events. I warrant the above information is complete and correct. I further release Bella Dance Academy Inc. of all liabilities associated with my child's participation at BDA and all associated events.
The following must be signed to indicate that you have read and understood Bella Dance Academy's Policies and Registration Agreement and Release of Liability. Students under the age of nineteen must have a parent/guardian signature.
