A non-refundable registration fee of $35 for the first student and an $10.00 for each additional student of the same immediate family is required annually at the time of enrollment. Tuition is also non refundable. Monthly tuition payments are based on a ten month calendar and are not payable on a per-class basis.
There is a $15.00 fee for any late payments for returned checks or credit cards due to insufficient funds. If you choose to pay by check or cash an additional $8.00 handling fee will be applied to your account. Please ensure your student's full name is in the memo section of the check before mailing it. If you pay in cash, you must always get a receipt from the receptionist.
By signing this document I accept all financial responsibility for all services and products provided to my student by the Pure Movement Dance Institute (PMDI). I am aware that Fall - Spring tuition is based on a ten month payment schedule, the first payment being due at the time of registration and on the 1st or 15th of the month from then on (per my selection) until the conclusion of classes in May 2019. If I choose to pay by check, I understand that all payments are due on the 1st of the month and any payments received after the due date will receive a $15.00 late fee. Regardless of payment method I have the right to discontinue class at anytime, however, I am also aware that all fees paid prior to or in advance of my cancellation are non-refundable and I may be held responsible for any balance due on costumes that have already been ordered. I am eligible to have a late fee excused once per year, per family upon completion of a late fee waiver at the studio office. I also understand that it is my responsibility to ensure that all tuition and fees are paid on time regardless of days of the week, holidays, school closings, weather or mail service.

Registration Fees are withdrawn at the time of registration.
Tuition Payments 1-10 are withdrawn August 2018- May 2019. A monthly $10.00 payment will be deducted from my account for each costume required for my class. In the event that I have a costume balance in May of 2019, I authorize PMDI to make a one-time charge to my account for the remaining funds. In the event that I have a credit I understand that PMDI will apply this credit to my tuition.

In addition I acknowledge that I have been informed that all financial information I provide is kept securely through a PCI (payment card industry) compliant merchant system.
I authorize PMDI to debit my account as identified above to the terms stated here. This authorization shall remain in effect until PMDI receives a written notification form of intent to terminate at such time and in such manner as to afford PMDI a reasonable opportunity to act (Minimum 30 days).
I understand that if the total amount owed to PMDI is increased, I authorize this plan to continue as long as the payment amount remains unchanged until the amount owed PMDI is paid off, or unless the plan is terminated earlier by me as stated above. I understand any added amount can be applied for with a new authorization form.
All other changes such as payment amount, frequency, or account numbers change, will require me to update my information with PMDI 15 days prior to any change being implemented. I understand that this payment plan may be cancelled by PMDI due to ISF (Insufficient funds). I will be liable to pay an ISF fee of $15.00, which may be automatically debited for each ISF.
I represent and warrant that I am authorized to execute this payment authorization for the purpose of implementing this payment plan. I indemnify and hold PMDI harmless from damage, loss or claim resulting from all authorized actions hereunder.
Dress Code: Required dance wear and dance shoes must be worn to all classes. Failure to wear required dance attire to class could result in students being asked to sit out. For safety reasons street shoes are NOT to be worn on the dance floors and dancers may not wear jewelry. Hair must be kept up off the neck, if possible preferably in a bun. Tardiness: The school reserves the right to have students who come late to class sit out. The instructor will determine if the student will be able to warm-up effectively in order to avoid injury during class.
Attendance: If a student is in Preschool/ Kindergarten, 1st and 2nd Grade classes as well as 3rd, 4th and 5th Grade Academy classes misses class they are encouraged to take a make-up class. Students attending Studio, Conservatory or Institute Division classes are required to take a make-up class if they miss class. Make-up classes must be approved by the instructors and must take place the following week. The school reserves the right to deny any student who has missed four or more classes in their section after January the privilege of participating in the recital or other performance opportunities regardless if they take a make-up class or not. There are no refunds for missed classes or costumes.
Parent's Responsibility & Guardians: It is the responsibility of the parent, guardian or adult students to be aware of all school activities such as visitation day, dates the school is closed, when tuition is due and all schedules for picture day, dress rehearsal, recital and all other special events and performances. The school will post all such notices on the bulletin board and the studio website. It is the responsibility of the parent, guardian or adult student to inform the school of any address, telephone number and email changes. It is also the responsibility of the parent, guardian or adult to notify the schools of any student absences.
Care of Students: The school is not responsible for before or after class care for students. Students under the age of six may not be dropped off in front of the studio or be left unattended in the lobby. Students over the age of six are not to be left at the school for excessive amounts of time before or after class. If students are left at the studio and PMDI staff are required to supervise them the studio reserves the right to charge a babysitting fee of $8.00 for every 30 minutes.
Recital: The school is not responsible for size changes after measurements and orders have been taken. Costumes cannot be exchanged or returned once they are ordered. Tickets will be sold several weeks before the recital and due to fire codes all in attendance must purchase a ticket.
Class Cancellation: If Lincoln Public Schools are closed due to weather, PMDI will also be closed. In cases when weather becomes worse after school please call the studio's phone to check for cancellation. A cancellation notice will be on the answering system after 3:00pm. There will be no make-up for snow days as the calendar permits two snow days per class.
Combination Classes: Preschool/ Kindergarten classes preform one dance in recital. 1st and 2nd Grade classes perform two dances in recital. Preschool / Kindergarten, 1st Grade and 2nd Grade perform in one show please note which show prior to completing registration.
Visitation: Each year (Fall-Spring session only) there is a class visitation day (visitation date TBA). We ask that parents do not observe on regular class days as it is a distraction and can cause behavior problems.
Instruction Policy: Please note that it may be necessary for instructors to touch students in order to correct body placement during class. Physical student correction is a normal and necessary part of technique class. By signing this document you are acknowledging and accepting studio teaching policy and practices when it comes to your student's dance education.
Injuries: Parents or legal guardians of minor students and adult students waive the right to any legal action for any injury sustained on school property resulting from normal dance activity or any other activity conducted by the student(s) before, during or after class time.
Photo Release: The school is hereby granted permission to take photographs and video of the students or use yearly class and individual photos in brochures, websites, posters, advertisements and other promotional materials the school creates. Permission is also hereby granted for the school to copyright such photographs and video in its name.
