ASSUMPTION OF RISK: As the legal parent or guardian, I recognize that severe injuries can occur in activities involving height or motion, those activities including but not limited to dance, gymnastics, and tumbling. Being fully aware of these dangers, I hereby give consent for my child(ren) to participate in any activities at The Arts Center School of Music and Dance and affiliated entities, programs and activities and I ACCEPT ALL RISKS associated with such participation.
RELEASE OF LIABILITY: In consideration for me or my child(ren)'s participation I hereby, for myself and my child(ren) and our respective heirs and successors, PROMISE NOT TO SUE and FOREVER RELEASE The Arts Center School of Music LLC, their respective officers, directors, shareholders, employees, contractors and volunteers from all liability resulting from damages or injuries incurred as a result of participation including those resulting from acts of negligence.
MEDICAL EMERGENCIES: In the event of an accident or emergency I hereby authorize my child(ren) to be transported to a hospital for medical treatment and I hold The Arts Center School of Music and Dance, LLC and their representatives harmless in the execution of such. Additionally, I hereby agree to individually provide for all medical expenses which may be incurred by me or my child(ren) as result of any injury sustained while participating at or for The Arts Center School of Music and Dance, LLC.
PHOTO RELEASE: I am aware that individual and group publicity photos and videos are taken from time to time and in consideration for my child(ren)'s participation I hereby grant my permission for my child's likeness to be used in The Arts Center School of Music and Dance LLC publicity or advertising. The Arts Center will not post personal information without permission.
CLASS PLACEMENT: Advancement is based on the individual student's progress, as recommended by our teaching staff. The first month of classes the school director will evaluate each student's class placement. Proper placement is vital to the musician's and dancer's success. It is our aim to have students of the same skill level enrolled in the same class.
WEATHER CANCELLATIONS: In case of severe weather, please call The Arts Center office or visit the web site for cancellations. There will be a message indicating any changes in our schedule. Classes cancelled due to weather will be subject to available make-up times. No refunds on weather cancellations.
COMMUNICATION: The Arts Center communicates by email. Newsletters will be posted in the lobby of The Arts Center office and will be sent to our families by email on a monthly basis. Notify the office if you have a change of address, phone number, and/or email so you will stay informed with the latest news. Please be sure to read all the information and contact the office if you have any questions.
SUBSTITUTIONS: The Arts Center School of Music and Dance reserves the right to provide a substitute teacher if the regularly scheduled teacher is ill or otherwise unable to teach classes. If a teacher is ill and the studio cannot arrange a substitute, any missed classes will be made-up.
ATTENDANCE: In order to take full advantage of the instruction offered, regular attendance is essential. If you are unable to make class, please call The Arts Center office. There are no refunds
WITHDRAWS: Withdrawals must be done in person at The Arts Center office, not over the phone. To withdraw, a parent or adult student must complete and sign a withdrawal form provided by The Arts Center Office.
PERFORMANCE: The musicians and dancers have worked hard all year and this is their time to shine. In the spring, we will host a music concert and a separate dance recital. Participation in the annual spring recital is not mandatory, but is a beneficial and enjoyable opportunity for students. Please notify the The Arts Center office in writing by October 15th if you are not going to participate. Spring Recital information will be available in October.
Be on time and come prepared for each class.
Attend class regularly to achieve optimal results.
No street shoes are allowed in the dance studio.
No eating, gum chewing or drinking in the dance studio.
Positive and courteous behaviors are expected.
We never speak negatively about teachers, dancers, or parents from The Arts Center or other schools.
