Covid-19 Protocol
As a Parent /Guardian, you knowingly and willingly consent to attending Eagles Landing Dance Center during the COVID- 19 pandemic.
• I understand the COVID-19 virus has a long incubation period during which carriers of the virus may or may not show symptoms and could be highly contagious.
• I understand that I may have an elevated risk of contracting the virus by attending classes at Eagles Landing Dance Center.
• If I start to show symptoms of COVID-19 within 48 hours (2 days) of being at the studio I will contact the studio to let them know.
• I will let the studio know if my dancer tests positive for Covid-19, especially if they had been at the studio 2 days prior to the test.
• I understand that a dancer cannot attend classes at the studio while waiting for Covid-19 results to come back.
• I understand that if I have been around someone (within 6 feet for longer than 10 minutes) that has tested positive for Covid-19, then I must quarantine for 14 days, which means not attending classes in the dance studio.
• I understand that should my child test positive for Covid-19, they must quarantine for 10 days from testing positive and must be fever /symptom free for 72 hours before returning to the studio.
• I understand that no parents will be allowed in our waiting room for dancers 5 and up. For dancers under 5 years old, we will allow 1 parent in the lobby. That parent must wear a mask (no siblings) and must stay socially distant from other parents in the lobby.
• I understand that my child(ren) will need a mask for entering the studio and leaving the studio to wear when not socially distanced in the dance room. For all of our dancers, masks are optional, but are highly advised especially now while the spread is high and also especially for classes where the teacher will need to spot the dancer and be in a closer proximity. We advise dancers to wear more of a form styled mask that doesn't have the fabric directly up against their mouth. The ones that hold their shape and form away from their mouth and nose will be better for them. We will also closely watch dancers and anyone that is having a hard time with the mask will be allowed a break or to step outside and remove their mask if needed. These rules and procedures could change throughout the year due to the pandemic changing daily, but for now while the spread is high this is what we feel is best.
I confirm that I(parent/guardian) as I wait in the lobby, am not showing any of the following symptoms of COVID-19 listed below:
1. Fever-Temperature Above 99 Degrees
2. Shortness of Breath
3. Loss of Sense of Taste or Smell
4. Dry Cough
5. Runny Nose
6. Sore Throat
7. Chills
8. Headache or Muscle Pain
• I understand that at any given time, the class might be required to meet virtually. Example would be, if the guidelines by the county require us to close again or should there be too many cases. Our goal is to continue in person with the virtual option as long as we are allowed to do so. Should the classes have to switch to zoom classes, dancers will still remain enrolled and normal withdrawal policies still stay in effect. (See withdrawal policies)
• Withdrawal policies- Anyone wishing to discontinue classes must let us know ASAP, before the month before they wish to discontinue.
• In the event that we are notified that a dancer in a class does test positive for Covid-19, we will notify dancers that were exposed. Exposed is defined by the CDC as being closer than 6 feet for longer than 10 minutes.
• Classrooms will be wiped down between groups and all rooms will be deeply disinfected each night after classes. Should we have too many cases of Covid-19 and feel that the studio needs to be closed for an even deeper clean, we will notify everyone and classes will go virtual temporarily.
• If I choose "in person" training but needs to switch to online (zoom) training, I will let the studio know by 12noon on the day of the class by emailing info@eagleslandingdancecenter.org.
• If I choose "virtual training" and would like to switch to in person training, I know that I must email info@eagleslandingdancecenter.org to make sure that there is availability in the class to switch to in person.
• I confirm that I have read and will follow the studio's strict guidelines.
• I confirm that I have not traveled outside the United States within the past 14 days.
Registration Fees
A non-refundable registration fee of $40 for the first dancer and $15 each additional family member is due upon registration. Payment in full is required for the registration fee.
Spring Showcase Fees
Spring Showcase: We produce a fabulous production at the end of the year with all ELDC dancers. The show week (including dress rehearsals) will be May 31 - June 5. This is an exciting and rewarding part of the season that everyone looks forward to! We encourage all dancers to participate! Stay tuned for 2022 Spring Showcase details to be released throughout the year.
SPRING SHOWCASE COSTUMES: Costume costs are generally $65.00-$90.00 per costume (you will receive the exact amount in November 2021). Costume fees will be auto-drafted on December 18, 2021 & January 31, 2022. After these fees are processed, it is not refundable for any reason. Any costume alterations needed will be the responsibility of the parent. If your dancer will not be participating, you are required to notify us by email to info@eagleslandingdancecenter.org no later than December 5, 2021. Please do not rely on your dancer to inform us that she or he will not be dancing in the Spring Showcase!
The Spring Showcase Fee: This will be approximately $130 and will include 2 show tickets, t-shirt and more. This fee will be auto-drafted on March 15, 2022.
SPRING SHOWCASE TICKETS: Tickets to our Shows will need to be purchased. Dancers performing in the show do not need a ticket. More information on Show Tickets, including when they will be available to purchase, will be included in the Show packet released mid-April. The ticket cost will be $15! In order to ensure all dancers have the opportunity to have their immediate family watch them dance in the show, we may limit the number of tickets each family may purchase for a designated time period. After the designated time period, if tickets are still available, we may lift the limit and open ticket sales to everyone. Note: 2 tickets will be included in your Spring Showcase Fee.

2021-2022 CALENDAR:
We generally follow the Henry County School System Calendar for closing. Please note: We will NOT close for Henry County's Winter Break. Please see the full calendar for details. If you attend a school district outside of Henry County, please let us know if your child will be missing class (for example, a different Spring Break, etc.). Our Spring Show Rehearsals will be held during the days of Tuesday, May 31 and Saturday, June 4, 2022. A schedule with specific times for your dancer will be provided at a later time.

Dress Code
Proper shoes and attire must be worn to participate in class. Hair should be pulled back off the face for all classes. A proper bun must be worn for all ballet class. Don't forget to write your child's name in all shoes, bags, etc. You can find descriptions and examples of the proper attire for each class in the Dress Code section of our website.
Christmas Show Fees
For the past 14 years, the holiday production has been a tradition here at the studio, one in which we've looked forward to year after year.
We will be having our show on Sunday, December 12, 2021 (tentative). The time is TBD. The show will take place at The Henry County Performing Arts Center. The show will consist of dances taught by our ELDC instructors, and will be indicative of the work that they've done from August to December.
The cost to participate will be $50.00 which covers venue rental, videography & equipment rental and accessories. The dancers will not wear costumes. They will be wearing ELDC dance attire and additional accessories, if needed, and all dance shoes for each routine represented. More details will provided in September.
The rehearsals will take place on Friday and/or Saturday during the months of October and November. Exact dates will be provided in September.

Exemption Notice To Parents
Eagles Landing Dance Center is not a licensed facility with Bright From The Start. This is to advise you, the parent, that this is an exempt program through Bright From The Start.
Payment Policies
2021-2022 registration and tuition payment information: Eagles Landing Dance Center tuition is based on an entire season of dance. Our season runs from August 16, 2021 - June 4, 2022. All dancers are highly encouraged to pay for the entire season. If you enroll mid-season we will prorate your tuition based on when you begin dancing with us. Tuition is not based on the number of classes in a month or on attendance. There is NO refund for missed classes.
You may pay the entire season in full or pay it in monthly installments. At the time of registration, a payment for that month's tuition and the registration fee will be due. These fees are non-refundable. The other installments are due the 1st of each month, September 2021 - May 2022, and will be automatically billed to the card on file on the 1st of each month.
All monthly tuition payments, costume payments, and the Christmas & spring showcase fees will be auto-drafted to the card on file. You will be emailed a receipt after your tuition is auto-drafted. The recital fee will be approximately $130 and will include 2 year-end show tickets, t-shirt and more. Once the exact fee amount is determined, this fee will be auto-drafted on March 15, 2022.
If you opt to pay the entire season in full prior to August 31, 2021, you will receive a 10% discount. Please note: If you pay your dancer's tuition for the full year and then withdraw from classes for any reason (loss of interest, school obligations, injury, illness, etc.), a 30% penalty will be applied to your refund for the remainder of the year. We suggest you only pay the year in full if you are 100% confident your dancer will participate fully in the entire season. If for any reason we provide a tuition discount during one or more months of the season, this discount will not apply to prepayments. No further discounts are available.
RECEIPT OF PAYMENT: You will not receive a billing statement or invoice prior to any charges, however, you will receive a receipt of payment each month after the auto-draft.
LATE FEE: If your card on file is declined, an alternate payment must be made within 48 hours. A $10 Late Fee will be charged for any payments not made in full by the 5th of the month. This includes declined charges for cards on file for any reason.
PAYMENT AUTHORIZATION I hereby authorize Eagles Landing Dance Center to charge my credit or debit card for all services and products related to my family's enrollment in classes and activities at Eagles Landing Dance Center including but not limited to tuition, supplemental materials, special events, costumes, Christmas & Spring Showcase fees, accessories, etc. Fees are subject to change. If I elect to pay annual tuition via monthly installments, I understand that my credit or debit card will continue to be charged on the monthly, session or periodic basis unless I notify the studio in writing 30 days before intent to discontinue via email to info@eagleslandingdancecenter.org and receive an acknowledged returned response from Eagles Landing Dance Center staff. I understand that it will not be sufficient notice to merely tell an instructor or other employee of our intent to discontinue. I have read this entire agreement and understand that I will be held fully responsible for its terms and conditions of service, including a 30-day written notice by email to Eagles Landing Dance Center of any intent to discontinue. I understand that once the monthly installments for my annual tuition, my Christmas & Spring Showcase fee or my Spring Showcase costume fee have occurred there will be no refund given if I decide to discontinue. I agree to notify Eagles Landing Dance Center immediately of any change in the status of my charge account including but not limited to card expiration, name change, limitation of use, loss or theft of the card, etc. In the event that the amount charged is refused for whatever reason, I accept responsibility for full payment for the amount charged as well as an NSF fee of $35.00.

General Rules
Please discuss these rules with your dancers! Thank you!
No food or drinks other than water inside the studios. No exceptions!
No cell phones are allowed in the dance rooms at any time. If they are inside a dance bag, they should be turned off.
Please keep an eye on your children, no running or climbing on furniture PLEASE!
All children must be accompanied by a parent/guardian at all times unless they are in class.
All personal belongings may be kept in the studio with the dancer if needed. ELDC is not responsible for items left behind at the Center. Please make sure you check for all your belongings before leaving the studio.
Primary & Mini Combo Parents: Please take your dancer to the restroom BEFORE class begins to avoid accidents and disruptions to class.
Please make an effort to be prompt for class and in picking up your child! Excessive tardiness when picking up your child will not be tolerated.
No parents, friends, or siblings are allowed in the classroom at any time, unless given approval from the director. This is for the benefit of your dancer. They will learn more if we have their complete attention.
Viewing windows will be available, but you must get approval from the front desk representative.
If your dancer drops a class, a 30-day written notice is required. You will be responsible for payment until the written notice is received.
For the safety of our dancers, most times we follow Henry County's decisions on inclement weather school closings. However, you should always use your own judgement when deciding to travel due to weather. We will confirm our closing via email and the website when inclement weather affects Eagles Landing Dance Center. We will also communicate all closings on social media as well!

Withdrawal Policy
We require a 30-day written notice via email if you need to withdraw from Eagles Landing Dance Center. Please email info@eagleslandingdancecenter.org to withdraw from our dance season. After the 30-day notice, we will terminate your monthly billing, or, if a full year was paid, we will refund the remaining tuition installments minus 30%.
This withdrawal policy remains in effect even if we have a shelter in place mandate from the federal, state or local government. In the event of a shelter in place, classes will convert to virtual classes and instruction will continue. There may be an approximate one-week transition period from the date of the shelter in place order until virtual classes begin in order to allow us to implement our action plan. This brief time period, if needed, will be treated as inclement weather days. No refunds will be given for this week. Remember that a 30-day written notice will still be required for withdrawal.
Performing Company and Ensemble Members Only: If we are mandated to close our physical location due to any government order, classes will convert to virtual classes and instruction will continue. If your dancer chooses to withdraw due to this change in format, you will receive a full refund of all company fees minus any costs that have already been paid out. These include costumes, competition, conventions, etc. If you decide to withdraw for any other reason, all original penalties will apply.
Waivers of Liability
PHOTO RELEASE: I hereby grant Eagles Landing Dance Center permission to use my child's photograph/video for promotional purposes and/or social media and Eagles Landing Dance Center's website.
RELEASE OF LIABILITY: I understand dance can be a strenuous activity and accept responsibility for any injuries my child may incur while participating in any activity at Eagles Landing Dance Center. Eagles Landing Dance Center and its employees are not liable for personal injury, or loss of or damage to personal property.
ASSUMPTION OF THE RISK AND WAIVER OF LIABILITY RELATING TO CORONAVIRUS/COVID-19: The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies have put in place recommended social distancing, occupancy restrictions and guidelines for businesses to follow. Eagles Landing Dance Center has put in place preventative measures to reduce the spread of COVID-19; Eagles Landing Dance Center will be implementing all recommendations from Georgia state officials as per ELDC's Sanitation Practices & Social Distancing Guidelines document available on our website.
Eagles Landing Dance Center will make every effort to keep our studio sanitized and to prevent symptomatic employees and dancers from entering the building. However, COVID-19 has been proven to be asymptomatic in some persons. Therefore, Eagles Landing Dance Center cannot guarantee that you or your child(ren) will not become infected with COVID-19 or any other unknown virus. Further, attending Eagles Landing Dance Center could increase your risk and your child(ren)'s risk of contracting COVID-19.
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending Eagles Landing Dance Center and that such exposure or infection may result in personal injury, illness, permanent disability, and death. By signing this agreement, I agree that my child and any accompanying persons will not enter Eagles Landing Dance Center if we exhibit any of the following new or worsening signs or symptoms of possible COVID-19: A temperature greater than 100.4 degrees Fahrenheit, cough, shortness of breath or difficulty breathing, other respiratory symptoms, or at least two of the following symptoms: chills, repeated shaking with chills, muscle pain, sore throat or new loss of taste or smell. I also agree that, neither my child(ren) nor any accompanying adult will enter Eagles Landing Dance Center if we have had known close contact with a person who is lab-confirmed to have COVID-19.
I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)'s attendance at Eagles Landing Dance Center. On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless Eagles Landing Dance Center, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Eagles Landing Dance Center, its employees, agents, and representatives, whether a COVID-19 or other currently unknown virus infection occurs before, during, or after participation in any Eagles Landing Dance Center program or class.
