COVID 19/ Infectious Disease or Illnesses
I understand that Thrive Gym LLC. follows the guidelines put in place by the CDC and local health departments in regards to all viruses and infectious diseases including but not limited to COVID-19. I understand that Thrive Gym LLC. requires that ALL staff, volunteers, families, guests, and visitors maintain physical distancing and sanitization requirements and comply with any health and safety policies implemented by Thrive Gym LLC. By acknowledging that you comply with the guidelines above, failure to do so may result in your removal from the premises.
Assumption of Risk
I understand that any activity involving motion, tumbling, height, swinging, etc., involves the possibility of serious, permanent or fatal injury. I understand the risks of participating in the sport of gymnastics and physical activity.
Release of Liability
I hereby forever release Thrive Gym LLC., it's owners, officers, employees, teachers and coaches from all liability for any and all damage and injuries suffered by my child/children while under the instruction, supervision or control of Thrive Gym LLC. it's owners, officers, employees, teachers, coaches and booster club. This acknowledgment of risk and waiver of liability, having been read thoroughly and understood completely, is signed voluntarily as to its content and intent.
Medical Emergencies
I hereby give permission for my child/children to participate in classes/events conducted by Thrive Gym LLC.. I understand that it is my responsibility to carry my own accident and medical insurance. In the event of an injury or accident, I authorize customary medical treatment if it becomes necessary, and transportation and emergency medical services if warranted. The enrolled child/children is/are capable of participating in the sport of gymnastics and have had a physical within the last (12) twelve months.
Payment Policies
I understand that I am responsible for the payment of the classes that I am enrolling my child/children. I understand that Thrive Gym does not offer tuition refunds for any reason. I understand that if my child does not attend class, I am still responsible for the payment of tuition. A credit or debit card on file is required to register for classes. All tuition is due on the 25th of each month (for the following month) and can be paid by cash, check, debit, or credit card. If tuition is not paid by the 25th and no one has informed us of dropping a class, all unpaid balances will be charged to your card on file. If your payment does not go through for any reason, you will receive an email regarding a problem with your account. If delinquent tuition is not paid before the first of the month, your child will be tagged inactive by the computer, will be un-enrolled from class, and children on the waiting list will be called. Once we have taken a bad check, all tuition must be paid from then on by cash or debit/credit card.
Year Around Enrollment
I understand that Thrive Gym offers monthly enrollment instead of session. I understand that monthly tuition is based on a 4 week month. If there are 3 classes in the month I will not receive a refund and if there are 5 classes in a month I will not be charged extra. I understand that is is my responsibility to notify Thrive Gym in writing when I want to end my child's enrollment through a drop form at the front desk. If the request is made before the 15th of the month, my child will end class on the last day of the month. If the request is made after the 15th of the month, my child will end class on the last day of the next month.
Make Up Classes
I understand that if my child will be missing a class and I would like for them to have the opportunity to make up the class, I must call ahead of the missed class to let Thrive Gym know my child will not be attending. At that time I may schedule a make up class. Make up classes must be scheduled within a month of the missed class date and can be done prior to the missed class if appropriate. Students may do make ups in any age and skill appropriate class.
