Having been informed of the activities to be conducted by the Bart Conner Gymnastics Academy Inc., I, a parent or guardian of the participant, give my approval for the above named student's participation in any and all activities of the program, including the use of inflatables. In consideration of my or the student's membership acceptance in the Bart Conner Gymnastics Academy, Inc.,I hereby forever waive, and forever release and discharge the Bart Conner Gymnastics Academy, Inc., and Paul Ziert & Associates, Inc., their officers, owners, directors, professional consultants, and employees, from all liability for any and all damages and injuries suffered by the participant in connection with said use of the aforementioned equipment, instructors, and facilities.
Severe Weather Policy for Bart Conner Gymnastics Academy
In the case of a tornado or other severe weather, the Bart Conner Gymnastics Academy is not to be considered a safe shelter. As a parent/guardian, it is your responsibility to determine whether your child should attend gymnastics on any day where severe weather could be a threat. We have cable television and will be monitoring the situation as it unfolds. With this in mind, we have the following procedures in regard to severe weather.
1) If your child is at the gym and you feel they need to be picked up, because of severe weather, you are welcome to do so. It is your responsibility to stay informed and aware of the weather situation. You may pick your children up at any time.
2) If some children remain at Bart Conner Gymnastics during a severe weather emergency and, if time permits, the BCGA management will evacuate all the children and staff to the Norman Healthplex on Tecumseh. This facility is an approved storm shelter which is one mile from the gym. By enrolling in our program, you authorize management to make the judgment and give us permission to transport your child to that location, in your absence. We will make every effort to keep you informed of the situation.
3) If management determines there is not sufficient time to transport your child to the Healthplex, we will stay at the gym. We will gather whomever is remaining and go into the pits. This is a 3 1/2 foot deep reinforced concrete, underground trench.
We have everyone's utmost safety in mind and these are the procedures that we will perform under a imminent severe weather threat.
