COVID 19
In consideration of being allowed to participate in classes with ACROSS THE FLOOR and related events and activities, the undersigned acknowledges, appreciates, and agrees that:
1. Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and,
2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,
3. I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring attention to ACROSS THE FLOOR immediately; and,
4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS ACROSS THE FLOOR, their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct classes and other events (RELEASEES), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IF FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. FOR PARTICIPANTS OF MINORITY AGE (UNDER AGE 18 AT THE TIME OF REGISTRATION)
This is to certify that I, as parent/guardian, with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/ward including the risks of presence and participation and his/her personal responsibilities for adhering to the rules and regulations for protection against communicable diseases. Furthermore, my child/ward understands and accepts these risks and responsibilities. I for myself, my spouse, and child/ward do consent and agree to his/her release provided above for all the Releasees and my spouse, and child/ward do release and agree to indemnify and hold harmless the Releasees for any and all liabilities incident to my minor child EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent provided by law.
Release of Liability
As the legal parent or guardian, I release and hold harmless Across the Floor School of Dance & Gymnastics, Inc., its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Across the Floor School of Dance & Gymnastics, Inc., its owners and operators or in route to or from any of said premises.
In addition, I give permission for my child to be photographed/video taped during the program. I understand that the photographs/videotapes will be used by Across the Floor for the purpose of publicizing and promoting the studio's programs and services. Furthermore, compensation will not be expected.
I fully understand and accept the terms and conditions of this agreement. I will be responsible for all charges/payments for this account.
Medical Emergency
The undersigned gives permission to Across the Floor School of Dance & Gymnastics, Inc., its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restictions, or condition and/or declare the paricipant to be in good physical and mental health. I request that our doctor/physician be called and that my child be transported to the hospital.
Policies
SCHOOL POLICIES
*Our school year runs from September through June based on a 38-week session including holiday closures, Across the Floor events and an annual recital.
*Students are encouraged to make up any missed classes. Make-up classes must be completed within the month they were missed. Once recital routines are in progress, make-up classes are not guaranteed. Holiday closures and Across the Floor events do not require make-ups.
*No eating, gum chewing, or drinking in the studios and dressing room.
*No students are allowed in the studio without an instructor.
*Disruptive behavior is not tolerated in the classroom. Students will be dismissed.
*Parents are welcome to stay and wait for students in our reception area. Disruptive or inappropriate behavior from parents, their guests, and visitors will not be tolerated and will be asked to leave.
ABSENCE
When possible, students are encouraged to make-up any classes missed based on the availability of the class schedule. However make-up classes are not guaranteed. There are no credits or refunds issued in cases where classes cannot be made up due to personal scheduling conflicts, illness, etc.
CANCELLATION POLICY
We reserve the right to cancel or combine classes with insufficient enrollment and to change the time, day, and instructor when necessary. In these instances, refunds will be issued.
Fees/Refunds
*There is an annual registration fee of $30.00 per student/ $50.00 per family enrolling.
*There is a late fee of $15.00 charged on tuition payments made after the 15th of the month for those in payment option 1. No exceptions regardless of absence, holidays or vacations.
*There is a $25.00 service charge on returned checks.
*Any changes in class scheduling (drop or switch) will incur a $10.00 processing fee, after 1 month of classes taken, unless initiated by Director or staff.
*If a student adds or drops a class mid-month, a charge or refund amount will be pro-rated.
*If a student withdraws enrollment from the school after 1 month of classes taken, a withdrawal fee of 1 month's tuition (not at discounted rate) will be applied. The remaining balance will be refunded.
*Costumes are non-returnable. Once a costume has been ordered, you are responsible for full payment. There will be no refund/credit issued on any costumes ordered.
*There will be a $15.00 fee for any costume exchanges.
*There will be a $10.00 late fee charged on costume deposits and balances for each week past the due date.
*There will be no refunds for any missed classes not made up.
