Annual Registration Fee
I agree an annual enrollment fee of $15 per dancer and $20 per immediate family will be collected at the time of registration.
Tuition & Billing
I am aware Expression Dance Studio will send me an invoice, via email, 14 days prior to the beginning of the upcoming session (PLEASE DOUBLE CHECK THE E-MAIL ADDRESS PROVIDED IN THE CONTACT #1 FIELD ABOVE). Tuition is due the 1st of every month. A late fee of $15 will be applied after the 5th of every month. Tuition is based off of 10 month-long sessions. You do not need to re-enroll every month. If you choose to withdraw simply call or stop in the office to complete the withdrawal process before the new session begins on the first of the month A $20 fee will be charged to all returned checks. There are no refunds given for dancers who discontinue taking classes in the middle of a session. The tuition rate is a flat monthly fee, regardless of how many classes are in the month. You are not charged more when there are 5 classes in a month. Similarly, you are not charged less during months when there are fewer than 4 classes due to planned studio breaks.
Payment Policies
I am aware Expression Dance Studio will bill my credit card for the first month's tuition within 1-2 business days of registration. After the initial registration, Expression Dance Studio accepts cash, check, or credit card as payment. Payments may be mailed to the studio or turned in to the office. Payments may also be made online. If payment is not made before the 1st of the month, your credit card on file will automatically be charged for the balance due.
Refunds
I understand Expression Dance Studio will not issue refunds for any missed class or early withdrawal during a session. Dancers are encouraged to complete a makeup class(es) in the event of an absence. Classes must be made up before the recital in June 2021.

I understand if any of my contact information should change, especially my email address, I will contact the studio to provide this new information for billing and mailing purposes.
Parking Agreement
All parking requirements are in place for our students' best interest. The safety of our dancers and families is of
utmost importance to Expression Dance Studio and the Village of Westmont. We have worked closely with the
Westmont Police Department to develop the safest system for your children. This document must be read,
initialed, signed and dated before your dancer's first class at Expression Dance Studio.
1) Expression Dance Studio's parking is located directly south of the 25 S. Cass Building. ( )
2) Vehicles must enter the studio parking lot by traveling north on Cass Avenue; making a direct right turn into
the parking lot. ( )
3) Vehicles are not allowed to enter the lot by traveling south on Cass Avenue. Left turns and crossing through
traffic are strongly discouraged. ( )
4) Clients must park in one of the 16 exclusive dance studio-parking places to "drop off" your dancer. ( )
5) Vehicles are NOT allowed to stop in the middle of the parking lot to drop off dancers. ( )
6) Clients must exit through the rear of the parking lot directly onto Richmond Avenue. ( )
7) The Studio Parking Lot is a ONE WAY lot, with the only entrance on Cass Avenue. Vehicles may not enter
the parking lot from Richmond Street. Going against the traffic flow is strictly prohibited. ( )
8) Clients are not allowed to use the North Lot that is shared with L&F Pizza and Tasty Treat. Vehicles in
violation are at risk of being towed. ( )
9) Parking is available on the west side of Cass. Students and parents who choose to park on the west side
are required to cross the street at the intersection of Cass and Richmond, or the crosswalk at East Quincy and
Cass Avenue. ( )
10) I have read through all of the parking requirements and agree to abide by all of the rules. ( )
Please note that the Westmont Police Department strictly enforces the above rules and those in
violation are at risk of parking tickets or towing.
Video & Photo Waiver
I give Expression Dance Studio my permission for the public display of pictures and or video of my child for possible advertisement purposes (i.e. brochures, newspaper ads, web site, etc. - no names will be used).
Class Cancellation Policy
In the rare event a class may be cancelled due to inclement weather, dancers may make up the class anytime throughout the year. Simply choose a class in any equal level and age.
Medical Emergencies
I give my permission to Expression Dance Studio. and its owners and operators to seek medical treatment for the students listed above in the event they are not able to reach a parent or guardian. In the fields above, I have declared any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health.
Release of Liability
As the legal parent or guardian, I release and hold harmless Expression Dance Studio, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant while in or upon the premises or any premises under the control and supervision of Expression Dance Studio, its owners and operators or in route to or from any of said premises.
EXPRESSION DANCE STUDIO has taken certain steps to implement recommended guidance and protocols issued by the Public Health Agencies for slowing the transmission of COVID-19, including, without limitation, the protocols listed in the "Safe Studio Guidelines". The undersigned acknowledges and agrees that EXPRESSION DANCE STUDIO may revise its procedures at any time based on updated recommended guidance and protocols issued by the Public Health Agencies and further agrees to comply with EXPRESSION DANCE STUDIO's revised procedures prior to utilizing the facilities, services, and programs of EXPRESSION DANCE STUDIO. The undersigned further acknowledges and agrees that, due to the nature of the facilities, services, and programs offered by EXPRESSION DANCE STUDIO, social distancing of 6 feet per person among children and their teachers, employees and agents in a class setting may not be entirely possible. The undersigned fully understands and appreciates both the known and potential dangers of utilizing the facilities, services, and programs of EXPRESSION DANCE STUDIO and acknowledges that use thereof by the undersigned and/or such participating children may, despite EXPRESSION DANCE STUDIO's reasonable efforts to mitigate such dangers, result in exposure to COVID-19, which could result in quarantine requirements, serious illness, disability, injury and/or death.
Class Withdrawal
In the event that you choose to withdraw your dancer from class, you must fill out a class withdrawal form found in the front office. You will be charged for class until you complete this process.
