As legal guardian of my designated student(s) (student(s)), I hereby consent to all student(s) participating in Barlow Arts Conservatory's program(s). I recognize that potentially severe injuries can occur in any activity involving height or motion, including dancing, related activities, and physical activity in general. It is also understood that dance instruction involves kinetic corrections that may include physically touching the student as part of regular classwork and rehearsals. I understand that it is the express intent of all staff and personnel of Barlow Arts Conservatory to provide for the safety and protection of my student(s) and, in consideration for allowing my student(s) to use these facilities, I hereby COVENANT NOT TO SUE and FOREVER RELEASE Barlow Arts Conservatory, affiliated and partner companies and organizations, staff, contractors, teachers, coaches, owners, directors, and other members involved in this facility's program(s), from all liability and for any and all damages and injuries suffered by my student(s) during instruction, supervision, and/or control during any and all classes or extra activities.
I understand that the registration fee of $30 is non-refundable. I understand that tuition for the Summer Intensive and the Musical Theatre Camp is also non-refundable.
Tuition for the year is based on the number of hours danced per week, costume rentals, performance fess, digital downloads of the performances, pictures, and insurance. It is calculated for the entire session, which starts in the Fall, and runs until late Spring, specific dates vary from year to year. Tuition may be paid in full at the time of registration to receive a discount, or paid in 11 equal installments. Annual tuition paid in full must be paid by cash or check and is due upon registration. A valid credit/debit card or ACH information must be provided for installment payments. Tuition is due on the 15th of each month, or next business day. All accounts will be charged a $25 late fee after 5 days, and the student will not be permitted to take class until overdue tuition is paid in full.

I understand the the 2018/2019 Session runs Aug. 27th, 2017-May 31st, 2018 for the Lehi and Murray location. I agree to pay the published tuition rate and fees, plus a $30 non-refundable registration fee per student. If I choose to withdraw I agree to the $50 withdraw fee and understand that I must give 4 weeks notice in writing. I agree that I am responsible for the publish tuition rate four weeks after I give written notice that we will be withdrawing from Barlow Arts Conservatory.
I represent and warrant that if I am purchasing something or paying for a service from Barlow Arts Conservatory or from other merchants through this facility that (i) any credit card or bank account draft (ACH Draft) information I supply is true and complete, (ii) charges incurred by me will be honored by my credit card company or financial institution, and (iii) I will pay the charges incurred by me at the posted prices, including any applicable taxes, fees, and penalties.
I hereby authorize (if online payment is made or autopay information is provided) this facility to charge my ACH draft, or credit card account. I understand that a 30 day written notice is required to terminate billing and I am responsible for payment whether or not my student attends classes until I notify this facility in writing to drop my student from class(es).
Should I dispute a charge through my financial institution this will constitute a breach of contract possibly resulting in, but not limited to, penalties, additional fees, collection, legal action, and/or termination of any and/or all current and future services.
Parents must notify the BAC office in writing of any student dropping from classes, and must receive a confirmation from the BAC Director. Dancers in Petite, Trainee, Apprentice, Aspirant, Jr., Teen, Sr., BBTII, and BBT are responsible for the full annual tuition, with no option for withdrawal or refunds. Fairy Feet students must notify the BAC office in writing of their withdraw. They will be responsible for tuition 30 days after the email was sent, and must pay a $50 withdraw fee.
Early Drop Off/Late Pickup
Often teachers are arriving to unlock the studio, and staying until all students are picked up. Please be respectful of our time and make sure students are dropped off and picked up no more than 10 minutes before or after their class. An additional $10 fee for late pick-up may be charged if a student is not picked up on time.
I understand if my dancer is accepted into the ballet companies he or she is required to perform in The Nutcracker, Ballet Gala, and the Spring Performance either La Bayadere and/or Peter and the Wolf failure to perform in any of the above productions may result in dismissal from the company at the sole discretion of BAC Directors. I also understand that by participating in a Ballet Company I will need to sign a company agreement and participate in all company activities. I also understand that as a company member of a ballet or musical theatre company that I am committing to the entire year and I am responsible for the full years tuition and fees associated with these companies. If I choose to withdraw early I am still responsible for the entire year's tuition. I understand that company tuition and fees are non-refundable.
Casting for all BAC productions is done by the BAC artistic staff and faculty.
Students are advanced to the next level during our placement auditions and on an individual basis. Some students may stay in the same level for two or more years. This is completely normal, and should not have a negative connotation. Advancement is considered according to the following criteria:
* Attendance: Does the student show a dedication through their consistent attendance.
* Technical Development: Every level has its own set of technical skill mastery that is required to advance to the next level. Some of the aspects of technical development that are considered are: alignment and proper placement, use of turn-out, coordination, classical line, physical strength, musicality, and performing skills (in class, rehearsals and stage).
* Attitude: Students must maintain a positive attitude before, during, and after class. Students must show a willingness to receive corrections from teachers. They must also maintain a good work ethic and professional relationship with BAC faculty and other students.
Parents are not allowed in studios during class time. BAC does hold occasional in-class parent observation days, and parents and family are encouraged to attend. Please be respectful and arrive early and stay through the entire performance.
Our Policy Toward Nondiscrimination
BAC welcomes all students interested in studying dance. We do not discriminate based on race, nationality, color, or ethnic origin.
The BAC Director and faculty determine level placement for all dancers. Level placement is for the year, and once placed in a class a student's level will not be changed or reviewed. Parents and students are asked to trust BAC faculty on what is the best placement for an individual dancer. Careful consideration is given to each dancer on what is the best place for them to flourish and achieve, and every level is taught by highly trained and experienced teachers.
What to wear to class
Boys may wear tucked in T-shirts and shorts purchased through BAC. Girls must wear the assigned class Leotards also purchased through BAC. More information on class leotards will be given at time of registration. NO chewing gum or dangling jewelry. Hair should be pulled neatly and securely up into a ballet bun for girls, and kept out of the face for boys. Girls should not wear bows or other large hair ornaments. All students should have activity-appropriate footwear during class. Personal items should be left at home. Jewelry should not be worn during classes. PLEASE LEAVE JEWELRY ARTICLES AT HOME. BAC faculty and staff is not responsible for ANY items that may be lost or stolen. Be sure your student's personal items are marked with their name.
BAC will maintain a lost and found throughout the year that will be donated at the end of each year session.
Barlow Arts Conservatory does not pro-rate tuition or give credit for missed classes due to illness, vacations, weather related closures, or other conflicting activities. Missed classes may be made up by attending class for the level immediately below the dancer's class level.
Students are expected to attend all regularly scheduled classes and rehearsals. Please do not rely on your student to verbally let us know that he/she will not be attending class. Any student that will miss a class for a scheduled activity or for illness or injury must communicate the absence to the BAC office via email, and receive a confirmation of receipt from BAC office staff. Repeated excused or unexcused absences may be grounds for dismissal.
Barlow Arts Conservatory reserves the right to dismiss any student at any time for any of the following: negative attitude, distracting or disrespectful behavior, class attendance, poor work ethic, missing performances, overdue balances, and any other activity that is deemed in violation of BAC policies and standards. If a student is dismissed, all tuition, enrollment, performance, or any other fees paid to BAC are non-refundable.
BAC maintains the following code of conduct at all studio locations, rehearsal and performance spaces, or during any competition or other activity or event where BAC dancers are present:
* No use of tobacco, drugs or alcohol
* BAC has a strict zero tolerance policy for bullying or other harassing behavior such as: teasing, demeaning comments, insults, biting, or any other form of unwanted physical contact
* No stealing or vandalizing any property belonging to another student, or to BAC or its faculty members.
* No inappropriate or disrespectful behavior or language
* For security reasons, students must wait inside the building for pickup.
* Students voices must be kept low enough so as not to disturb other classes or offices staff.
* No food or drink is allowed in any BAC studio, except for water. Students are encouraged to bring a refillable water bottle.
BAC does not offer refunds for missed classes, even with an excused absence. No exceptions. However, for any excused absence, students may take a make-up class with the level below the student's current level. Make-up classes must be approved by BAC office staff.
