Release of Liability

I hereby release, indemnify, and hold harmless CELTS Swim School, their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and owners and lessors of the premises used for the activity ("RELEASES"), with respect to any and all injury, disability, death, or loss or damage to person property, whether arising from the negligence of the RELEASES or otherwise, to the fullest extent permitted by law. The terms of this release shall serve as a release and assumption of risk for me and my son(s)/daughter(s), heirs, executors, and administrators and for all family members.

Assumption of Risk

The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular skills, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist. I knowingly and freely assume all such risks of injury or loss, both known and unknown, even if arising from the negligence of the Releases or others, and assume full responsibility for my son(s)/daughter(s) participation.

Use of Image Statement

Coach Eve Learn To Swim, LLC Dba CELTS Swim School often host events that are open to the public such as, but not limited to, athletic competitions, water safety fairs, swim lessons, and various activities held at sports arenas and aquatic centers. Those events are considered news events. Such an event may be photographed, video-taped or Webcast for purposes of archiving the event, educational use, or publicity. Participants, staff, and the public who attend those events may have their image or voice captured on video, Webcast or photograph(s). Due to the nature of these events, Coach Eve Learn To Swim, LLC and its various other properties (Chicago Southland Aquatic Teams, CELTS Swim School, Swim Chicago Southland, ACHIEVE Chicago Southland, H2WHoa Pool Parties, Gazzilion Gallons of Fun), has no means by which to prevent such photographs, videotaping or webcasting from including a specific attendee's image or voice. By attending the event, such person is granting the site and Coach Eve Learn To Swim, LLC the right to use any such still or motion images or voice recordings in future publicity or publications, including web postings as needed and without compensation. No release shall be required by Coach Eve Learn To Swim, LLC or its sites to utilize in an appropriate manner any images captured during a public event, even if the subject is a minor.

Refund/Cancelation Policy

Register for a class that will fit into your schedule. Rescheduling for classes prior to the start will be allowed, depending upon available space. No Rescheduling will be allowed after classes have begun. Coach Eve Learn To Swim has a NO REFUND policy. We do not refund tuition for missed classes. If there are extreme circumstances such as broken bones, etc. a doctor's note given to the director or site supervisor will allow your account to be put on hold until your child can resume lessons.

Make-Up Policy

Pool time is secured for in advance and in order to maintain the pace and small size of our classes, we do not do make-up lessons for any reason including sickness, recitals, or vacations. Lessons are non-refundable. We have found that this policy allows us to provide the highest quality experience for your child

Payment Policies

All registration fees and tuition are due at time of enrollment and are nonrefundable. If a check is returned for any reason, a $25.00 fee will be charged to your account. Once notified, payment is due immediately. Non-payment will result in your reservation being cancelled.

Medical Emergencies

Please notify our director if your child has any medical problems, disabilities or fears. Do not bring a sick child to class. This includes yellow or green nose secretions, pink eye, fever, etc. If your child displays any of these or other symptoms of illness, it may be necessary to remove the child from class.

In an Emergency The undersigned gives permission to Coach Eve Learn To Swim, LLC Dba as CELTS Swim School, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health. I request that our doctor/physician of record be called and that my child be transported to the closest emergency center or hospital.

Signature Agreement

I have read this release of liability and assumption of risk agreement, fully understand its terms, acknowledge that I am authorized and empowered to sign this agreement, understand that I have given up substantial rights by signing it, and sign it freely and voluntarily without any inducement.
