In connection with the registration of yourself and/or child for the dance, music, theatre arts classes/camps to be conducted by Greensboro Performing Arts, Inc (GPA), and in consideration of your enrollment and/or consent of said student(s) in such classes, I/we the undersigned parent(s) and/or legal guardian, acknowledge that I/we have been informed fully of the activities to be offered to said student and consent thereto; participating in the programs to be offered by GPA, Inc; I acknowledge the risks and hazard of physical injury inherent in dance, music, and/or theatre arts classes and hereby assume all such risks and hazards; and I/we hereby release, waive and agree not to assert against GPA, Inc, its owners, employees, contractors, or volunteers, any claim for injury to said child in consequence of or incident to such training. In case of an emergency, if GPA staff is unable to reach me/us by phone, or it is a situation that appears to require immediate emergency medical assistance, I/we hereby give my/our permission for the staff to contact 911 and secure treatment for said student.
CLASS ATTENDANCE POLICY: Regular attendance to all classes is necessary for participation at GPA. Please be on time for all classes, rehearsals, and performances. (Ten to fifteen minutes prior to class start is appropriate "on-time" policy in order to assure dancers begin class promptly at scheduled time). Communicate any and all delays, emergencies or absences as soon as possible by text and/or call GPA's office line 336-549-2228. It is the student's responsibility to arrange to learn choreography from any missed class or rehearsal and must be prepared by the following rehearsal time. If teachers decide that in any technique a student is not making the attendance commitment required, changes will be made in the Fall prior to costumes being ordered. GPA staff reserves the right to discontinue classes for any student misconduct or inappropriate behavior.
DANCE ATTIRE: Appropriate dance attire is required for all dance classes. This enables the teacher to properly observe the students body movements for correct form and technique. Hair must be arranged securely in a bun with all hair smoothed and off the face. Absolutely NO JEWELRY as it is a hazard. Dance shoes ONLY are worn inside the studio. Please care for your dance shoes and our floors by not wearing them outside the studio. Street shoes are not permitted on the dance floor at any time. Please see the GPA dress codes flier for detailed information. Remember to write both first and last name in all shoes and other items with a permanent marker.
CLASS LEVEL AND STAFF: For student safety, dancers are only permitted to register and attend classes at the level the Greensboro Performing Arts staff places them. We will assess and adjust levels/classes during the first two weeks of Fall classes and will continue to assess throughout the year. Please note Ballet/Dance Technique classes are required in order to register for certain jazz/hip hop classes. No student or parent may promote themselves or their child into a higher level without teacher approval. Parents may request an evaluation of student progress at any time.
INFORMATION AND HOLIDAYS are posted through the customer portal, on the studio bulletin board, and at www.GreensboroPerformingArts.com. We do not follow the Guilford County Public School Schedule for class cancellations. Stay informed by checking the bulletin board, website, and customer portal. It is the parents' responsibility to follow up and stay informed with studio current events. Email and text reminders as well as monthly newsletters are used for reminders upcoming events, closings, holidays, etc. Any emergencies or inclement weather cancellations will be posted through the customer portal, on the website and/or communication will be received via email, text and phone.
I hereby authorize Greensboro Performing Arts, Inc., to charge my bank account or credit card for all services related to my family's enrollment in classes and activities at Greensboro Performing Arts, Inc., including but not limited to tuition, special events, costumes, and performance fees. I understand that my credit or debit card will continue to be charged on a monthly, session or periodic basis unless I notify the office otherwise. I have read this entire agreement and understand that I will be held fully responsible for the terms and conditions of services rendered. I agree to notify Greensboro Performing Arts, Inc. immediately of any change in the status of my bank or credit card account including but not limited to card expiration, name change, limitation of use, loss, or theft of the card. All such information may be updated through the new GPA app and customer portal.
LATE FEES AND DRAFT NON-SUFFICIENT FUNDS (NSF): Tuition is due by the 1st of each month. After the 10th day of the month, a $10 late fee will be charged for outstanding monthly payments and $2.00 per additional day the account remains unpaid. Declined credit card charges or returned checks will be charged a $30 NSF fee. I understand that my tuition is non-refundable and will not be adjusted for absences or for discontinuing classes mid-month. Please contact instructor concerning possible make-up class options suitable to your level.
COMMUNICATION: If there are specific needs, such as appointments for assessments, photo shoots, questions about rehearsals, pointe shoe fittings, questions about placement, comments, suggestions, complaints and so forth; kindly direct such inquiries to firstname.lastname@example.org. All appointments must be coordinated through the front office, in order to give consideration to both studio and staff availability.
This mode of communication helps us filter questions for our teachers and allows our teachers to address your concerns in their time, via email messages, or even text messages, as each teacher prefers. Please refrain from phoning our teachers or contacting through social media. Many of them have pressing issues to address when they are away from the studio, and we know that you want to respect and support that. Further, they have their days planned so that they can prepare, take care of their bodies, spend time with their spouses, families, etc.
