Due to Covid precautions: we are asking that all payments be made online. All major credit cards are accepted. If you need to make an in person payment, contact our office to set up payment.

We are accepting only quarterly payments.
In order to have less contact we are going to move to all online payments. Please use your email we have on file to enter your account. If you are not sure, you can check with us to confirm your email. Please do not start a new account if you are a current student.
No tuition refunds will be issued after September 7th. If at any time you wish to discontinue, we ask that you finish out the quarter or non-recital 8 week session then you will not be obligated to continue once the quarter/session has ended.
In the event of a government shutdown or studio closure, all classes will transition to virtual instruction.
If after two weeks of virtual instruction, we are still on the Virtual Plan. Families who choose to not participate in virtual learning after two weeks, may freeze their account and receive a credit for the balance of the current quarter to use toward future classes. Credits will not be given before the built-in two weeks of virtual learning via the Virtual Plan. Refunds for tuition will not be issued after September 1st even under the Virtual Plan.
CREDIT CARDS WILL BE CHARGED WITHIN 7-10 BUSINESS DAYS OF REGISTERING.
TUITION AND COSTUME PAYMENTS WILL BE TAKEN OUT AUTOMATICALLY. Costume Payments will be taken out automatically between 11/15/20- 11/20/20. No costume refunds after 11/30/2020.
We, at Mulford Dance Studio, never see your information nor do we store it.
We use a credit card processor that has a secure system.
STUDENTS MUST BE REGISTERED BEFORE STARTING CLASS.
IMPORTANT NOTE ABOUT ONLINE REGISTERING: This online account system does not make adjustments for tuition discounts or family registration fee discounts, nor does it automatically apply gym insurance fees. Therefore, we must manually make the adjustments to apply the discounts. Once these adjustments are made, you card will be charged.

Please note: We strongly encouraged parents to complete transactions online if possible. We can only accommodate 2 customers at a time in the studio, so you may need to wait outside.
REFUND POLICY
No refunds on the registration fee.
Make-up Classes. If a student misses class they can choose to make it up in another class appropriate to their age and level.
Students may attend class virtually if they are not feeling well or for other reasons.
*We do not give refunds for missed classes.

Fall season 2019-2020 Sept-June TUITION TERMS AND CONDITIONS FOR BILLING.
10 Installment Plan Option: The 1st and 10th Installment is DUE at Registration and then starting with October, on the 1st lesson of each month through May. Fees will be collected automatically onto your card on file between the 1st-5th of each month. Once tuition has been charged you are responsible to pay for that month whether you attend classes or not. NO REFUNDS will be given for classes not attended. Makeup classes are optional. Tuition stays the same whether it is a 3, 4 or 5 week month. Fees are calculated on an annual basis and divided into 10 installments. Holidays have been accounted for so there will be no refunds for holidays. If you have not paid by the 10th of the month, you will be charged a $10 late fee.
THERE ARE NO TUITION REFUNDS! Inclement Weather missed days no refunds, may be made up. Late Installments are not acceptable. FOR the 10 INSTALLMENT PLAN- Tuition is due on THE FIRST OF EACH MONTH. After the 10th of the month there will be a $10 late fee for unpaid tuition. Students two months or more behind in tuition will not be permitted to take class.
Drop Out Policy
We must have written notice at least 2 weeks prior to the end of the intended drop out month or you may be charged.
Drop Class Form- Parents must notify the front desk if the student is dropping from a class.
IF AT ANY TIME YOU WISH TO HAVE YOUR CREDIT CARD TAKEN OFF FILE PLEASE CONTACT THE FRONT DESK OR IF YOU NEED IT CHANGED, YOU MAY DO SO BY GOING INTO YOUR ONLINE ACCOUNT AND MAKING THE CHANGES. PLEASE CONTACT THE FRONT DESK IF YOU NEED ASSISTANCE.
Class placement- Age and Level of the student
CLASS PLACEMENT for FALL CLASSES
WE HAVE THE RIGHT TO SWITCH STUDENTS OUT OF CLASSES THAT WE FEEL ARE NOT THE PROPER LEVEL AND/OR AGE GROUP.
New Students and Children under age 7- Most classes will be open online for you to reserve your spot.
Current Students- Placing students in the proper level class is very important. We recommend classes to each student. We are basically holding these classes for past students to enroll.

Attendance
Class Attendance: Missed classes may be made up in any group classes of the same level or lower. There are no refunds. Classes should be made up within the same month as the absence. Makeups are not mandatory.
If the studio closes due to inclement weather, you may take a makeup class. No refunds will be given.
There are no make-ups or refunds for missed classes.

I hereby understand and am fully aware that the student/participant named below may be participating in Mulford Studio activities in which I and/or the student/participant named below may be photographed or videotaped (the Property) from time to time. I hereby irrevocably grant to Mulford perpetually, exclusively and for all media throughout the world (including print, non-theatrical, home video, cd-rom, internet and any other electronic medium presently in existence or invented in the future), the right to use and incorporate (alone or together with other materials), in whole or in part, photographs, sound bites or video footage taken as a result of participation in Mulford activities. I hereby agree that I will not bring or consent to others bringing claim or action against Mulford, its owners, members, Board of Directors, and all employees and agents of these parties from and against any all claims, demands, actions, causes of actions, suits, costs, expenses, liabilities and damages whatsoever that I may hereafter have against Mulford in connection with the Property. This agreement shall not obligate Mulford to use the Property or to use any of the rights granted hereunder, or to prepare, produce, exhibit, distribute or exploit the Property. Mulford shall have the right to assign its rights hereunder, without my consent, in whole or in part, to any person, firm, corporation or organization.
Assumption of Risk
In consideration of participating in the Nancy Mulford Studio of Dance, Inc, I represent that I understand the nature of this activity and that I am qualified, in good health, and in proper physical condition to participate in such activity. And I, as the minor's parent and/or legal guardian, understand the nature of the above referenced activities and the minor's experience and capabilities and believe the minor to be qualified to participate in such activity. I acknowledge that if I believe event conditions to be unsafe, I will immediately discontinue participation in the activity. I fully understand that this activity involves risks of serious bodily injury, including permanent disability, paralysis, and death, which may be caused by my own actions, or inactions, those of others participating in the event, the conditions in which the event takes place, or the negligence of the "releasees" named above; and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume all such risks and all responsibility for losses, cost, and damages I incur as a result of my, or the minor child's, participation in the activity.
Unforseen viruses, coronaviruses, and any other illnesses that spread.
-I understand if my child is sick they will not come to class or shows.
-I will take my Child's and my own temperature within 24 hours of a class or performance. If the
temp is more than 99, I will keep my child and myself home.
-I understand I am willingly and voluntarily allowing my child to participate in an activity where
they are around other people, thus increasing their risk of contraction. I understand the risk
associated with allowing my child to participate in dance classes regarding COVID-19.
-I hereby release and discharge
Mulford Dance Studio
and employees from any liability whatsoever which may arise as a result of the student
and any of its owners, directors, officers
participating in events sponsored by Mulford Dance Studio.
Release of Liability
I hereby release, discharge, and covenant not to sue Nancy Mulford Studio of Dance, Inc, it's respective administrators, directors, agents, officers, volunteers, and employees, other participants, any sponsors, advertisers, and owners and leasers of premises on which the activity takes place, (each considered one of the "releasees" herein) from all liability, claims, demands, losses, or damages, on my account, or the minor's account, caused or alleged to be caused in whole or in part by the negligence of the "releasees" or otherwise, including negligent rescue operations and future agree that if, despite this release, waiver of liability, and assumption of risk I, or anyone on my behalf, or the minor, and anyone on the minor's behalf, makes a claim against any of the releasees, I will indemnify, save, and hold harmless each of the releasees from any loss, liability, damage, or cost, which any may incur as a result of such claim.

Payment/Refund Policies
Students are not considered registered until registration fee and 1st quarter payment is paid.
Dropping out policy-
Payments are not refundable. Registration and gym/tumble fees are not refundable.
Students dropping may opt to finish out for the remainder of the quarter or 8 week course they paid for. We must have written notice of dropping out- please use Drop form available in front office.
Medical Emergencies
As the parent legal guardian of the student/participant named above, I request and authorize that in my absence the student/participant named above be admitted to any hospital or medical facility for diagnosis and treatment. I request and authorize physicians, dentists, and staff, duly licensed as Doctors of Medicine /Osteopathy or Doctors of Dentistry or other such licensed technicians or nurses, to perform any diagnostic procedures, treatment procedures, operative procedures and x-ray treatment of the student/participant named above. I have not been given any guarantee as to the results of examination or treatment. I hereby authorize Mulford Studio, its owners, members, Board of Directors, and all employees and agents of these parties to act for the student/participant named above according to their best judgment in providing or arranging for emergency care in any emergency circumstance requiring medical attention. I authorize the hospital, medical or care facility to dispose of any specimen or tissue during the course of any diagnosis, treatment or other normal and customary procedures.
