I hereby indemnify, hold harmless, release, waive, discharge and covenant not to sue Short Dance Studios, Inc. including its directors, officers, agents, and employees, all for the purposes herein referred to as "Releasees," FROM ALL LIABILITY, TO THE UNDERSIGNED AND THE STUDENT, FOR ANY AND ALL LOSS OR DAMAGE, AND ANY CLAIMS OR DEMANDS THEREFORE ON ACCOUNT OF INJURY TO THE STUDENT OR PROPERTY OR WHILE IN THE ACT OF BEING TRANSPORTED TO AND FROM SAID ACTIVITY OR ACTIVITIES RELATED TO SHORT DANCE STUDIOS INC. OR COST they may incur ARISING OUT OF OR RELATED TO THE EVENT(S), WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE. I understand that by agreeing to this clause I am releasing claims and giving up substantial rights, including my right to sue.
Regular attendance is necessary for progress. Unnecessary absences are a hindrance to the student, as well as, to the teacher and classmates. Only due to contagious illness should a student miss class. If injured, the student should still come and observe class. Please call the studio and leave a message if your dancer needs to miss class. SHORT DANCE STUDIOS INC. RESERVES THE RIGHT TO DROP ANY STUDENT THAT HAS MISSED 3 CONSECUTIVE CLASSES OR MORE THAN 5 CLASSES TOTAL.
We use our experience to place all students in the proper class according to their age and anticipated ability. We strive to work with your schedule to find a class time that works for you.
Parents and friends are encouraged to visit class one lesson per month to see the students' progress. We have a two way mirror or T.V. monitor for viewing. Please do not abuse the viewing privileges. Once the student is comfortable in the class, it is easier for the teacher to keep their attention if there are NO distractions.
STUDENTS SHOULD ARRIVE FOR THEIR CLASS NO MORE THEN 15 MINUTES PRIOR TO CLASS AND MUST BE PICKED UP WITHIN 15 MINUTES OF THEIR ENDING CLASS TIME FOR THAT DAY. A $5 FEE WILL BE ASSESSED FOR EACH 15 MINUTE INCREMENT THAT THE STUDENT REMAINS BEYOND THAT TIME. (Ask office for more details.)
Required attire and materials must be at all classes. Check the bulletin board, website or ask desk staff and teacher to find out required attire/shoes/materials, etc... for each class. Failure to wear or bring required attire and materials to class could result in students being asked to sit out of the class. Students are not permitted to wear jeans or shoes worn on the street or outdoors, to any dance class. For safety reasons students are asked to not wear jewelry and long hair MUST BE tied back and out of the face.
COSTUMES for the dance program are ordered through the studio for the annual dance recital. All dancers will be measured in the fall and given the total cost of each costume. A costume deposit, of $40/dance in the recital, is due by Oct. 15th. The remainder of the costume balance for each dance in the recital will be due by Nov. 15th. Deposits and balance due dates may be different for higher level performing and competition dancers. The average amount of a costume is approximately $80. Once a costume is ordered for a student, the total amount due is non-refundable as it is non-refundable with the costume company for the studio.
NO GUM IS ALLOWED ANYWHERE IN THE FACILITY. Food and covered drinks must remain in the waiting area. PLEASE HELP KEEP OUR FACILITY CLEAN!
