AGREEMENT (SCHOOL YEAR PROGRAM)

TUITION AND FEES
An annual $25 Registration Fee is charged upon registering for class. $50 Registration Fee after Nov. 1 for year round program.
Tuition fees are pre-paid monthly through our secure AutoPay system. Tuition is due the 1st of every month for that month's lessons, however, Auto Pay is run 2 days prior to the 1st of each month. Ebills are sent 2 weeks prior to the due date giving families their current balance. Tuition is considered late if paid after the 10th of the month whereas a $10 fee will be added to your account.
ATTENDANCE AND MAKE UP CLASSES FOR SCHOOL TERM SESSION (AUG-JUNE)
I understand that although tuition is non-refundable, if a class is missed due to an illness, holiday or snow closing, students or parents must call in the absence to the office and schedule a make up class within 2 months of the missed class. Students and parents should make every effort to attend classes regularly. Any missed class in a school term must be made up before the end of that school term. Makeup classes are non transferable to different sessions or programs. A student must currently be registered in order to make up missed classes.
LONG TERM ABSENCE FOR FALL SESSION
If a student misses more than one month of classes, we will only charge 50% of tuition for that month. 50% of tuition will still be due for the missed month of classes to hold the student's spot the class(es). A parent will need to call in and speak to a staff person to inform the office and teacher about the long term absence.
LEVEL ADVANCEMENT FOR SCHOOL TERM SESSION (AUG-JUNE)
I understand that level advancement in ballet and dance in general does not happen annually. Children in the PreBallet Division most likely will advance each year, but once students graduate into the Ballet 1-9 Division, approximately 2 years of training is required in each many levels. Evaluations are mailed out in the spring and level placement letters for the next school term are mailed out in June.

RELEASE OF LIABILITY
I understand and agree that I will not hold Saint Louis Ballet School (SLBS), Mr. Gen Horiuchi, owner & director, or any faculty member liable for any injury sustained or any illness contracted by me or my children while enrolled as a student.
MEDICAL
I have provided all medical information in my child's online Saint Louis Ballet School account. I have provided emergency contact phone numbers for Saint Louis Ballet School to call me if there is an emergency.
WITHDRAWING FROM THE SCHOOL'S YEAR ROUND SESSION
We would hate to see you go but if you must, understand that the last day to withdraw is March 31, 2020. Withdrawing requires a 30 day notice by coming into the studio office and filling out an official withdrawal form. I understand that if I withdraw after March 31, then I am still responsible for tuition through the end of the school term (June ). If my withdrawal circumstance requires a refund a 5% transaction fee will be deducted from the refund total.
CONDUCT AND STUDENT HANDBOOK
I understand that I must show respect to all faculty, staff and peers in order to continue the positive environment that the school takes pride in. My child and I will read over the School Handbook available at stlouisballet.org and apply all policies to the best of my ability. STUDENT DANCE IMAGES FOR MARKETING PURPOSES ONLY
Saint Louis Ballet School will use student images for the sole purpose of marketing the studio in print ads, on its website, and social media. Student images will only be used for these business purposes are representations of Saint Louis Ballet School.
PRIVATE DANCE LESSONS
If a student has scheduled a private lesson and needs to cancel a scheduled private lesson time, a parent must email the school at least 24 hours in advance if the office is not available to call. If you do not let us know within 24 hours or do not show up, you will be charged for the full amount of the private lesson. To schedule a private lesson please contact the office for the "call-in" date to schedule.
MUSIC LESSON AGREEMENT
If a student must miss a lesson and it is known in advance a make up lesson can be scheduled and a credit can be issued to the
student's account, but only if we are notified via email 36 hours in advance by emailing school@stlouisballet.org or by calling the office.
If a student is sick and the absense is reported the same day as the lesson they cannot receive a credit for the missed class. However, each music student is given a maximum of 2 free make ups per year scheduled within one month of the missed class.
Music students must commit to their scheduled private time for the term of the school year.
Music Performances will be in November and May.
SUMMER SESSION: A non-refundable deposit of $50 is required once registration is received.
Balances are due no later than May 31st. All fees are non-refundable once paid in full. If injury prevents attendance, 50% tuition is refundable.
COVID IN-PERSON POLICIES
BEFORE ENTERING THE STUDIO
Anyone with any symptoms including but not limited to fever, shortness of breath, loss of taste or has been in contact or lives with someone diagnosed with Covid will not be allowed into the studio. Upon arrival, all students will have their temperatures checked by a staff member or volunteer. Students should only attend classes in the studio if they know they have not felt unwell and have not been in contact with a sick person. Only students will be permitted inside the building; family members must remain outside until their student's class is finished.
WHILE IN THE STUDIO
Students in Level 1 and higher will be required to wear a mask at all times while in the studio. We ask that students only bring a small bag with their essential items to be taken into the studio with them. Their bag should include personal hand sanitizer and a full water bottle. The water fountain will only be open to refill bottles. Restrooms will only be open for emergencies, so students should arrive already dressed for class.
ZOOM & IN-PERSON CLASS ROTATION
To further ensure social distancing, students will be assigned to a group, or a "house" as we call them, with which they will rotate between in-person and virtual classes via Zoom. All in-person classes will be released 5 minutes early to prevent traffic in the hallways and to allow our staff time to disinfect the studios.
COVID RELEASE OF LIABILITY
I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I maybe exposed to or infected by COVID-19 by attending classes in-person. The studio will follow CDC guidelines for safety using social distance, small class sizes, regular disinfecting, staggered dismissal, and mask wearing.
ZOOM POLICIES
There is to be no recording of classes on the participants' end, and it is forbidden to post any class on any social media platforms for student and teacher privacy reasons.
RELEASE OF VIRTUAL LIABILITY
I understand and agree that I will not hold Saint Louis Ballet School (SLBS), Mr. Gen Horiuchi, owner & director, or any faculty member liable for any injury sustained while taking virtual classes in or around my residence. I will do my best to provide my child a safe space to perform his or her virtual class via the zoom platform.
PRIVATE DANCE LESSONS

MUSIC LESSON AGREEMENT
ABSENCES AND MISSED LESSONS
Music students must email school@stlouisballet.org at least 36 hours in advance if they cannot make their scheduled private lesson time. This will allow us to have an official time stamp of when we were notified. If you need to call regarding your absence please make sure to leave the time of when you called in your message if you are not speaking to someone directly.
If a student calls at least 36 hours in advance we will issue a credit to the account. We will allow a maximum of two credits only for missed private lessons. If a student is absent and the school is notified less than 36 hours in advance, the student will not be given credit for that missed lesson.
Music students must commit to their scheduled private time for the term of the school year.
There will be a scheduled make up week for any missed lessons due to snow days or teacher conflicts. Dates will be announced at the start of the session.
SCHEDULED PERFORMANCES:
There will be 2 planned recitals each school term (Winter and Spring). Recital fee per performance is $15 per student.
TEACHER ASSISTANTS
We encourage levels 6 and up to inquire about becoming a teacher assistant. Teacher assistants gain experience on learning how to teach young children, receive 2 free tickets to every Saint Louis Ballet performance (5 performances a season), special recognition at the recitals, as well as volunteer credit for your schools. Please let us know if you are interested in becoming a teacher assistant.
