Consent To Electronic Signature
This authorization concerns consent to communicate electronically in the submission of forms and documents related to the enrollment of your student(s) at Pegge Lee School of Dance, Inc. dba Cabarrus Dance Academy. (Note: If you are enrolling multiple students, this consent is valid for each one.) Please review the consent below and indicate if you are willing to communicate electronically (send /receive EMAIL) on these matters by providing your electronic signature. If you consent to electronic submission and change your mind later, you may withdraw your consent for further electronic communications by submitting a written notice of your withdrawal to info@cabarrusdance.com or by mail at Cabarrus Dance Academy, 2810 Poplar Tent Road, Suite 100, Concord, NC 28027.
Your decision to decline or withdraw your consent to electronic communications (E-MAIL) will have no impact on your enrollment with Cabarrus Dance Academy. IF YOU DO NOT CONSENT TO ELECTRONIC COMMUNICATIONS YOU MUST ENROLL IN PERSON AT THE STUDIO. DO NOT CONTINUE!
I consent to communicate electronically concerning the enrollment of my student(s) at Cabarrus Dance Academy. I authorize Cabarrus Dance Academy to send me notices, disclosures, and forms electronically, and I agree that information and responses that I provide may be stored in electronic format. I further agree that an electronic signature by me on authorization or agreement shall serve the same purpose as a written signature.
Note: If you have elected NOT to consent to electronic communications (EMAIL) concerning the enrollment of your student(s), please contact Cabarrus Dance Academy directly to enroll. Do not complete this online registration form.
Dress Code
Cabarrus Dance Academy adheres to a specific dress code for all students so that emphasis is placed on instruction and not attire. Please refer to the Student Parent Handbook for Dress Code Requirements in each dance discipline.
Camps/Workshops/ Intensives
Camp, Workshop and Intensive fees must be paid in FULL upon enrolling. There is a 25% cancellation charge on these fees. There are no refunds for cancellations within 10 days of the event. Any camp, workshop, or intensive that does not meet the minimum enrollment requirement will be cancelled and full refund will be given. Build Your Own Classes and Private Lessons must be arranged by phone call and paid in advance.
Limited Term Classes 2021-22
Tuition for LlMITED TERM CLASSES are payable in full for each session. When enrolling in a Limited Term Class, the Tuition Fee for the entire term is charged upon enrollment. Cancellations incur a 25% fee. There is no refund for a cancellation within 10 days of the beginning of the limited term session.
School Term Tuition 2021-22
Tuition is based on the entire dance season, not by the individual month or class. There are no deductions for missed lessons. Tuition is the same regardless of the number of lessons in the month. Tuition is due the 1st of each month. An additional fee $10 will be automatically applied to any account not paid by the 10th of the month. Tuition fees are outlined in the Student Parent Handbook. Dance Families may pay in person, enroll in AUTODRAFT, or use the Customer Portal for payments. We accept Cash, Checks, and Credit Cards for payment of fees. For the 2021-22 School Term, the first payment is due September 1, 2021 with the final payment being due May 1, 2022. Families consenting to the electronic communication, enrolling in Autodraft (E-Payments) with a CREDIT CARD will receive 2% off monthly tuition fees. When a family enrolls in Autodraft (E-Payments) with BANK draft, they receive 5% off fees for the 2020-21 school term. The autodraft will occur on the 5th of each month.
Release of Liability
As the legal parent or guardian, myself, my spouse, and our respective heirs release and hold harmless Pegge Lee School of Dance Inc. dba Cabarrus Dance Academy, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Cabarrus Dance Academy, its owners and operators or in route to or from any of said premises. I further acknowledge and understand, appreciate and agree that my participation may result in possible exposure to illness from infectious diseases including but not limited to, MRSA, Influenza, and Covid-19. While personal discipline may reduce this risk, the risk of serious illness and death does exist. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releases or others and assume full responsibility for my participation and exposure.
In the event that the student is not a minor, I warrant and agree that by signing below, I do hereby consent to the terms and conditions stated above and release on by own behalf as the student.
I understand that it is my responsibility to consult with a physician prior to the student's participation to determine if the student is physically fit and does not have any medical condition that would prevent his/her full participation in Cabarrus Dance Academy programs. I agree to keep the student's instructor informed of any health condition that could be affected by participation. If a student is out for an injury, I must provide Cabarrus Dance written confirmation from a health professional that he/she may return to participation. I acknowledge that Cabarrus Dance Academy will not render any medical services to the student.
Medical Emergency
The undersigned gives permission to Pegge Lee School of Dance, Inc. dba Cabarrus Dance Academy, its owners and operators, staff, volunteers, to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, resrtictions, or condition and/or declare the paricipant to be in good physical and mental health.
Handbook
I am responsible for abiding by the information contained in the Student Parent Handbook which is available online. The Student/Parent Handbook outlines Policy, Tuition Fees, Dress Code, Dance Calendar, Performance, the measures we will use to keep our dancers in a safe environment.
Dance Recital
All students (except Wiggle & Whirl) are invited but not required to perform in the spring recital. Each participating student will be responsible for paying a recital fee to defray the cost of auditorium rental, and purchase a costume for each class he/she takes. (Pointe & Sacred students do not purchase an extra costume). COSTUMES MUST BE PAID IN FULL PRIOR TO ORDERING. COSTUME AND RECITAL FEES ARE NOT REFUNDABLE AFTER DECEMBER 1. Families who are on the AUTODRAFT will have these fees drafted November 19. The Recital Awards are given to students who complete 5, 10, and 15 years at our school.
Costume pricing: Pre-Dance & Pre-Ballet Costumes $59, Elementary Costumes $69, Combo $72, Intermediate $73, Lee-ettes $80
Parents can elect to make monthly installments on costumes beginning in September.
Due November 19: Recital fee $45, Additional Child - $15 FULL AMOUNT for Costumes
Please notify our office BEFORE NOVEMBER 19 if your child will NOT be participating in the spring recital.
Marketing Release
I give Cabarrus Dance Academy, its employees and representatives permission to take photos or video of my child in connection with activities at the Academy, and give authorization to use and publish such media with or without name for any lawful purpose including publicity, advertising, and social media. To OPT OUT of Marketing Release - please reply "NO PHOTOS" in the comments/concerns section below.
Enrollment Fee
I give permission for Cabarrus Dance Academy to charge my Credit Card or Auto Debit the FULL AMOUNT for any SUMMER CAMP, WORKSHOP, INTENSIVE, OR PRIVATE LESSON. No refunds within 10 days of program start.
I give permission for Cabarrus Dance Academy to charge my Credit Card or Auto Debit the $30 REGISTRATION FEE PER STUDENT to complete the Registration for students enrolling for the 9 month 2021-22 Dance Season. I UNDERSTAND THE REGISTRATION FEE IS NOT REFUNDABLE.
Consent to Pay Fees
I authorize Pegge Lee School of Dance, Inc. dba Cabarrus Dance Academy to debit my account on the 5th of each month for the full Tuition Fee in Nine Monthly Installments beginning September 5, 2021 with the final payment May 5, 2022. The tuition fees are outlined in the Student Parent Handbook. Charges that are declined will be assessed a $10 Late Fee in addition to a $25 NSF Fee. I will give the studio office WRITTEN notice 30 days in advance to discontinue these charges. I understand FULL PAYMENT FOR COSTUMES and RECITAL FEES will be AUTODRAFTED on NOVEMBER 19, 2021. Audition fees, Christmas Ballet Fees or other incidental fees must be paid separately and will not be automatically drafted. Incidental Fees may be paid in person or by using the Customer Portal.
When enrolling in a Camp, Workshop, Intensive, or Limited Term Class, I authorize Pegge Lee School of Dance, Inc. dba Cabarrus Dance Academy to debit my account for the FULL AMOUNT upon registering.
To OPT OUT of the E-Payment (AUTODRAFT) System- you must reply "NO AUTODRAFT" in the Comments below - however, you MUST PROVIDE A CREDIT CARD TO ENROLL ONLINE and consent to pay Tuition and Fees as outlined in our Handbook. To pay by check or cash, please register in person.
