Payment Terms
I understand that payment is required for registration. Upon registration, my credit card or bank account will be charged tuition(s) plus the registration fee of $40.00 per student. When my credit card is charged, an emailed statement, explaining all charges, will be sent to the email I provide. I understand that tuition is due on the first day of each month and will be charged automatically to my credit card on file. Any account balance that is not paid by the 10th of each month, will be assessed a $15 late fee, regardless of the amount due. I also understand that if payment is not received by the 20th of the month, that my child will be dropped from classes and will not be allowed to attend class until the balance is paid in full. If my child is dropped for non payment, I understand that I have forfeited a spot in his or her regularly scheduled class, and that upon re-enrollment that they may have to attend class at a different day or time. Once tuition is paid no refunds are available.
Membership Terms
I agree that in order to cancel our membership that written notification must be sent to Elite Xtreme at least 5 days prior to the 1st day of the month. Notification must be emailed to info@owassoelite.com. I understand that verbal notification cannot be accepted. I understand that there are no refunds if cancellation policy is not followed. I understand that there are no refunds for membership fees. If any member remains inactive for longer than 6 months, a new registration fee of $40 will be applied upon reenrollment.
Release of Liability
• I recognize that severe injuries, including permanent paralysis or death can occur in sports or activities involving height or motion, those activities may include but are not limited to gymnastics, tumbling, and trampoline. In addition, being fully aware of these dangers, I hereby give consent for my child(ren) to participate in all Elite Xtreme Gymnastics and Tumbling programs and activities and I ACCEPT AND ASSUME ALL RISKS associated with this participation.
• I agree that parents / siblings / guardians and any other persons are NOT allowed on the floor or equipment at any time.
• In consideration for my child(ren)'s participation I hereby, for myself and on behalf of my child(ren) and our respective heirs and successors, COVENANT NOT TO SUE and forever release and hold harmless Elite Xtreme Gymnastics and Tumbling, it's owners, employees, contractors and volunteers from and against any and all claims, injuries, damages, losses, or liabilities (here in after "claims") arising from or related to the participation in activities including claims of negligence.
As the legal parent or guardian, I release and hold harmless Elite XTreme Gymnastics & Tumbling LLC, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Elite Xtreme Gymnastics & Tumbling LLC, its owners and operators or in route to or from any of said premises.
Medical Emergency
In any event of an accident or emergency I hereby authorize my child(ren) to be transported to a hospital or medical facility for medical treatment and I hold Elite Xtreme Gymnastics and Tumbling and it's representatives harmless in the execution of such. Additionally, I hereby agree to individually provide for all medical expenses which may be incurred by me or my child(ren) as a result of any injury sustained while participating at or for Elite Xtreme Gymnastics and Tumbling. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health. I understand that my child will be transported to the nearest hospital.
Photography
Occasionally pictures in the facility or at away events and competitions are taken for promotional purposes. I give my permission to allow Elite Xtreme Gymnastics & Tumbling, LLC, to use pictures that my be taken of my child(ren) in any flier, brochure, publication or website.
