At the time of registration, GAA will charge your credit card on file for June tuition and any past due balances. We will NOT be charging any registration fees at this time. If you have any questions about your balance, please contact the Front Desk (770-975-8337).
If you would like GAA to continue to charge your credit card each month, please pick a date to be charged under the "epayment schedule" tab. We will not continue to charge your credit card unless you grant us this permission. Autobill will start in July.
Classes start Monday, June 1st and the session concludes on Saturday, July 25th. We will hold your space in a class and charge your account until a 2 week notice is provided to the Front Desk (NOT THE COACH). We will be closed on Saturday, July 4th for Independence Day. Saturday students may do a make up in another class of the same level.
All participants should come dressed in a leotard or shorts and t-shirt (no buttons or zippers), no jewelery, hair up and barefoot. If you have any other questions, please do not hesitate to contact us (770-975-8337) and thank you for choosing GAA.
In consideration of participating in any event or program at The Gymnastics Academy of Atlanta I acknowledge and represent that I fully understand the nature and scope of the Activity involved in such participation and that I am qualified, in good health, and in proper physical condition to participate in such Activity. I acknowledge that if I believe event conditions are unsafe, I will immediately discontinue participation in the Activity. I fully understand that this Activity involves risks of serious bodily injury, including, but not limited to, permanent disability, paralysis and death, which may be caused by my own actions or inactions, the actions or inactions of others participating in the event, the conditions in which the event takes place, or the negligence of the "Releasees" named below; and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume all such risks and all responsibility for any and all losses, liabilities, cost and damages that I may incur as a result of my participation in the Activity.
I hereby release, discharge, and covenant not to sue The Gymnastics Academy of Atlanta, its respective administrators, directors, agents, officers, volunteers and employees, other participants, any sponsors, advertisers, and, if applicable, owners, and lessors of premises on which the Activity takes place (each considered one of the "RELEASEES" herein), from any and all liabilities, claims, demands, losses, or damages, on my account caused or alleged to be caused, in whole or in part, by the negligence of the "Releasees" or otherwise, including negligent rescue operations, and further agree that if, despite this release, waiver of liability, and assumption of risk I, or anyone on my behalf, makes a claim against any of the Releasees, I will indemnify, save, and hold harmless each of the Releasees from any and all loss, liability, damage, or cost, which they may incur as the result of any such claim.
I have read the RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT, understand that I have given up substantial rights by signing it and have signed it knowingly, voluntarily and without any inducement or assurance of any nature and intend it to be a complete and unconditional release of all liability to the greatest extent allowed by law and agree that if any portion of this agreement is held to be invalid the balance, notwithstanding, shall continue in full force and effect.
AND I, the Minor's parent and/or legal guardian, understand the nature of the above-referenced Activity, and the Minor's experience and capabilities and believe the Minor to be qualified to participate in such Activity. I hereby release, discharge, covenant not to sue, and AGREE TO INDEMNIFY AND SAVE AND HOLD HARMLESS each of the Releasees from any and all liabilities, claims, demands, losses or damages on the Minor's account caused or alleged to have been caused, in whole or in part, by the negligence of the Releasees or otherwise, including negligent rescue operations, and further agree that if, despite this release, I, the Minor, or anyone on the Minor's behalf makes a claim against any of the above Releasees, I WILL INDEMNIFY, SAVE AND HOLD HARMLESS each of the Releasees from any and all litigation expenses, attorney fees, losses, liabilities, damages or costs that any Releasee may incur as the result of any such claim.
Covid-19, Coronavirus, SARS-CoV-2 Warranty of Guardian and Visitor
Responsibility and Waiver of Liability
As the legal guardian of my student(s), I hereby acknowledge the Covid-19,
Coronavirus, SARS-CoV-2 pandemic and any mutation thereof ("hereinafter "Covid-19"). I understand that the virus is highly contagious and that individuals carrying the virus may exhibit no symptoms, making detection of carriers impossible. I hereby
consent to my student(s) participating in this program at The Gymnastics Academy of Atlanta at its facility at 2995 Cobb Pkwy NW, Kennesaw, GA 30152 ("Facility"). I further consent to my visiting the Facility. If someone else visits the facility, that person must also sign this Waiver. I further accept my responsibility to guard against the spread of Covid-19 at the Facility and I WAIVE, COVENANT NOT TO SUE and FOREVER RELEASE, The Gymnastics Academy of Atlanta affiliated and partner companies and organizations, property owners and lessors of 2995 Cobb Pkwy NW, and the staff, contractors, subcontractors, teachers and coaches of GAA and its affiliates involved in this facility program, from all liability and for any and all damages and injuries suffered by me or my student as a result of contracting Covid-19. Further, I hereby warrant and represent on behalf of myself and my student
and anyone accompanying my student to the Facility the following:
1. Only my student and one other person will enter the Facility during participation in a Facility program.
2. The person accompanying my student will stay in the lobby area behind the wall and will not pass beyond the wall or in the parent viewing area upstairs.
3. All persons associated with my student will maintain at least 6' distance from other persons while at the Facility.
4. The person accompanying my student will wear a facial mask covering the nose and mouth at all times while at the Facility, including the parking area.
5. Neither my student, nor the person accompanying my student, nor I have any of the following symptoms when entering the facility
a. A fever above 99 degrees Fahrenheit
c. Sore throat
d. Shortness of breath
e. Any other symptom associated with Covid-19
6. Neither my student, nor the person accompanying my student, nor I, nor anyone residing with any of us have traveled outside of the United States in the past thirty (30) days;
7. Neither my student, nor the person accompanying my student, nor I have come into close contact with anyone confirmed of having the following in the past fifteen (15) days:
a. Covid 19 Virus
b. Influenza (any strain).
I warrant and represent that if my student or the person accompanying my student or I meet any of the above listed factors, we will notify GAA and not visit the Facility for at least fourteen (14) days after the latest date upon which one of the above factors was confirmed.
A concussion is a brain injury that results in a temporary disruption of normal brain function. A concussion occurs when the brain is violently rocked back and forth or twisted inside the skull as a result of a blow to the head or body. Continued participation in any sport following a concussion can lead to worsening concussion symptoms, as well as increased risk for further injury to the brain, and even death. Concussion symptoms include: headache, dizziness, poor balance, moves clumsily, reduced energy levels, nausea/vomiting, blurred vision, sensitivity to light and sounds, fogginess of memory, difficulty concentrating, slowed thought processes, confusion about surroundings, unexplained changes in behavior/personality and loss of consciousness. ANY ATHLETE SUSPECTED OF SUSTAINING A CONCUSSION IS NOT ALLOWED TO RETURN TO PRACTICE/COMPETITION ON THE SAME DAY OF DIAGNOSIS AND MUST BE CLEARED TO RETURN BY AN APPROPRIATE HEALTH CARE PROFESSIONAL TO PARTICIPATE IN FUTURE ACTIVITIES.
