Tuition Policies
Accounts will be drafted on the 25th of each month for the following months tuition. If the 25th falls on a Saturday or Holiday, tuition will be drafted the business day prior to the 25th. If you choose not to auto draft, tuition is due by the 1st of each month. Statements are e-mailed on or before the 20th of each month. If you choose to pay after the 1st, there will be a $15.00 late fee charged to your account. If payment is not received by the 2nd week of the month, your child will be dropped from the class. Checks may be payable to Maximum. Credit card payments may be made online for free, or over the phone with a $2.00 processing fee.
Family Discount: Beginning with the second child enrolled, you will receive a $10.00 discount per child.
Annual Registration Fee: All members of Maximum Athletics, LLC are required to pay an Annual Registration Fee. This fee is non-refundable. Fees are prorated throughout the year according to the month you enroll.
Monthly payments will be drafted from the credit card or debit card of your choice. We will automatically draft your account each month on the 25th of the month for next months tuition. If you decide to leave Maximum Athletics, LLC please give us written notice by the 15th of the month so we can stop the auto draft for the following month.
If you choose not to use our auto draft option, a security deposit will be required at registration per child. This deposit will be held in your account and used as your last months' tuition if you should ever decide to leave the gym. You MUST give a 30 day written notice in order to use the deposit. Otherwise, it will be forfeited.
Privacy Policy of payment information. All credit card/bank draft information is kept in a locked secure place, and or stored securely online. At the end of each year, all payment information is shredded and destroyed. Maximum Athletics, LLC is the only company to use the information for it's intended purpose. It will never be shared with other parties.
Declined Drafts: If your draft is declined, you will receive a $35.00 decline charge.
Make-ups
When you register your child for a class, you are paying for your child's place in that class, not their attendance. We do not promise or owe make-up classes or credit for missed classes. However, we understand that children get ill; therefore we will offer your child one make-up class during the month that your child was absent. One make-up class may be carried over into the next month. Make-up classes must be scheduled through the front office and will be granted upon availability in a class. All make-ups must be completed while the child is still enrolled.
Holidays: There will be no prorating for holidays. I understand tuition is based on an annual session of August through July. Throughout the annual session, 48 weeks of classes will be offered. The number of classes in each month may vary. Therefore, the monthly tuition is a flat charge and full tuition is due regardless of the number of classes offered that month.
Rules and Policies
Students should arrive 10 minutes prior to class. This should give them ample time to get dressed and report for warm-up.
Students are not permitted on the equipment without an instructor. Please stay out of the workout area until your class starts. Gum chewing is not allowed, as it can be dangerous during upside down activities.
Parent Conduct: Parents are NOT permitted into the gym work out area unless participating with a Parent and Me class. Instructors may invite parents to accompany their child out on the floor or sit nearby. This is ONLY at the discretion of the instructor or other gym staff. Do not distract your child or other members of the class by calling or waving while the class is in progress. Every students attention should be with the instructor. This is for safety as well as courtesy. There is a time and a place for parental praise after class. You are welcome to observe all classes from the observation area.
Other General Policies
We will make every effort to provide a consistent coach for your class on an ongoing basis; however, we reserve the right to change instructors for classes at any time. The use of inappropriate language by children or adults at the gym will not be tolerated. Adults and children, whether in the program or on site as spectators, are expected to be respectful of staff, coaches, students, and others on the premises.
Rules and policies are subject to change and will be posted when they are made. It is a member's responsibility to read and understand these updates as they become available.
No food or drink is allowed in the gym area. They MUST be left in the lobby or seating area. Please label your child's name on the drink. This rule will be strictly adhered to.
Waiver and Release of Liability
MAXIMUM ATHLETICS, LLC IS NOT RESPONSIBLE FOR ANY LIABILITY IN THE EVENT OF INJURY OR ACCIDENT. Gymnastics and Tumbling involve rotation and height. These sports pose the risk of catastrophic injury, including the potential for permanent paralysis and even death. While safety rules, equipment, discipline, and precautionary measures may reduce these risks, the potential for serious injury or even death still exists. I knowingly and freely accept and assume such risks, both known and unknown. My child's participation is strictly voluntary. My child and I are fully aware of the potential for injury.
Assumption of Risk
I, on behalf of my child, myself, my heirs, representatives, and assigns, hereby release and hold harmless Maximum Athletics, LLC (hereafter referred to as MAC), it's owners, officers, agents, employees, volunteers, or participants, sponsoring agencies and advertisers from any and all claims for injury, disability, death, loss or damage to person or property, whether arising from inherent risks or from ordinary negligence of the releasees mentioned herein or otherwise.
Permission
I give permission for Maximum Athletics, LLC personnel to administer first aid in case of minor injury, and in case of severe injury until trained medical personnel arrive. I authorize Maximum Athletics, LLC personnel to authorize emergency medical treatment for my child if qualified medical personnel consider treatment necessary. This authorization is granted only if a parent or guardian cannot be reached and a reasonable effort has been made to do so.
I give permission for Maximum Athletics, LLC personnel to administer first aid in case of minor injury, and in case of severe injury until trained medical personnel arrive. My child is in good physical health. My child has no disease or injury, nor is my child taking any medication which would interfere with full participation in gymnastics/tumbling or related activities. I will inform Maximum Athletics, LLC in writing of changes in any information contained herein.
I will keep informed of Maximum Athletics, LLC rules and regulations or changes thereto by reading all notices posted on the gym bulletin board, handed out, or emailed.
I give Maximum Athletics, LLC permission to use my child's photo online and in advertising. (No names will be used).
Written Drop Notice
I understand that written notice is required to drop from the program. If I submit written notice by the 15th of the month, I will be able to withdraw my child from the program effective the following month with no additional charges due. I am responsible for continued payment of tuition until Maximum Athletics, LLC receives written notice, since I am holding a class spot. ALL FEES ARE NON-REFUNDABLE.
Release of Liability
I have read this waiver and release of liability, fully understand the potential for injury or death and have discussed this with my child in age-appropriate language. My child and I accept risks involved. Having accepted the risks and being fully aware of the potiential for serious injury I give permission for my child to participate in gymnastics/tumbling at Maximum Athletics, LLC. In consideration for participation in gymnastics/tumbling at Maximum Athletics, LLC I agree not to sue and to hold harmless Maximum Athletics, LLC and all other releases mentioned herein for any damage to person or property, injury or death resulting from all inherent risk or from ordinary negligence for now and forever in the future. If any part of this agreement is held void, such will not affect with the remainder of the agreement.
Drop Off and Siblings
Students MAY NOT be dropped off any earlier than 10 minutes prior to their class and picked up no later than 10 minutes after their class. Students are also not permitted to wait outside after class for their ride.
Siblings are welcome at Maximum WITH parental supervision. Siblings MAY NOT be dropped off with a student that has a class. We cannot and are not equipped to monitor siblings of our students. If it is reported that a sibling has been dropped off, we will have the student taken from class to supervise their brother or sister until the parent returns.
