Assumtion of Risk and Liability Release
While gymnastics is exciting and challenging, participation involves certain risks. Even under the best conditions, accidents can occur. Most result in minor sprains or contusions. Although the incidence is extremely rare, some accidents result in serious or permanent injury. The directors and teachers of Damar Gymnastics, Inc. and Spirit of Kentucky All Stars are dedicated to providing the safest possible teaching environment. Parents and students must appreciate, however, that no amount of matting, pit use, or excellent teaching can guarantee an injury-free program. I have read, undertand, and consent to my child's participation in Damar Gymnastics, Inc. and Spirit of Kentucky All Stars. In addition, I release Damar Gymnastics Inc./ Spirit of Kentucky All Stars, its directors and staff, from liability for failure to warn that injury may occur as a result of my child's participation in the aforementioned gymnastics program.
I hereby for myself, my children adopted or otherwise, my heirs and executors waive and release any and all rights against Damar Gymnastcs, INC and/ or Spirit of Kentucky All Stars, their agents or representatives, for any injury or damages that may be suffered by me, my children adopted or otherwise, in connection with my association or entry in gymnastics, cheerleading, or other activities sponsored by Spirit of Kentucky or Damar Gymnastics, INC.
Authorization of Medical Care
In case of illness or injury (if a parent or emergency contact cannot be reached.) the Staff of Damar Gymnastics, Inc. and Spirit of Kentucky All Stars may authorize medical care and treatment for my child(s).
Permission to publish photos and videos
I give my permission to Spirit of Kentucky and Damar Gymnastics, INC to copyright and re-use, publish, and republish photographs or video of my child(ren) or myself or pictures in which my child(ren) and/or myself may be included, in whole or in part, separately or in conjunction with other photographs, in any medium now or hereafter known, and for any purpose whatsoever, including but not limited to illustration, art, promotion, or advertising, that could expose a recognizable member of my family to the public.
Payment Policy
I understand that my child has enrolled in Gymnastics and/or All-Star Cheer. I am responsible for all contractual obligations, and and payments for services or products that pertain to my child's enrollment and will be subject to fees associated in collecting any past due balances if those occur. I understand that the Insurance/Registration Fee is due each August ($25.00, pro-rated for new students). We do not close for KHSAA Dead Period nor do we close for Fall Break. Anytime the gym will be closed it will be posted at Damar and/or Damar's Facebook page and website.
I understand that class and/ or tuition fees are due the first class of each month or my child may lose their spot or be required to sit out in class if payment is not received, I understand that a $10 late fee will be applied to my account after the 10th of each month.
I understand the I am responsible for all past due balances and auto draft may be used to collect those past due fees.

DISCONTINUING YOUR CHILD'S ENROLLMENT
30 DAY WRITTEN NOTICE!
If you wish to drop your child from the program, you must notify us in writing, 30 days prior to the next month's billing cycle. A "Notice Of Withdraw" can be obtained at the front desk. This is the ONLY way to discontinue your enrollment that will be recognized by our system, as this gives us a HARD COPY to give to our accounting department.

NOTE: Having your child notify their instructor or emailing the gym will NOT get to the proper personnel to officially drop your child from the program and/or stop your billing. If a student stops coming to class without notification then that student's account will be charged for the additional 30 days. This charge will be for holding the student's place in that class instead of offering that place to one of the many on a waiting list.

General Gym Policies
WHAT TO WEAR
Boys or Girls may wear T-shirts and shorts OR Leotards for Girls. NO chewing gum or dangling jewelry. Hair should be pulled neatly and securely away from the face so that is stays up for the entire workout. Personal items should be left in cubby holes. Jewelry should not be worn during classes. Damar/Spirit of Kentucky will not be responsible for ANY items that may be lost or stolen. Be sure your student's personal items are marked with their name.
ARRIVAL AND PICKUP
Be sure your student arrives 5 minutes before (no earlier please) his/her scheduled class time. Please pick up your student on time. Please inform us if you know you will be late picking up your student. Instruct your student to wait inside the building and you should escort them from the building to your car. During peak times the parking lot is crowded. Please take into consideration that our students may include young children. Please drive slowly and carefully. Do not take a chance on your student running to and from your car.
MAKEUP POLICY
If your child misses his or her class, you may schedule a make-up class. We will do our best to arrange a make-up that fits your schedule, however we cannot guarantee availability. Make-up classes must be scheduled within two weeks of your missed class. If we cannot find another class that fits your schedule, you can get an OPEN GYM pass (at the front desk) to use instead.
Enrollment
After registering for a class you DO NOT need to wait to hear from us. Once you select a class and click submit you are enrolled, class fee is posted, and your ready to attend the next day and time for your class. If your first class is on a holiday, please call the gym to see if we are open that day.
