Risk and Waiver of Liability
As the legal parent or guardian, yes, my daughter/son has my permission to attend classes/activities at B&T Gym, LLC dba Missouri Elite Gymnastics Academy (MEGA). In addition, my child/ward and I are participating in gymnastic, cheer, tumbling or other classes/activities under the terms and conditions set out below.
To the extent permissible by law, I/we hereby release, discharge and/or otherwise hold harmless and indemnify MEGA, it's owners, officers, directors, employees and associated personnel, from and against any and all demands, claims and causes of action arising, directly or indirectly, from my or my child's/ward's participation in its programs. THIS RELEASE SPECIFICALLY INCLUDES ANY DEMANDS, CLAIMS AND CAUSES OF ACTION ARISING OUT OF THE PAST OR FUTURE NEGLIGENT ACTS AND/OR OMISSIONS OF MEGA, ITS OWNERS, OFFICERS, DIRECTORS, EMPLOYEES AND ASSOCIATED PERSONNEL.
Medical Emergency
I give permission for MEGA owners, officers, employees, and/or agents to seek emergency medical treatment for the participant(s) in the event they are unable to reach any parent or guardian. The undersigned also agrees that they themselves will be responsible for any financial debt incurred by said action.
Marketing Releases
Occasionally MEGA uses photos or video of its students in print ads, on its website, or other marketing mediums. I understand that my child's likeness may be used in such advertising. These images will be used for MEGA purposes only and will not be given or sold to outside companies or individuals.
Exposure
I acknowledge, understand, appreciate and agree that participation may result in possible exposure to and illness from infectious diseases, including but not limited to MRSA, Influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist. I knowingly and freely assume all such risk, both known and unknown, even if arising from the negligence of the releases or others, and assume full responsibility for participation in the activity.
Parent/Guardian Signature
I have read and understood the above RISK AND WAIVER OF LIABILITY and MEDICAL AUTHORIZATION and MARKETING RELEASE. In addition, I confirm that I am the parent/legal guardian of this registrant(s).
Agreement of Policy
TUITION
Session tuition is due by the 1st of each month. If tuition is paid on the 2nd or later, there will be a $10.00 late fee applied to the account balance. For non-team members, Early Bird tuition is due by the 25th of the preceding month.
Monthly tuition is due by the first of each month. If accounts are paid after the first of the month, there will be a $10.00 late fee applied to the account balance. There will be a $1/day late fee applied each day after the 10th. There is a $25.00 returned check charge for any checks returned by the bank.
ENROLLMENT FEE
The enrollment fee is paid when you initially enroll and every year on the anniversary month of your original enrollment. The fee covers administrative cost and student/athlete secondary insurance.
ONLINE POLICIES
You need to agree to the policies online in order for your child to participate at MEGA.
EMAIL ADDRESS
It is important that we have your current email address and that you are opening/reading our emails. As our company becomes more automated and green conscience, emailing is becoming our preferred form of communication. Your email address will allow us to inform you of important billing information, facility closings, upcoming events, special activities, and specials.
PRO-RATE
Student registration is accepted at anytime and tuition will be calculated according to the number of weeks remaining for the session/month.
MAKE-UP CLASSES
Classes missed due to an illness must call by 8pm the day of your child's absence to schedule a make-up class that needs to be attended by the end of the following month as long as the child is still an active student. You may also receive an Open Gym pass instead of a make-up. Stop at the front desk to receive a pass when attending a make-up. If you need to cancel a previously scheduled make-up class, you must call by 8pm on the day of the make-up or you forfeit your make-up class.There are no refunds, discounts, or prorates for absences.
DROPPING CLASSES is done by letting the front desk know by phone, e-mail, or in person. You will be billed (not charged) unless we are otherwise notified but your spot in class will not be reserved without payment.
INCLEMENT WEATHER
In the event of inclement weather, please call or check social media to find out if classes are canceled. Daytime classes will be cancelled if Raymore-Peculiar schools are cancelled. Information regarding evening classes will be available by 2:30 PM.
